Payment Information
Tuition is due according to the fee schedule. If accounts are paid more than 10 days late, there will be a $20.00 late fee applied to the account balance. There is a $30.00 fee for any checks returned by the bank or any declined auto-debit credit card payments.
If I wish to make full payment up front, I can choose "one payment" under "membership type," and include my credit card or bank information under the "billing" tab.
If I wish to make monthly payments, I can choose "installments" under "membership type" and include my credit card or bank information under the "billing" tab.
The School will review balances to ensure payment of registration fees and any prorated amounts that may be due before processing payment. Payments are generally processed within 48 hours of registration.
I can also make payment via check either at the office or through the mail.
I also understand that notifying the School of my intent to enroll, either verbally or via email, when combined with class attendance, is a binding registration which authorizes the School to charge my account.
Length of Enrollment
All enrollments are for the full term of the session. Parents are responsible for the full tuition for the session in which they enroll their child.
Academic year dance enrollments are September through May; summer sessions are six weeks long. Samson Academy enrollments are by the semester.
Under certain circumstances, the School may allow a student to enroll for a partial session. Permission to enroll for a partial session must be obtained from the School.
Make-up Classes
In cases of illness or similar circumstances, students may make up missed classes in classes of the same level or lower on a space available basis. Parents must schedule a make up class with the office and take a copy of the email confirmation with them to the make up class to give to the teacher. Students are limited to one make-up class per semester. There are no makeup classes for camp days.
Withdrawal from Classes
For academic year withdrawals once dance classes have started:
In cases of serious illness or similar circumstances, or where the School deems it in the best interest of the child to withdraw from class, all unused tuition will be promptly refunded, minus any credit card fee.
For other withdrawals once classes have started, we require one month's notice. Except for serious illness or similar circumstances, no withdrawals are allowed after November 15. Parents will continue to be responsible for tuition regardless of whether student attends class.
For students who withdraw prior to the start of classes: There will be a $35 processing fee. For students who withdraw after the start of classes there is a processing fee equivalent to one month's tuition.
In all cases, registration fees are NON-REFUNDABLE..
For summer classes or camps:
When the School deems it in the best interest of the child to withdraw from class, all unused tuition will be refunded, minus any credit card fee. In cases of serious illness or similar circumstances, tuition may be refunded if credit toward future classes is impractical. In general, there are NO REFUNDS for summer programs.
In all cases, withdrawal must be IN WRITING via email.
For Samson Academy enrollments: except extraordinary circumstances there are NO WITHDRAWALS and NO REFUNDS.
Locations
The Claire School of Dance has two locations: 1703 Heights Blvd, known as our Lambert Hall location, and 1245 Heights Blvd, known as our Grace United location. All enrollment confirmations will contain a class title which includes the location. Class location may also be verified by logging into individual accounts. Release of Liability
As the legal parent or guardian, I release and hold Claire School of Dance its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any illness, loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned or other family members, while in or upon the premises or any premises under the control and supervision of Claire School of Dance, its owners and operators or in route to or from any of said premises.
Opt In Text Messages
In order to receive notice of class cancellations in a timely fashion, I hereby give permission for the Claire School of Dance to send text messages regarding last minute cancellations to my cell phone. I understand that the ONLY texts sent by the Claire School of Dance are regarding class cancellations.
Medical Emergency
The undersigned gives permission to the Claire School of Dance, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
Transportation Release
As legal parent or guardian, I give my permission to the Claire School of Dance to transport my child as may be necessary. I further absolve and indemnify the Claire School of Dance from any and all responsibility related to this travel.
Photo Release
As legal parent or guardian, I understand that the Claire School of Dance may from time to time photograph or videotape classrooms, theatrical performances, instructors, and participants. Acceptance of this enrollment authorizes the Claire School of Dance to use these photographs in its official publications, advertisements, and website, or for other promotional or commercial purposes.
