PLEASE READ THIS SECTION CAREFULLY! YOU HAVE THE OPTION TO REGISTER IN PERSON USING A PAPER VERSION OF THIS ONLINE REGISTRATION FORM. IF YOU DO NOT WISH TO CONSENT TO THE USE OF ELECTRONIC SIGNATURE AND TRANSMISSION AS SET FORTH BELOW, DO NOT PROCEED WITH THIS ONLINE REGISTRATION. PLEASE COME TO THE STUDIO TO REGISTER IN PERSON USING PAPER REGISTRATION.
This authorization concerns consent to the use of electronic means for transmission of information and documents, and the use of your electronic signature for authorizations and agreements, related to enrollment and participation in Gilliam Dance Project ("GDP") programs for the 2020-2021 enrollment year (February 2021-June 2021). If you are enrolling multiple participants, this consent is valid for all students enrolled.
In order to access and provide information and documents electronically through this system, you will need to have a smartphone, tablet, or computer that can access and submit information through the Internet.
By checking the box below, you are: (1) consenting to use of electronic means for the receipt and transmission of information and documents related to enrollment and participation in GDP programs for the 2021 season (February 2021-June 2021); (2) authorizing GDP to store your information and responses in electronic format; and (3) agreeing that your electronic signature on an electronic authorization or agreement will serve the same purpose and have the same effect as a written signature.
If you enter into this Consent to Electronic Signature / Transmission but change your mind later, you may withdraw consent by submitting written notice of your withdrawal to Kara Gilliam by hand delivery or U.S. Mail addressed to Gilliam Dance Project, 228 East Glenwood Avenue, Smyrna, DE 19977. Your withdrawal will be effective upon receipt by the studio. Note that electronic submissions and signatures made prior to the date of withdrawal remain effective following withdrawal.
Your decision to decline or withdraw this Consent to Electronic Signature / Transmission will have no impact on the enrollment of your student(s) at GDP.
You may request a paper copy of the electronic record of any information or documents covered by this Consent to Electronic Signature / Transmission by submitting a written request to Kara Gilliam by hand delivery or U.S. Mail addressed to Gilliam Dance Project, 228 East Glenwood Avenue, Smyrna, DE 19977.
COVID-19 / Public Health Concerns
GDP is committed to the health and wellness of its students, staff, and their respective families. To that end, parents and students are expected to follow appropriate precautions to limit the spread of illness, including but not limited to influenza, COVID-19, and other communicable diseases. By checking below, you acknowledge and agree that you, your student, and your student's other parent and guardian(s) (if any), will comply with all policies, requirements, and guidelines GDP may issue aimed at limiting the spread of communicable diseases, including those issued by the State of Delaware. If your child is experiencing any COVID symptoms or has had exposure to anyone who has tested positive you agree to keep your child at home. If you child has been at the studio in the past 7 days, you agree to notify the studio so that we may take precautions.
All those entering the studio should wear a mask and practice social distancing - this includes dancers waiting for class. At no time, are parents allowed to wait in the lobby for their child. To help with keeping appraised of what your child is learning in class, GDP will occasionally share class videos to parents.
*** Effective December 14, 2020 until January 11, 2021, dancers are required to wear masks during their dance class. ***
GDP instructors will ensure the appropriate distancing is adhered to during class. If necessary, GDP will enforce class limits, when applicable, to adhere to maximum class size requirements. In that event, additional classes will be added to the schedule to accommodate. If your child has a medical condition or a disability that prevents wearing a mask, a request can be made for reasonable accommodation. GDP will update this policy as state requirements change.
In the event that GDP programs are canceled or GDP's means of instruction is altered (including but not limited to remote live and prerecorded video instruction) in order to comply with the directives or guidance issued by state public health authorities, there will be no refund of tuition fees for the month in which the programs are canceled or alternate form of instruction is implemented. In the event that public health concerns necessitate the cancellation or rescheduling of recitals, recital fees and costume fees will not be refunded.
Tuition Policies/E-Payment Authorization
By using the e-payment portal on GDP's website, you are authorizing GDP to use the account information for all of your account billing (including registration fee, tuition, costume fees, late fees, recital fees, and any other studio or dance related items purchased on account).
By checking below, you agree to take full responsibility for payment of all charges and understand that a $25 late fee may be applied to your account if your credit card/bank account is declined and cannot be processed for the amount due at that time.
To discontinue enrollment in e-payment, you must fill out an "ePayment Removal Form" (available at the studio) and return it to the studio office.
Registration Fee: A $25 non-refundable registration fee per family is due at time of registration. As a promotion, registering siblings for the Feb 2021 - June 2021 season is FREE (typical costs are $25/student)!
Tuition is due by the 1st of the month, no later than the 5th, and can be paid via cash, credit/debit card, or check.
Tuition for each class is the amount listed when registering per month per student. After the 5th of the month, a $25 late fee will be charged and enforced on all accounts past due, including e-payment accounts. Late notices will be sent via email. Tuition is not pro-rated for absences or studio closures. Students are encouraged to attend makeup classes for classes missed.
In the event that (a) you overpay for a specific payment obligation owed to GDP (whether for tuition, recital fees, costume fees, competition entry fees, or otherwise), or (b) you are otherwise entitled to a refund of a specific payment made to GDP, you agree that GDP may apply such overpayment or refund to any and all outstanding and unpaid balances owed by you to GDP.
Advanced Payment: You may pay in advance for tuition and fees; however, there is no discount.
Medical Conditions / Emergency
Students and parents/guardians are responsible for making sure all medical conditions and allergies are fully described on the student's tab in the Parent Portal and kept up to date. If your child’s medical condition may or could possibly require medical assistance during your child’s class/es, the parent/guardian must remain on the premises during that time.
Notwithstanding the above, by checking below, you acknowledge and understand that GDP does not render medical care, including medical diagnosis or treatment, to students or others, and that any assistance that GDP staff may offer to a student will be limited to responding to an emergency.
GDP does not carry medical insurance for its students. It is required that all dance students be covered by their own family insurance policies and if injury occurs it is understood that the student's own policy is your only source of reimbursement.
Attendance/Inclement Weather
Good attendance is imperative, as absences and tardiness can hold back an entire class. The studio cannot jeopardize its responsibilities to the rest of the class for one student. Please make every effort to have your child at every class.
In the event your student needs to miss class, please contact the studio by calling or texting the studio phone.
Missed Classes: If a student will be missing 2 or more consecutive weeks of class, please contact the studio office. The office staff will relay the information to the instructors. During a recital year, students with excessive class absences may be pulled from the recital dance with no refund. This is at the teacher’s discretion due to their subsequent impact on the rest of the class.
Make-Up Classes: There are no refunds for missed classes whether it is due to the student’s absence or studio closure. Teachers can suggest comparable make-up classes.
Illness Policy: Parents should keep their students at home for any of the following conditions:
• Pain – any complaints of unexplained or undiagnosed pain.
• Runny nose (green mucus indicates infection), watery eyes, coughing, sore throat, or productive cough. Difficulty in breathing – wheezing or a persistent cough.
• Fever (100°F/38.3°C or more) or nausea and vomiting.
• Infected skin or eyes or an undiagnosed rash. Severe itching of body and scalp.
• Unexplained diarrhea or loose stool combined with nausea, vomiting, or abdominal cramps. These may indicate a bacterial or viral infection that is easily passed from one child to another.
• Children with any known or suspected communicable diseases or lice.
Inclement Weather: In the case of inclement weather there will be information on the telephone’s outgoing message as well as social media accounts (Facebook & Instagram). We will also send an email notification. It is always best to check email or social media 1-hour before your scheduled class if there is inclement weather. No make-up classes will be scheduled for weather closing, but you may make-up the lesson in a comparable class on another day. Make up classes must be used with-in the current session.
Photography and Video Release
GDP uses photographs and videos of its own dancers in its marketing and promotional materials. However, GDP will not include names of any dancers under the age of 18 beside a photograph or video unless specific permission from a parent/guardian has been obtained.
By checking the box below, you are granting GDP full permission to take, use, reproduce, and publish photos, videos, and digital images of your student (collectively, "Images") for GDP marketing and promotional purposes during and after the student's participation in GDP programs. Further, by checking the box below, you acknowledge that GDP shall be the exclusive owner of all rights, including copyright, in the Images, and you irrevocably transfer, assign, and otherwise convey to GDP your entire right, title, and interest, if any, in and to the Images and all copyrights and other intellectual property rights in the Images.
IF YOU WISH TO OPT OUT:
If you do not wish to have photographs or videos of your student(s) used in marketing and promotional materials by GDP or otherwise want this Photography and Video Release to not apply to your student(s), FIRST, indicate that you have read this policy by checking below. SECOND, please return to the Student Information Section of this form and type "I OPT OUT OF PHOTO/VIDEO" in the box provided.
Withdrawal and Termination
To withdraw a student from any GDP class, you must notify the studio office and complete a Withdrawal Form. Failure to attend classes does not constitute a withdrawal. Students and their parents/guardians are responsible for giving at least 2 weeks’ written notice of withdrawal, paying tuition and other fees incurred during this timeframe. During recital years, GDP will refund recital fees for withdrawals prior to February 28, and will refund tuition payments for classes beyond the 30-day notice period (subject to GDP's right to offset against outstanding unpaid balances), but no costume fees, competition fees, or other fees will be refunded.
GDP may, at any time and in its sole discretion, terminate a student's further participation in in any GDP program. In that event, tuition will be pro-rated through the date of termination. Students and their parents/guardians will continue to be responsible for past due tuition amounts and pick-up of costumes. GDP will refund unused recital and competition fees and any tuition paid for classes beyond the termination date (subject to GDP's right to offset against outstanding unpaid balances), but no costume fees or other fees will be refunded.
Pick-Up, Drop-Off, and Time Between Classes
The premises of GDP outside of the classroom studios are not supervised. Students under the age of 7 may not be left unattended by a parent/guardian at any time before, after, or between classes.
Students 7 and older must be dropped off and picked up on time (no more than five minutes before class or five minutes after class). Due to traffic and other safety concerns, parents must park and come inside GDP to drop off and pick up students. Students will not be released to older siblings under age 16. GDP reserves the right to charge up to $1/minute late fee after 2 late pick-ups.
If a student is 7 or older and is participating in multiple classes on the same day, the student may remain inside GDP's building unattended by a parent/guardian between classes.
No student under the age of 18 may leave GDP's premises unattended by a parent/guardian unless they are driving themselves to class. If a student is leaving with another student or someone other than the parent/guardian on file, the parent/guardian must notify the studio desk ahead of time.
Pick-up and drop-off procedures, as well arrangements for time between classes, are subject to modification by GDP at any time, including without limitation during the COVID-19 pandemic. You will be notified by email of any modifications to procedures and arrangements for drop-off, time between classes, and pick-up.
Final Acknowledgement & Release
Having agreed that your electronic signature will serve the same purpose and have the same effect as a written signature, by checking the box below you specifically acknowledge and represent that you have read and understand, and that you agree to, the following statements regarding each student you are registering (referenced below as "Student"):
1. I recognize that programs at GDP require physical exertion that may be strenuous and involve inherent risks of serious physical injury (including death). I also recognize that most GDP programs, by their nature, place students in close physical proximity to each other and to GDP staff and, as such, present inherent risks of illness (including but not limited to influenza, COVID-19, and other communicable diseases) caused by exposure to airborne and surface-borne pathogens. I am fully aware of the foregoing risks and hazards to Student presented by his/her participating in such programs. I KNOWINGLY AND VOLUNTARILY ASSUME ALL SUCH RISKS, EVEN IF ARISING FROM THE NEGLIGENCE OF THOSE PERSONS AND ENTITIES RELEASED FROM LIABILITY BELOW, AND I ACCEPT AND ASSUME FULL RESPONSIBILITY FOR ANY AND ALL INJURIES AND ILLNESSES TO STUDENT THAT MAY ARISE AS A RESULT OF PARTICIPATING IN ANY GDP PROGRAM.
2. In the interest of protecting Student, other GDP students and staff, I acknowledge and agree that Student, Student's other parent and guardian(s) (if any), and I shall comply with the State of Delaware Guidelines During COVID-19 Pandemic ("Guidelines"), and any and all subsequent revisions to the Guidelines that GDP may issue.
3. I understand that the activities involved in GDP programs are only suggested, and that not everything will be appropriate for everyone. I accept responsibility for assuring that Student will: listen to his/her own body; be aware of any physical conditions in his/her body; and know that he/she is not obligated to perform the activities suggested by GDP personnel.
4. I understand that it is my responsibility to consult with a physician prior to Student's participation in any GDP program to determine the appropriateness of Student's participation. I represent and warrant that Student is physically fit and does not have any medical condition(s) that would prevent his/her full participation in GDP programs. I agree to immediately notify GDP of any pertinent changes in Student's health, including updating all medical information on the Parent Portal and keeping Student's instructor fully informed. If Student is out for an injury or illness, I agree that Student will not return to participate in GDP activities until released by his or her physician to do so. I acknowledge that GDP has not and will not render any medical services to Student, including but not limited to medical diagnosis or treatment of any physical condition, injury, or illness.
5. I understand that GDP does not carry medical insurance for Student. I agree to maintain sufficient medical insurance and to pay all costs associated with any injuries and illnesses that may occur as a result of Student's participation in GDP programs.
6. IN CONSIDERATION OF STUDENT BEING PERMITTED TO PARTICIPATE IN GDP PROGRAMS, I, ON BEHALF OF STUDENT, MYSELF, STUDENT'S OTHER PARENT AND GUARDIAN(S) (IF ANY), AND OUR RESPECTIVE HEIRS AND ASSIGNS (THE "RELEASORS"), HEREBY HOLD HARMLESS, WAIVE, AND RELEASE GDP AND ALL ITS OWNERS, OFFICERS, DIRECTORS, REPRESENTATIVES, EMPLOYEES, AGENTS, VENDORS, CONTRACTORS, INSURERS, SUCCESSORS, AND ASSIGNS (COLLECTIVELY, THE "RELEASEES") FROM AND AGAINST ANY AND ALL LIABILITY, CLAIMS, ACTIONS, AND CAUSES OF ACTION, WHETHER KNOWN OR UNKNOWN, WHETHER NOW EXISTING OR ACCRUING IN THE FUTURE, ON ACCOUNT OF PERSONAL INJURY, ILLNESS, DEATH, PROPERTY DAMAGE, OR OTHER LOSS ARISING OUT OF OR RELATED IN ANY WAY TO STUDENT'S PARTICIPATION IN GDP PROGRAMS. THIS RELEASE SPECIFICALLY INCLUDES INJURY, ILLNESS, DEATH, PROPERTY DAMAGE, AND OTHER LOSS CAUSED BY THE NEGLIGENCE OF GDP AND/OR OTHER RELEASEE(S), BUT EXCLUDES INJURY, ILLNESS, DEATH, PROPERTY DAMAGE, AND OTHER LOSS CAUSED BY THE GROSS NEGLIGENCE OR WILLFULL OR WANTON MISCONDUCT OF GDP AND/OR OTHER RELEASEE(S). WE COVENANT THAT NEITHER I, NOR STUDENT, NOR THE OTHER RELEASORS WILL MAKE OR BRING ANY SUCH CLAIM AGAINST GDP OR ANY OTHER RELEASEE, AND WE FOREVER RELEASE AND DISCHARGE GDP AND ALL OTHER RELEASEES FROM LIABILITY UNDER SUCH CLAIMS.
7. In the event that Student is a minor: I warrant and agree that I am the parent or legal guardian of Student; that I have the legal right to consent to this Acknowledgement and Release on behalf of Student, myself, Student's other parent and guardian(s), if any, and our heirs and assigns; and that by signing below, we hereby do consent to the terms and conditions of this Acknowledgment and Release.
8. In the event that Student is not a minor: I warrant and agree that by signing below, I do hereby consent to the terms and conditions of this Acknowledgment and Release on my own behalf as the Student.
