Payment Policies
Payment Policies for the 2021 Season
SMDC will now be moving to a monthly tuition system via our customized Parent Portal. To enroll in a class, a credit card must be held on file. Parents are given four options of how they would like to pay.
Payment & Membership options:
Session in Full: This option allows parents to pay for the session in full.
Auto-Pay - Payments will automatically be posted and withdrawn from your payment on file on the first of every month. (No risk of late fees)
Self-Pay - Parents must manually submit payment monthly by credit card or bank draft through the Parent Portal, or pay by cash in person. Self-pay accounts will be subject to a $25 late fee if monthly payment is not received by the 12th of the month.
Charter - This option is only for students who receive tuition funds through their charter school. SMDC currently works with iLead, Excel, and Blue Ridge. A purchase order must be submitted in the enrolled student's name for the dates of service.
Early Cancellation Fee - If a student withdraws early from the session for a non-medical emergency their credit card on file will be charged a $55 early cancellation fee.
Showcase/Costume Commitment - All classes will be participating in our 2021 Showcase. A $75 showcase commitment will be posted to accounts on February 24th and must be paid by March 5th. This is a non-refundable cost and will include your child's costume for their performance.
New Student Trial Fee - New students are allowed to participate in trial classes for a $20 non-refundable fee. If students choose to continue with the session, their monthly tuition amount will be prorated.
Insufficient Funds Fee - An account will be charged a $25 fee if there are insufficient funds.
Charge Accounts - Items in the studio such as merchandise, shoes, water and snacks can be applied to your family charge account. Parents must provide written permission for students to apply charges to the account.
COVID 19 Parent and Participant Agreement
PARENT AND PARTICIPANT AGREEMENT
To All Sierra Madre Dance Center (“SMDC”) Participants, Parents, and Families,
We, at SMDC, are extremely excited and eager to get back to training in the studio. As we navigate through this uncertain and difficult
time, we are committed to providing a safe environment for all the participants, staff, and families as we return to our dance activities
in some capacity.
As we begin to return to our in-studio dance classes, we will follow some extremely strict protocols and procedures to protect
the participants, staff, and families involved and will ask each family to agree to follow the following guidelines to help keep
everyone healthy.
Parent Protocols:
? If you are not comfortable, DO NOT return to in-studio participation. It is the parent’s decision when your child returns to the
studio. SMDC is also offering Zoom participation as an alternative for in-person dance classes.
? Parents should assess the level of risk to their child[ren], such as children who may have asthma, diabetes, or other health
problems.
? Review and adhere to the SMDC’s protocols and inform your child on the respective requirements for participation.
? Please provide any participant with a face mask. Students will be asked to wear masks at appropriate times (e.g., when
entering class, stretching, and when leaving class).
? Conduct a temperature check of your child for low grade fever (>100.4°F) before class.
? If you or your child or anyone in your household has any type of fever, DO NOT GO TO CLASS.
? Do not go to SMDC facilities with any of the CDC and/or CDPH COVID-19 symptoms.
? If you, your child, or anyone in your household has any CDC and/or CDPH COVID-19 symptoms, speak to a physician and
follow CDC and/or CDPH COVID-19 guidelines on self-quarantine.
? Notify SMDC immediately if you or your child or anyone in your household becomes ill with COVID-19 symptoms as stated
by the CDC and/or CDPH COVID-19 for any reason.
? If you or your child or anyone in your household has tested positive for COVID-19, written confirmation of COVID-19
negative status and clearance from your physician must be provided to SMDC for return to full participation in the studio.
? You acknowledge that should you or anyone in your household test positive for COVID-19, SMDC can inform other
parents/participants of a potential exposure to COVID-19 - although such information shall not include any names.
? Ensure participant’s clothing is washed after every session.
? Ensure all equipment is sanitized before and after every training session.
? If possible, supply participant with individual sanitizer.
? Ensure participant has plenty of water.
? Follow all drop-off and pick-up procedures provided by SMDC and maintain appropriate social distancing protocols when
doing so.
? Wear face-masks whenever on SMDC property.
Participant Protocols:
? If you are not comfortable, DO NOT return to in-studio participation. You may participate by Zoom video conference.
? Review and adhere to the SMDC’s protocols on the respective requirements for participation.
? Wash hands/sanitize hands thoroughly before and after every session.
? Wash and sanitize training equipment/clothes after every session.
? Wear a face mask to class, when leaving or whenever asked to wear it in class.
? Do not share water, food, or equipment.
? Respect and practice social distancing – and follow directions provided in class for social distancing.
? Place equipment, bags, etc. at least 6 feet apart.
? No high 5’s, handshakes, knuckles or group celebrations.
? Notify your teacher immediately if you are feeling ill or notice someone else who is ill.
General Hygiene Protocols:
Parent and Participant further recognize that the following practices generally help to mitigate transmission of any
infectious/communicable disease.
? Avoid touching your face.
? Frequently wash hands with soap and water for at least 20 seconds or use an alcohol-based hand sanitizer.
? Cover mouth and nose (with arm or elbow, not hands) when coughing or sneezing and wash hands afterward.
? Wear a face mask or covering when outside as appropriate, and especially if you have any symptoms of upper respiratory
infection.
? Frequently clean commonly used surfaces (doorknobs, for example) with an antiseptic cleanser.
? Avoid contact with other individuals (shaking hands, for example).
? Maintain a distance of 6 feet between you and others.
? Stay home if you feel sick and contact your health care provider.
Further acknowledgements:
? Parent and Participant acknowledge that being vigilant and forthright with respect to each Participant’s health during this
time is important.
? Parent acknowledges that he/she has read the above-stated protocols and agrees to comply with them. Parent further
acknowledges that Participant has read the Participant and General Hygiene Protocols or parent has reviewed the protocols
with the Participant in an age appropriate manner.
? Parent and Participant acknowledged that failure to comply with these protocols and procedures may lead to SMDC to
suspend and/or refuse to allow the Participant to participate in any in-person training or performances.
? Parent recognizes that SMDC may have to suspend classes on either a short-term or longer-term basis based upon current
conditions at SMDC and/or County and/or State Health Regulations.

COVID 19 Waiver
WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19
ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT
In consideration of being allowed to participate in in-person dance programming and events (collectively
“Dance Activities”) related to the Sierra Madre Dance Center (SMDC”), the undersigned acknowledges,
appreciates, and agrees that:
1. Participation in such Dance Activities inherently includes possible exposure to and illness from
infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular
protocols, rules and personal discipline may reduce this risk, the risk of serious illness and death does
exist.
2. The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health
Organization. COVID-19 is reported to be extremely contagious. The state of medical knowledge is
evolving, but the virus is believed to spread from person-to-person contact and/or by contact with
contaminated surfaces and objects, and even possibly in the air. People reportedly can be infected and
show no symptoms and therefore spread the disease. The exact methods of spread and contraction are
unknown, and there currently is no known treatment, cure, or vaccine for COVID-19. Evidence has
shown that COVID-19 can cause serious and potentially life-threatening illness and even death.
3. SMDC cannot prevent you and your child[ren] from becoming exposed to, contracting, or spreading
infectious diseases, including COVID-19, while utilizing SMDC’s services or premises. It is not
possible to prevent against the presence of the disease. Therefore, if you choose to utilize SMDC’s
services and/or enter onto SMDC’s premises you and your child[ren] may be exposing yourself to
and/or increasing your risk of contracting or spreading an infectious disease, including but not limited
to, COVID-19.
4. I willingly agree to comply with the stated and customary terms and conditions for participation in such
Dance Activities in regards to protection against infectious diseases. This includes my agreement to
comply with Parent/Participant procedures set forth by SMDC with respect to protection against
infectious diseases. However, if I (or my child[ren]) observe any unusual or significant hazard during
my presence or participation, I or my child[ren] will remove myself from participation and bring such to
the attention of the nearest official immediately.
5. ASSUMPTION OF RISK: I have read and understood the above warning concerning infectious
diseases and COVID-19. I hereby KNOWINGLY AND FREELY ASSUME ALL SUCH INHERENT
RISKS, both known and unknown, EVEN IF ARISING FROM THE ACTIVE OR PASSIVE
NEGLIGENCE OF SMDC or others, and assume full responsibility for my participation and choose to
accept the risk of contracting an infectious disease, including COVID-19 for myself and/or my children
in order to utilize SMDC’s services and enter SMDC’s premises. These services are of such value to me
and/or to my child[ren], that I accept the risk of being exposed to, contracting, and/or spreading an
infectious disease, including COVID-19 in order to utilize SMDC’s services and premises in person
rather than enjoying the same services virtually (e.g., via Zoom video conference).
6. WAIVER OF LAWSUIT/LIABILITY: I, for myself and on behalf of my heirs, assigns, personal
representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS and waive my right to
bring suit against SMDC and its owners, officers, directors, mangers, officials, trustees, agents, and/or
employees, and if applicable, owners and lessors of premises (collectively “Releasees”) used to conduct
the Dance Activities, WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss
or damage to person or property, WHETHER ARISING FROM THE ACTIVE OR PASSIVE
NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I, for
myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby agree to
defend and indemnify Releasees for all such claims, causes of actions, allegations or matters arising out
of, relating to, based upon or in any way connected to my participation in such Dance Activities. I
understand that this waiver means I give up my right to bring any claims including for personal injuries,
death, disease or property losses, or any other loss, including but not limited to claims of negligence and
give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen. I
HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT,
FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL
RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY
INDUCEMENT.
7. CHOICE OF LAW: I understand and agree that the law of the State of California will apply to this
contract.
I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE,
AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING
LIABILITY AS DESCRIBED ABOVE:

Release of Liability
Release and Waiver of Liability
PLEASE READ THIS CAREFULLY. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR AND YOUR
CHILD’S LEGAL RIGHTS.
Participant desires to participate in certain dance classes and activities of Sierra Madre Dance
Center (“SMDC”) and assures that he/she is in proper physical condition to take part in dance
classes and activities at SMDC. Participant understands that there are risks of physical injury
associated with, arising out of, and inherent to dancing and activities associated therewith.
Participant understands that such risks simply cannot be eliminated, despite the use of safety,
without jeopardizing the essential qualities of the activity. In recognition of this acknowledged
risk of injury, and in consideration of being accepted into such dance classes or activities
Participant hereby freely, voluntarily and without duress executes this release and waiver of
liability under the following terms:
1. Release and Waiver. Participant does hereby release and forever discharge and hold
harmless SMDC, its owners, operators, members, employees and agents for any and all
claims or demands for damages, loss of services, costs, expenses, and causes of actions
whatsoever, in any way resulting from any and all injury to Participant or property of
Participant arising directly or indirectly out of Participant’s participation in any and all
activities of SMDC including, but not limited to, dance classes, competitions, travel to
and from competitions or performances, and any and all activities associated or
performed in conjunction with SMDC. Participant understands that this Release
discharges SMDC from any liability or claim that the Participant may have against
SMDC with respect to any bodily injury, personal injury, illness, death, or property loss
or damage that may result from Participant’s activities with SMDC, whether caused by
the negligence of SMDC or its Members, employees, agents or otherwise. Participant
also understands that SMDC does not assume any responsibility for or obligation to
provide financial assistance or other assistance, including but not limited to, medical,
health, or disability insurance in the event of injury or illness
2. Indemnification. Participant agrees to indemnify and hold forever harmless SMDC, its
Members, employees and agents from any and all injuries, damages, costs and attorney’s
fees whatsoever which may arise out of the Participant’s participation in any SMDC
activity or presence on the premises of the dance studio.
3. Acceptance of Inherent Risk: Participant expressly accepts and assumes all of the risks
inherent in this activity or that might have been caused by the negligence of SMDC. My
participation or my child's participation in this activity is purely voluntary and I elect to
participate or allow my child to participate despite the risks. In addition, if at any time I
believe that event conditions are unsafe or that I or my child am unable to participate due
to physical or medical conditions, then I or my child will immediately discontinue
participation.
4. Medical Treatment. I, the undersigned parent or guardian of the Participant, hereby
authorize SMDC and its owners, employees, independent contractors and other agents to
consent to and authorize the emergency medical treatment of the below named
Participant by a physician duly licensed under the provisions of the California Medical
Practice Act or by a dentist duly licensed under the California Dental Practice Act. I
understand that this Consent to Emergency Medical and Dental Treatment will be used
by the SMDC only if it is unable to reach me within a reasonable period of time given the
circumstances of the emergency. On behalf of myself, any other parents/guardians, and
Participant, I forever release SMDC and its owners, employees, independent contractors
and other agents from any and all liability related to the exercise of the authorization
provided herein.
5. Insurance. Participant understands that, except as otherwise agreed to by SMDC in
writing, SMDC does not carry or maintain health, medical or disability insurance
coverage for any participant. Participant understands that it shall be Participant’s sole
responsibility to obtain his or her own medical or health insurance coverage for
Participant. Participant represents that he/she has adequate insurance to cover any injury
or damage he/she or his/her child may suffer or cause while participating in this activity,
or else agrees to bear the costs of such injury or damage. Participant further represents
that he/she or his/her child has no medical or physical condition which could interfere
with Participant’s or his/her child’s safety in this activity.
6. Agreement To SMDC Policies and Code of Conduct. My signature below confirms
that in addition to this Release and Waiver, that I, the undersigned parent or guardian of
the Participant, have read and understood SMDC’s Policies, Student Code of Conduct,
and Parent Code of Conduct. SMDC reserves the right to drop students or families from
enrollment for reasons not limited to: disruptive behavior, non-payment of tuition,
violations of SMDC’s Codes of Conduct, and/or extreme disregard for SMDC policies.
7. Photographic Release. It is understood SMDC reserves the right to use any photography
and videotaping of a student’s performance or classes for purposes of advertising,
promoting or publicizing the studio. Participant does hereby grant and convey unto
SMDC all right, title and interest in any and all photographic images and video or audio
recordings made by SMDC during Participant’s participation of any and all SMDC
activities, and further consents to use Participant’s photo or likeness in or on any form of
advertisement for SMDC or a SMDC sponsored event. SMDC will not use Participant’s
name without the consent of the Participant or a parent/guardian.
8. Miscellaneous. This Release and Waiver of Liability shall be binding on the heirs,
successors and personal representatives of the Participant. Participant expressly agrees
that this Release and Waiver of Liability is intended to be as broad and inclusive as
permitted by the laws of the State of California, and that this Release and Waiver of
Liability shall be governed by and interpreted in accordance with the laws of the State of
California. Participant agrees that in the event that any provision herein shall be held to
be invalid by any court of competent jurisdiction, such provision is severable, and the
invalidity of such provision shall not otherwise affect the remaining provisions of the
Release and Waiver of Liability which shall continue to be enforceable to the full extent
permitted under the law.
I have read the foregoing Release and Waiver of Liability and acknowledge that I understand
that I am bound by the terms hereof by placing my signature herein. If Participant is a minor, the
parent having legal custody and/or the legal guardian of the minor hereby represents and
warrants that they have the legal authority to bind the Participant and such parent or guardian
executes this release on behalf of the minor as well as the parent or guardian, and this release is
binding on the child and all such parents and guardians.
