Assumption of Risk
By signing below, you agree that you are aware that your child named above will be engaging in a physical activity offered by Triple Arts Academy ("Triple Arts Inc." or "Triple Arts Studios"). Classes offered by Triple Arts Academy ("Triple Arts Inc." or "Triple Arts Studios") are of a physical nature and include the risk of injury either from oneself or another participant or in the use of equipment and facilities, or of illness such as, but not limited to Influenza, MRSA, or COVID-19 that may lead to paralysis or death.
Release of Liability, Covid-19
Following the pronouncements above I hereby declare the following: I am fully and personally responsible for my own safety and actions while and during may participation and I recognize that I may be in any case be at risk of contracting COVID-19.
With full knowledge of the risks involved, I hereby release, waive, discharge the Organization, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19.
Payment Policies
1. By registering with Triple Arts Studios' class program(s), you are committing to the full season (September 1 to January 31, February 1 to June 30, July 1 to August 31).
2. New students in the Karate program can enroll for a 6 week program, then transition for the balance of the session. New students in the Dance program cam enroll for a one month trial, then transitions for the balance of the session.
3. Fees are due upon registration for the karate promo, dance trial, or for returning students, for the session, in the form of an email transfer, cheque or cash.
4. If the full amount of the tuition is not paid within 30 days, 5% late fee is applied and the total session amount is due immediately.
5. If you are changing a class membership type for the next month, a 30 days notice must be submitted to the front desk or via e-mail. The remaining balance owing for the session will be due within 30 days of the change.
6. Session fees are non-refundable. Memberships cannot be suspended.
7. A $29 fee will be charged for returned cheques.
8. Karate and Dance Classes will not be pro-rated for absences, whether they are planned or unplanned. Pro-rated classes are available for new students only, who join the class after the beginning of the month.
9. Music Lessons will only be credited if the front desk or administration is given 24 hours notification of absence, otherwise student must pay for the missed lesson.
Class Cancellation and Make Up Policy
- Triple Arts Inc. has the right to combine or cancel classes if registration for the class is fewer than 3 students.
- If at any time it is required to cancel classes due to bad weather, a recorded message will be on the answering machine after 2:30pm.
We will do our best to run classes except under dangerous driving conditions. Please use your own discretion regarding attendance if
the driving is difficult in your area.
- If at any time it is required to cancel classes due to an emergency, an e-mail or phone call will be placed.
- If Triple Arts Inc. cancels a class, a make-up date will be announced. No credit will be given for missed classes.
Parent & Student Responsibility
PARENT RESPONSIBILITY
- Teachers are only responsible for students during their class time. Parents are responsible for making sure their child arrives on time and
are picked up on time.
- Please do not send students and/or siblings into the studio with a considerable amount of time unattended or if they do not have class.
- Parents are not permitted in the dojo/studio during class time. Viewing areas are set up for parents.
Instructors will periodically invite parents into the dojo/studio on designated viewing days.
- If a relative, friend, or neighbor is picking up your child, please inform the instructor ahead of time for safety reasons.
- Students under 12 years of age must have their parent or guardian present within the premises
- Please try to keep missed classes to a minimum as students will be missing out on valuable training.
- No refunds will be given for missed classes.
STUDENT RESPONSIBILITY
- Students are asked to be considerate and respectful to all teachers, parents and other students.
- Only water is allowed in the dojo/studio.
- Please no jewelry.
- Please be punctual. A ten-minute grace period will be accepted for latecomers. However, to avoid injuries (lack of warm-up) students
later than 10 minutes will be asked to sit and watch the class.
- Students must wait inside the building for their ride to pick them up.
- Students must be in proper karate/dance attire for class and hair must be tied back securely.
- Cell phones: If a student decides to bring their cell phone into the dojo/studio, for security reasons, they will be required to place
them in their bag. While classes are in progress, all cell phones must be off or on silent.
Photographic Release
I agree that my or my child's picture or likeness can be represented and published in any Triple Arts
Academy ("Triple Arts Inc.") publication or media, unless I have provided written instructions to the contrary.
