2020/2021 Terpsichore Dance Center, Inc. Dance Programs/Activities
hereinafter called "TDC Programs/Activities", I as the consenting party, parent(s) or legal guardian(s), for myself, my heirs, personal representative or assigns, do hereby release, waive, discharge, and covenant not to sue Terpsichore Dance Center, Inc., its officers, employees, and agents from liability from any and all claims including negligence of Terpsichore Dance Center, Inc., its officers, employees, and agents, resulting in personal injury, accidents, or illnesses (including death) and property loss arising from, but not limited to, participation in TDC Programs/Activities.
Assumption of Risks: Participation in TDC Programs/Activities carries with it certain inherent risks that cannot be fully eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks include, by way of example and not by limitation, scratches, bruises, sprains, or other bodily injury disability, exposure to viruses, and/or death.
I have read the previous paragraphs and I know, understand, and appreciate these and other risks that are inherent in TDC Programs/Activities. I hereby assert that participation is voluntary and that students knowingly assume all such risks.
Indemnification and Hold Harmless: I also agree to INDEMNIFY AND HOLD Terpsichore Dance Center, Inc. HARMLESS from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney's fees brought as a result of my involvement in TDC Programs/Activities and to reimburse them for any such expenses incurred.
Severability: I as the consenting party further expressly agree that the foregoing waiver and assumption of risks agreement is intended to be as broad and inclusive as is permitted by the law of the State of Massachusetts and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
Acknowledgement of Understanding: I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am consenting to the agreement freely and voluntarily, and intend by my eSignature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
Health & Safety Policy
COVID-19 guidelines at our studio:
1. Please drop off and pick up your child at the bottom of the stairs. We will have a staff member at the front desk for you to call if you need to relay a message to your child.
2. Your child MUST wear a mask from the time they enter the stair case to the studio until they leave. Rules may change on this as we know more about safety for COVID-19.
3. Your child MUST use hand sanitizer before participating in dance class. There is hand sanitizer set up in each dance room, teachers will help reinforce this rule.
4. Students MUST socially distance while in the studio. We are reinforcing the 6 feet apart rule.
5. When your child first comes up to the studio they should go straight to their dance room. Please have your child come dressed for dance, the changing room will not be in use until further notice. Bathrooms are available to use.
6. We only ask that parents to children 5 and younger wait in our waiting room. We have spaced out chairs in accordance to being 6 feet apart. There is very limited seating. We ask for your help in keeping Terpsichore a place students can come dance safely.
Class Cancellations | Makeups
Classes may need to be canceled from time to time due to poor weather.
Staff will make every effort to communicate any cancellation as soon possible via multiple methods of communication. Families should be on the lookout for an email, a notification in their Parent Portal, and/or a notification on Facebook when weather is questionable.
Makeup classes are available for most classes, check with your teacher for proper make up classes. Students have 2 months from the time of a canceled class to make up a class.
Classes are held during most school vacations and Monday holidays, except Thanksgiving Day and Christmas break, see postings on our yearly schedule.
Dress Code
Ballet - leotard, tights, ballet slippers, ballet skirt (any color for all items)
Lyrical - leotard, tights, nude jazz shoes, optional dance short
Jazz - leotard, tights, nude jazz shoes, optional dance short or jazz pant
ModernJazz - leotard, tights, nude jazz shoes, optional dance short or jazz pant
Contemporary - leotard, tights, dance sock, optional dance short
Irish dance - leotard, dance short, ballet slippers or Irish dance shoes (speak to teacher)
Hip hop - athletic clothing, clean sneakers or dance sneakers
Tap - leotard, tights, jazz pants, leggings, tap shoes
Please Note - Hair should be up and out of face for ALL Classes
Costume Policy
Recital Costume sheets will go out on October 31st and will be due back along with payment by November 14th. The recital is optional but very enjoyable for our students and families. Tentative Recital dates are: June 19th and 20th of 2021.
Costume Fees:
Small Child, Medium Child, Large Child - $55
X-Large Child, Small Adult, Medium Adult, Large Adult - $65
X-Large Adult - $70
Media Authorization & Release
In consideration of the student(s), directly associated with the consenting party, I grant Terpsichore Dance Center, Inc. permission to:
• Use the aforementioned student(s) name, image, likeness, voice in connection to any media, promotion and/or marketing materials;
• Record, film and/or photograph the aforementioned student(s) participation and appearance at any Terpsichore Dance Center, Inc. program/activity, whether on or off the premises;
• Reproduce, transmit, broadcast, distribute, modify, publicly display and/or publicly perform, in print, electronic or any other medium, copies of any such recordings/films/photographs, including but not limited to advertisements, brochures, online videos, website images, television, and publications.
I forever release Terpsichore Dance Center, Inc. from any and all claims and liability arising out of the use of such recordings and the use of my name/likeness in any manner whatsoever, including, without limitation, claims for defamation, invasion of privacy, compensation, copyright and/or other rights of publicity.
Payment Policy
Registration Fee:
• There is a $20 registration fee per student or $30 per family with multiple students.
• Registration fees are non-refundable and non-transferable.
Tuition:
• Payments will follow 5-Week pay periods over 8 sessions ranging from Aug - Jun.

• To encourage and respect social distancing standards we are instituting touchless payments.
• Credit card and/or debit card information will be taken at registration and families are responsible for verifying the correct card information is on file at all times.
• By default, all families are enrolled into an auto payment plan that charges the card on file for recurring payments following the payment schedule.
• For unique situations, please inquire at terpsichoredancecenter@gmail.com or call 413-443-5392 to accommodate other accepted payment methods.
• A late fee of $15 will be charged for tuition received 10 days past payment due dates.
• If a family has failed to pay tuition by the 10th day after a payment due date, the credit card or debit card on file will be charged along with the $15 late fee.
• Accounts with declined transactions that result in non-payment will be charged a $30 bank fee.
• Tuition is non-refundable and non-transferable even if student(s) have dropped before the end of a session.
• Any balance owed on a dropped family account will be charged to card on file as soon as drop is confirmed.
• Tuition may be prorated, only during initial registration, if occurring in the middle of a session.
• At any time, scheduled on-premises classes may need to be canceled and instead held virtually via zoom class. Staff will give as much notice as possible in these cases. Payments will remain the same.
• No credits are given for missed classes or weather related cancellations.
• Previous dance year credit (owed to you) or debit (owed from you) balances resulting from state mandated shutdowns (COVID-19) will be carried forward to family accounts shortly after current year registration & enrollment.
