Tuition Autopayment
Online registration requires a credit/debit card and email address
Tuition for the following month will be posted to all accounts on the 25th. Tuition is due on the 1st of the month and can be paid online with a credit/debit card or at the studio with a check or cash. Any tuition balance remaining unpaid on the 5th of the month will automatically be charged to the credit/debit card on file, October 5, 2020-May 5, 2021. You will be emailed with the total first month's tuition and registration fee that was charged to your credit card. This amount will include all applicable discounts. Tuition for virtual classes will be the same as tuition for in person instruction.
Online payment Option
Tuition is charged per student. A full month's tuition is charged for all months, including those with 5 weeks, holidays and missed classes. September tuition and registration fee are due upon registration for the 2020-21 school year.

When registering online you will be charged a $50 annual registration fee and your first month's tuition. The registration fee and first month's tuition are non-refundable. You are also agreeing to the auto-payment option where tuition unpaid on the 5th of the month, will be charged to your credit or debit card on file (through May).
Costume Fees
Pertains only to fall enrollment
I hereby authorize The Ballet Conservatory to charge my credit/debit card the age appropriate costume fee per dance beginning with one costume in Oct. If more than one dance form is taken, those costumes will be charged one per month in Nov and Dec. Any remaining costume balance will be charged on January 5, 2021. After January 5, 2021 costume fees are non-refundable.
Recital Fee
There is a Recital Fee of $90 per student (Family Maximum of $180). The Recital Fee is due March 1, 2021. I hereby authorize The Ballet Conservatory to charge my credit/debit card the appropriate recital fee.
Discount Information
Students paying the fall and spring semester tuition in advance will receive a 5% discount. To receive the pay in advance discount, fall semester tuition is due upon registration and spring semester tuition is due Feb. 5, 2021. If a student withdraws from the BC, no refunds will be given on semester tuition paid in advance.
Family Discount--A family discount of 10% will be given if more than one child from the same immediate family is enrolled and a total of 4 classes or more are taken.
Families will be eligible for only one type of discount.
Withdrawal
I understand that I must give the studio 15 days notice by way of an email when dropping my child from a class. Tuition will continue to be charged until we receive the email notification.
Virtual Classes
By participating in The Ballet Conservatory's online classes, you agree to release and forever discharge The Ballet Conservatory, Inc.., its officers, agents, employees, faculty, independent contractors, and volunteers from any and all claims, actions, damages, or loss for which I or my minor child(ren)/ward(s) may incur or which may accrue to me or may child(ren)/ward(s) on account of participation in The Ballet Conservatory programs
Safer Studio Policy
Ballet Conservatory requires all staff and students to stay home when they are ill. In order to reduce community spread of COVID-19, we follow best practices and recommendations from our local health department and local government. BC understands that unlike older children and adults, young children cannot be expected to maintain social distancing at all times. Therefore BC focuses on a hierarchy of measures beginning with keeping ill people home, requiring masks to be worn when entering the studio and at all times inside the studio, checking temperatures before entering the studio, followed by frequent hand cleaning and good hygiene practices, amplified cleaning, and facility adaptations to minimize contact and mingling where possible. All studio classes are backed up on Zoom. In the event a class is unable to be held at the studio classes will convert to Zoom until in-studio classes can resume.
COVID-19 Waiver
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregations of groups of people.
The Ballet Conservatory (BC) has put in place preventative measures to reduce the spread of COVID-19; however, BC cannot guarantee that you or your child(ren)/ward(s) will not become infected with COVID-19. Further, attending BC could increase your risk and your child(ren)/ward(s)' risk of contracting COVID_19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren)/ward(s) and I may be exposed to or infected by COVID-19 by attending BC and that such exposure or infection may result in personal injury, illness, permanent disability and death. I understand that the risk of becoming exposed to or infected by COVID-19 at BC may result from the actions, omissions, or negligence of myself and others, including, but not limited to, BC employees, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren)/ward(s) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability or expense, of any kind, that I or my child(ren)/ward(s) may experience or incur in connection with my child(ren)/ward(s)'s attendance at BC or participation in BC classes. On my behalf, and on behalf of my child(ren)/ward(s), I hereby release, covenant not to sue, discharge and hold harmless BC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating hereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of BC, its employees, agents and representatives, whether a COVID-19 infection occurs before, during, or after participation in BC program, class, or event.
Inclement Weather
In case of inclement weather, notifications will be sent by social media and the 5 Pointes email. The studio does follow the Lewisville Independent School District (LISD) closing policy. The BC does not pro-rate tuition for weather cancellations, but make-up classes may be scheduled
Parent Release
I have read and agree to abide by The Ballet Conservatory's policies and I understand that The Ballet Conservatory reserves the right to refuse instruction to anyone not abiding by The Ballet Conservatory policies.
I understand that participation in a dance program involves risk and possible injury. I understand that The Ballet Conservatory and its staff will not be held responsible for injuries sustained in class, while performing, or traveling to and from its facilities.
I give permission for The Ballet Conservatory, Inc. to use photographs of my child in advertisements and/or the schools' website/Facebook page. If you would like to opt out of the photo release please contact the front desk at 972-317-0194.
