Assumption of Risk/Release of liability
Student and student's family understand that although Coy's Camps and Classes works to minimize any risks to students, it is still possible for the student to get injured and/or ill while attending any activities sponsored by Coy's Camps and Classes as well as A Tale Of Two Kings. Student and family agree to hold Coy's Camps and Classes, A Tale Of Two Kings as well as owners and employees harmless from any and all liability, injury, illness, or loss arising out of our child's (and family's) participation in any of the programs. I certify that my child is fit to participate in all program activities.
Medical Emergencies
In case of medical emergency, I grant permission for Coy's Camps and Classes to perform first aid and/or call 911 and transport to a hospital at their recommendation.
Media Release
I hereby grant permission to Coy's Camps and Classes and A Tale Of Two Kings to use photos, videos, and comments of my child(ren) for educational, informational, and promotional purposes. I understand that these materials may be used for instructor training and may be posted on the internet for a period of time.
Payment Policies
Payment for one time events (including camps, tournaments, etc) is due at the time of registration. Payment for tuition of ongoing classes/recurring events/clubs: 1st month is due at registration. Each of the following months will be charged on the 1st of the month (or first day of business of the month) to the method of payment on file. To cancel future payments, notify us in writing by the 25th of the preceding month. Any payments returned for non sufficient funds, invalid credit card, etc will be given an opportunity to correct the information and will be charged for any expenses we incur related to collecting payment (i.e. bank charges for bad payment). Missed classes can be made up but will not be refunded.
