COVID-19 Risk & Release of Liability
I understand that Ballet Arts Academy is permitting individuals to enter the premises despite the ongoing spread of COVID-19, commonly known as "the Corona Virus." Despite efforts to prevent the spread of this virus, Customer might contract this virus at Ballet Arts Academy. Said permission for Customer by Ballet Arts Academy to enter the premises is not to be relied as determination that it is safe or advisable even if done in accordance with Center for Disease Control guidelines, State of Washington guidelines, federal guidelines, local guidelines, and/or any other statutes, regulations, guidelines. All persons who enter the premises of Ballet Arts Academy do so at their own risk and peril. Ballet Arts Academy has no liability under any theory of liability for permitting Customer to enter the premises at Ballet Arts Academy. I, therefore, agree to assume and take on myself all the risks and responsibilities in any way associated with my entry onto the premises of Ballet Arts Academy. In consideration of and return for the opportunity to enter the premises of Ballet Arts Academy, I release Ballet Arts Academy (and all Ballet Arts Academy owners, and employees) from any and all
liability, claims, and actions that may arise from injury or harm to me or others, including death related to COVID-19.
Assumption of Risk & Release of Liability
We understand that Ballet Arts Academy involves physical movement, which may from time to time be strenuous, and that such practice carries some risk of injury.
We acknowledge that it is the students responsibility to inform the instructor at the beginning of class of any injury or other condition that may affect his/her ability to participate , and to inform the instructor immediately if any injury occurs during class.
We understand that from time to time during classes offered through Ballet Arts Academy, the instructor may give hands-on assistance to facilitate the understanding of postural alignment, technique and turnout.
We hereby agree to assume full responsibility for any risk ,injuries or damages, known or unknown, which the student might incur as a result of participating in classes and/or workshops offered through Ballet Arts Academy.
We give our permission to seek any emergency medical treatment for our child that might be necessary if you are unable to contact us.
We voluntarily waive any claim we may have against Ballet Arts Academy and its employees.
Studio Policies
STUDIO BEHAVIOR: BAA is a school. Students and family members are asked to be quiet and courteous upon entering the building. Students should arrive dressed or with enough time to prepare themselves and be waiting to enter their class before it starts. Staff is not available to supervise children before or after class time. Students are not allowed in the Pilates room, costume rooms or open studios without permission. Siblings of students need to be supervised at all times. Unsupervised or disruptive children may be asked to leave.
ATTENDANCE: Faculty prefers that absences be reported prior to class time. This may be reported to the teacher via text, email or phone call. Please give as much notice as possible for any vacations or appointments that will cause an absence. If your child was sick from school, please keep them home from class as well. A missed class may be made up in a lower level class with teacher approval. BAA reserves the right to cancel a class due to insufficient registration or attendance.
PROMPTNESS: It is essential that students come to class on time. In ballet terms, on time is at least 10 minutes before class begins. Lack of a proper warm up at the beginning of class can cause injuries. If your child is running more than 10 minutes late please let the studio know ahead of time. Late arrivals disrupt class concentration. Students over 10 minutes late for class may be asked to sit and observe or warm up silently off to the side.
ARRIVAL & DEPARTURE: Parents must drop off and pick up students in a timely
manner. If you are just dropping off or picking up, you may idle in front of the building as long as your flashers are on and depart within a few minutes.
DRESS REQUIREMENTS: Girls: Pink tights, pink ballet shoes, and a leotard in the class color and style assigned. Hair must be secured neatly in a bun off the face and neck. No jewelry. Boys: short-sleeved white t-shirt, black tights and black shoes. white socks and white shoes.
If a student forgets their uniform, they may look for a spare in the dresser in the girl's dressing room but will not be allowed to borrow from the costume rooms. For students levels 4 and higher, there should be no undergarments showing, this includes bra straps, the correct uniform should have this built in.
DRESSING ROOM: Please do not leave anything of value in the dressing room. We are not responsible for lost or stolen items. There are lockers available to the older students. If a student owns a cell phone, it must be kept in the dressing room with the ringer silenced. Students gathering in this space must keep volume down to not distract our studio classes.
BULLYING: Bullying or harassment will not be tolerated. If your student experiences unkind teasing please bring it to the attention of our instructors, they want to hear from you. Respecting our staff, faculty, other students and ourselves is what we teach at BAA. Disrespectful behavior will be brought up to a student's parent in a 1 on 1 meeting. If behavior continues, BAA can expel the student and tuition will not be refunded.
Tuition Policy
If I am registering for a free trial class, I understand that bank or credit card information is taken to complete registration and reserve my spot. I understand that Ballet Arts Academy will NOT initiate any charges to my account until I agree and enroll my students into classes.
When I enroll in classes, I understand that I am entering into a contract to pay tuition for the 9 month academic year (Sept-May).
An annual registration fee of $30 per student or $40 per family is due upon enrollment and will be charged to my account in order to reserve my child's spot at BAA.
During the registration process, bank ACH information or a credit/debit card will be held on file. I agree to pay monthly tuition on the 1st of every month. If I wish to pay annually or by trimester I will contact the office.
Invoices will be emailed before payment is due to allow me to pay through my Parent Portal with different banking information if I choose.
On the second of each month tuition will be charged to the bank information on file.
Payments are considered late 7 days after due date.
Additional Fees: NSF Check Fee $35, Late Fee $15
Withdrawal Policy
If it becomes necessary to withdraw from class, we agree to provide a written notice to the studio 30 days before the next billing cycle. We acknowledge that if withdrawal is due to prolonged illness or injury, a verified doctor's note is required. We agree that tuition will continue to be charged monthly throughout the school year if written notice is not received.
