ASSUMPTION OF RISK
By registering and enrolling in classes with Stockbridge Tae Kwon Do Academy, Inc. (hereafter referred to as STKD in all other policies), I hereby acknowledge, understand and agree with the following:
1. That martial arts training is an activity that involves physical contact and that participation could result in serious injury, including permanent disability or even death, and severe social and economic loss.
2. That such risk may be due to not only my own actions, but also the action, inaction or negligence of others, the regulations of participation, or the conditions of the premises, or of any of the equipment used.
3. That there may be other risks that are not known to me or to others or not reasonably foreseeable at this time.
4. Agree to inspect the facilities, equipment and pairings prior to participation. I will immediately inform an instructor if I believe that anything is unsafe or beyond my capability and refuse to participate.
RELEASE OF LIABILITY
By registering and enrolling in classes with STKD, I hereby acknowledge, understand and agree with the following:
1. Assume all of the foregoing risks and accept personal responsibility for any damages that may result from injury, permanent disability or death.
2. Enter martial arts training and/or competition entirely of my own free will and understand the importance of following the rules of training, belt promotion testing and competition.
3. I certify that I am in good physical condition, and have no disease, injury or other condition that would impair my performance or physical and mental well-being during intense training practice and/or competition.
4. Release, waive, discharge and covenant not to sue, STKD, its affiliated organizations and governing bodies, their officers, instructors and personnel, other members of the organizations, participants, supervisors, coaches, sponsoring organizations or their agents, and if applicable, owners and leasers of the premises from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses and damages which may be sustained and suffered on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.
5. Some activities will require additional signed liability waivers provided by affiliates, service providers or venues (i.e. swimming, field trips and tournaments).
MEDICAL EMERGENCIES
Provided the medical care and treatment of the participant is on the advice of licensed physician, I authorize and request all, hospital or other providers of medical services to follow the instructions of any person identifying himself or her as adult supervisor of STKD, at the time and under any circumstances whatsoever, I understand that authorization and consent herein provided includes an X-ray examinations, anesthetics, medical or surgical diagnosis or treatments, and hospital care rendered to the participant under the general or special supervision or, on the advice of a licensed physicians, surgeons and anesthesiologist, dentist, or other qualified medical personal acting under their supervision.
GENERAL TERMS & CONDITIONS
By registering and enrolling in classes with Stockbridge Tae Kwon Do Academy, Inc. (Hereafter referred to as STKD), I hereby acknowledge, understand and agree with the following:
Students and guardians are required to update their personal information with STKD in a timely manner immediately following any information changing.
Marketing Consent: We take pictures and video of our students and post them on our website (including virtual class, social media and advertisement. Enrollment in a class with STKD is agreeing with and giving consent that images, video, audio and/or stories are authorized to be used by STKD without restriction.
Uniforms: All students are required to wear STKD branded uniforms during each class. Female students are required to wear a white T- shirt under the uniform for modesty and optional for male students. Please write names on all belongings.
Sparring Gear: Students are required to purchase their own sparring gear once they obtain their yellow belt level. This includes chest pad, headgear, hand covering, foot covering, mouth piece and groin cup (mandatory for all male students) is required for every student participating in sparring class. Optional protection may also be used, but should be approved by instructor prior to use.
There will be no use of foul language or improper jesters int his school at any time and under no circumstances. All students are required to keep their hands and feet to themselves at all times.
At not time shall a student (minor) exit the building without the permission or without supervision by staff.
There will not be any tolerance of destruction to STKD property at any time, including but not limited to; scratching of mats in the training area, climbing on the training equipment, no throwing items, or any malicious damage to any school.
Any student caught stealing any item not belonging to them will be asked to leave the program immediately and appropriate authorities may be contacted.
No fighting of any kind will be tolerated. Students who engage in this kind of behavior will be asked to leave the program.
All students will be given three chances or three written warnings for conduct infractions before being removed from the program unless otherwise warranted for immediate removal.
FEES
Registration Fee: Is non-refundable and covers administrative account setup. Payment required prior to student being allowed to enroll/attend a class (in studio or virtual), that is not considered a free trial session.
Tuition Fees: Is non-refundable and payment required prior to student being allowed to attend a class (in studio or virtual), that is not considered a free trial session. Tuition is based on classes chosen for enrollment. Tuition cost is listed in the class information displayed prior to enrollment through our online portal. Tuition discounts are offered for multiple classes enrolled by the same student and/or for more than one student enrolled in classes within the same family account.
Belt Promotion Test Fee: based on current belt/stripe level

Late Payment: $25 will be billed to your account after payment grace period or due date as outlined in the applicable class type section of this document.
Late Pickup Fee: Students are expected to leave premises at the end of each enrolled class end time. $5 will be assessed to the account for every 15 minutes after the class scheduled end time until the student is picked up. Please be prepared to pay at the time of picking up the day the fee is assessed to mitigate any financial obligation misunderstandings that tend to be occur when paying at later date. Example: enrolled class ends at 6:00PM and student is not picked up until 6:20PM, only $5 will be assessed to the account at 6:15PM. If you are unable to pick up student on time, we respectfully request a courtesy call so we can make other arrangements if needed as this could require overtime or additional work hours by STKD staff. Failure to notify of a Late Pickup could result in the account being assessed a Fail to Notify Fee for each occurrence which is also expected to be paid at the time of picking up for the day being assessed.
Fail to Notify Fee: $25 fee will be assessed if student or family fail to communicate with reasonable notice prior to schedule start or end time for enrolled class. This includes when a student will not be picked up at the end of class or STKD picking up from school is not necessary for afterschool participants.
Early Termination Fee: $275 for one student, $365 for two students and $475 for three students.
NSF/Payment Declined/Chargeback Fee: $35 will be billed to your account for all non sufficient funds (NSF), declined payment processing or if payment is cancelled or challenged (chargeback) by cardholder.
EXPECTATIONS FOR STUDENTS
Student Conduct: STKD expects all students to treat staff, other students as well as property of STKD and affiliates (including, tournaments or field trips) with respect and courtesy and to act responsible and take responsibility for his/her actions.
Students should not participate in any program if sick with any cold, fever and/or contagious illness (or having potential exposure of contagious illness which requires isolation or social distancing). For daytime programs, please let us know the day before, if possible, or by 8:45 am if your child will not be attending as this could impact timeliness of pick-up schedules, departures or other preparations necessary, including food service.
Students will be on-time and in accordance to the class details that the student is enrolled in. If times or other details change, STKD will provide appropriate and reasonable notice and communications to impacted students.
Lost or Missing Personal Items: STKD is NOT responsible for any lost or stolen items that students bring with them. We recommend keeping valuables at home whenever possible. Lockers are provided for some classes, but is the responsibility of the student to use correctly and keep their personal items safe. Personal belonging, including but not limited to clothing, electronics, toys, snacks or any other item that is not the property of STKD, will be held in the appropriate lost and found location for up to 30 days after which any/all unclaimed items may be donated to a non-profit organization for the benefit of their patrons. Items with student/family name on it will be communicated with the family, otherwise no notice will be provided regarding lost personal items.
Participation: All students on STKD premises will be expected to participate in any/all enrolled classes for that day, unless otherwise excused from participation with either verbal or written communication from parent or other authorized adult on the account.
EXPECTATIONS FOR PARENTS, FRIENDS AND FAMILY MEMBERS
Nonparticipant Observation of Class: NO COMMUNICATION between observers and students allowed during the class. Students should be focused on listening to the directions from both instructors, upper belts asked to assist with class and other staff members. If you decided to stay and observe class, we ask that all non-participating family members and/or friends stay in the observation room (parent room) and keep discussions or other noises to a minimum to allow all students to focus on the lessons being taught.
BELT PROMOTION TESTING & AWARD CEREMONY
Belt promotion testing will be held to test student's abilities and knowledge and must receive a passing grade to progress to higher level of Martial Arts. Students are allowed one free retest if receiving less than passing grade and must be completed prior to award ceremony.
All promotion tests are by invitation only at the discretion of our instructors based on preparedness of the student, satisfactory attendance, active participation in class and adherence to the Tenants of Tae Kwon Do and the Student Oath.
Minimum required attendance to be eligible for testing is participation in two classes per week during the testing period. Participation can be done in the studio or through our virtual (online) classes, but prior to being allowed to test, instructor needs to see student in the studio to verify the student is prepared for testing by observing technique, forms and one-steps.
Students are also required to read a specified number of books (must provide book title and author details during testing registration/enrollment) and/or write essays to eligible for testing.
Tests are scheduled at the discretion of the Master Instructor based on the overall preparedness of students. Historically this testing period has been every 2-3 months. Test registration and testing fee must be completed prior to registration deadline to be eligible for current test. Testing fees are not included in tuition fees.
Late arrival on testing day can will result in removal of testing. If unable to attend testing day, a make-up test will only be conducted on a case-by-case basis if arranged by the student prior to testing day.
NO regular classes (all types) will be held on testing and award ceremony days. Test results will be not be provided prior to award ceremony night.
AFTERSCHOOL CLASS PROGRAM
By registering and enrolling in classes with STKD, I hereby acknowledge, understand and agree with the following:
Our afterschool program is a contractual agreement for the duration of the current Henry County School District calendar year. Registration and enrollment do not rollover and is required for each school calendar year.
Weekly tuition is due by the close of business on Friday for the next week's pickup week. If tuition is not received on-time, your enrollment slot could be given to or acquired by another student on a waiting list that assumes your contractual obligations. If there are no students on a waiting list or no one to take your enrollment slot, you will continue to be responsible for your contractual obligations for the duration of the contract period or pay an early termination fee.
Students will be picked up from participating schools at the end of school. Upon enrollment, please notify participating school officials that your student(s) will be picked up by STKD otherwise the school may not release to STKD staff. Class enrollment is specific to the participating schools based on room allocated on STKD transportation vehicles.
If your student does not attend a participating school, enrollment is still possible and is the responsibility of the family to drop student off at 3:00PM or later on days class is offered.
STKD is not a licensed daycare and their services should not be considered supervising and/or care giver in nature but rather the services rendered are to teach and practice martial arts and philosophical character-building skills as outlined in the Tenants of Tae Kwon Do and the Student Oath. STKD is exempt from the Bright from the Start: Georgia Department of Early Care and Learning and is not required to be licensed. STKD is not assuming responsibility for the provision of daily child care outside the scheduled program (from arrival to departure, including scheduled classes). All students must be picked up by 6:00PM, unless also enrolled in other class types (i.e. evening class) on the same day. Failure to do so without prior notification by phone could result in notification to local authorities and/or a late pickup fee assessed on the account. Discounts are available when a student(s) are enrolled in multiple classes (i.e. afterschool and evening) and for some families this is both financial responsible to avoid late pick-up fees based on their schedules as well as assists in the student(s) potentially progressing more rapidly with their martial art skills with greater exposure to instructors and variety of peer students to learn from and practice with.
Homework/schoolwork: Although a time is set aside for students to do quite study, STKD staff will only be providing assistance in reading instructions, pronunciation of vocabulary or words from books or reading notes from teacher/school at the request of the student. This is personal quiet study time and STKD is not responsible for tutoring or educating of students. Students may also ask their peers for assistance as needed, but must do so in a manner that will not disrupt other students studying. No sign-off of schoolwork or notes from teacher or school will be done by STKD staff and will be the responsibility of parent.
All Students must be in uniform each day. If a student forgets his/her uniform then the student's legal guardian will be contracted to make arrangements to pick their student immediately.
CLASSES OTHER THAN AFTERSCHOOL
Non-afterschool classes are a contractual agreement for 12 months commencing on your registration date or first available class, whichever is later. Contract will renew for an additional 12 months, unless written cancellation (termination) is submitted 30 days prior to the end of the current contract period. An early termination fee can be paid to opt-out of remainder of contract period. Terminated membership will require returning student to register again and pay the registration fee and tuition fee rates at the time of new registration and class enrollment.
A trial period will be for 30 days from the contract commencement date allowing student/family to change their mind and terminate the contract without future financial penalty, but no refund will be granted of fees already paid.
Monthly tuition is due by the 5th of each month, every month for the duration of the contract period.
If participation in the studio is not convenient, participation through virtual (online) option is acceptable. Please see Belt Promotion Testing & Award Ceremony policies for possible testing eligibility if attending virtually.
CAMPS/FIELDTRIPS
Children eligible to participate in this summer camp must be at least 6 years of age and completed one year of school. For safety of your child, we adhere to a staff to child ratio for all our programs. We cannot guarantee a spot for every child. Available weekly slots are allocated on a first-come, first-serve basis, for students who have fully completed both registration and enrollment, including tuition paid.
Credits and/or refunds will not be permitted for any day(s) your student(s) miss or are not in attendance as scheduled field trips that have hard costs are included in weekly tuition and many require advanced payment to reserve for our students.
Breakfast & Lunch will be served daily by food service program. If child has specific needs, parents are required to bring their own lunch and breakfast. If bringing food, it should be labelled with student's name. We also have prepackaged snacks for sale.
Attire: All clothing should be chosen with the scheduled activity in mind, including footwear. STKD has adopted Henry County Public schools non-uniform dress code. For example, shorts should comply with acceptable lengths for school as well as shirts or tops should comply with sleeve lengths. Swimming suits must be modest, including only one-piece suits will be acceptable for girls. Shirts are required to worn and depending on class type, a shirt may be provided by STKD.
Assistive Devices: Students may require devices assist them either swim or do other activities. These personal devices are the sole responsibility of the Student in keeping them safe and ensuring they are used correctly.
