Liability Disclaimer
I acknowledge and fully understand that Twisted Sisters Yoga, LLC takes all reasonable care to ensure that its classes are safe and fun; however, I understand that I or my child, will be engaging in physical activities that may involve some risk to injury. I acknowledge and have been advised that it is my responsibility to consult with my child's physician with respect to any past or present injury, illness, health conditions, or any other condition or medication that may affect my child's participation. I accept and hereby assert that my or my child's participation is voluntary and assume the foregoing risks and accept full personal and financial responsibility for any personal injuries sustained by my child or myself which may have been incurred as a result of participating in this program in any capacity. I individually, and as parent and / or guardian of the minor child identified above consent, agree to hold harmless Twisted Sisters Yoga, LLC, its owners, members, directors, employees and agents from any claim, cause of action, or liability for damages arising from any personal injury to my child or other persons or property damages caused by myself or my child's actions or participation in the Twisted Sisters Yoga, LLC program.
Medical Policy
I acknowledge there is no obligation for any person to provide medical care during, prior to or after any activity or program related to Twisted Sisters Yoga, LLC. In the event of an emergency I hereby give permission for staff to provide basic first aid and / or in the event of an emergency and in my absence, I agree to allowing Twisted Sisters Yoga, LLC or its staff to contact emergency services at will. In the event medical treatment is provided, I hereby waive any claim against Twisted Sisters Yoga, LLC their members, officers, employees, subcontractors or agents for any injury, damages or death caused by negligent provision of such medical care.
Photo Release
I grant consent for me and/or my minor's picture to be taken or to be filmed while participating in activities at all Twisted Sisters Yoga and events. I authorize Twisted Sisters Yoga to use and publish images, photographs, pictures, portraits, and audio, video and/or film footage of me and/or my minor in all forms of media and in all manners for publication including, but not limited to, advertising and marketing campaigns, press releases, social media, periodicals, and website use. I hereby waive any right I may have to review, inspect, edit or approve such publication and I release Twisted Sisters Yoga from any claims I may have against it for use of such images, photographs, pictures, portraits, and audio, video and/or film footage of me or my minor.
Payment Policy
All purchases including classes, workshops, birthdays, merchandise or other are final. We do not offer refunds, transfers or exchanges. No refunds for unused classes, workshops, birthdays or other special events.
Auto-Payments: You agree to leave a valid credit or debit card on file with Twisted Sisters Yoga, LLC, and authorize Twisted Sisters Yoga, LLC to charge your card automatically each month. If your card number changes, expires, or you have a change of address, please update your information by notifying us in writing at hello@twistedsistersyoga.com at least 10 days before your next scheduled monthly payment so that we can update your information for you. A $25/month penalty fee may be assessed if your card information is not updated and your scheduled monthly auto-pay is unsuccessful. Twisted Sisters Yoga, LLC reserves the right to cancel your program after any unsuccessful charge attempt and you agree to continue to be responsible for payment, late fee, attorney or late fees associated with the account balance. Twisted Sisters Yoga, LLC is not responsible for any fees assessed by your bank or credit card provider for lack of funds, overcharges, or any other fees.
