I hereby irrevocably grant to The Salvation Army, its successors and assigns, its agents and those by whom it is commissioned, the absolute, unrestricted and unlimited license, right, permission, and consent to use and reuse, disseminate, copyright, print, reproduce, publish and republish, for any and all trade purposes or commercial or other advertising or public purposes, and in any and all advertising, publicity, display, publication or media, my child's name, signature and likeness, and any portraits, pictures, photographic prints or other representations of my child, or in which my child may appear, or any reproductions or sketches thereof or parts thereof, photographic or otherwise, with such additions, deletions, alterations or changes therein as you in your discretion may make, either separately or together with my child's name or a fictitious name, or the name of another person, with or without any statements or testimonials made by my child, or authorized by me which you may, in your discretion, prepare for use in connection therewith. I warrant that I have not limited or restricted the use of my child's name or photograph to the use of any organization or person.
I hereby grant unrestricted use of audio tracks or text by The Salvation Army for such purposes as The Salvation Army may deem appropriate.
I hereby release and discharge The Salvation Army, its successors, assigns and agents from any and all claims and demands arising out of or in connection with the use of any of the foregoing, including any claims for defamation, invasion of privacy or violation of any statutory right.

VPK policies
The Salvation Army Child Development Center will be operating a Voluntary Pre-Kindergarten for 3 hours a day, 5 days a week for 180 days. All enrolled families will receive a calendar listing the days off during the operational period of August 13, 2024, through May 24, 2024.
Attendance during the scheduled VPK days is of upmost importance in order to remain in the VPK program. The Salvation Army's funding is linked to attendance as well as your child's success upon starting Kindergarten.
You are required to read, sign, and comply with the following polices to remain in the VPK Program.
Tardiness:
Children must arrive at the center between 8:30 and 9:00 a.m., early drop off will be available. The instructional day starts at 9:00 a.m. and all children are expected to be in class and ready to start the day. Late arrivals are disruptive to group learning as well as being difficult to the arriving child. We understand that an occasional late arrival might occur but more than twice a month will not be accepted and will be cause for termination from the VPK program.
Attendance:
Daily attendance in the VPK program is of upmost importance for optimal learning. You will be allowed 2 absences per month. Any absences beyond those will require a written note from the parent for one of the following reasons:
• Illness, injury of the child, or infectious disease, doctor's note may be required.
• Physician or dentist appointment, or compliance with court order.
• Funeral service or bereavement upon the death of the child's family member.
• Special education or related services for the child's disability.
• Family vacation, not to exceed 5 excused absence per program year.
Please note: 5 consecutive absences on instructional days will be considered a withdrawal from The Salvation Army Child Development Center VPK program.
We will allow one documented 5-day absence during the 180 days of instructional period.
Documentation must be submitted in advance and explain the reason for the 5-day absence.
Verifying your child's attendance and absences:
Center staff will ask you to stop by the office at the end of each month. You will be given a form to review and confirm your child's recorded attendance for that month. Your signature on the form will verify your child's attendance and direct the Early Learning Coalition of SWFL to submit payment for the months' VPK program for your child to The Salvation army.
Late Pick-up:
The instructional VPK day ends at 12:00 p.m. All children not staying for wrap around care must be picked-up at that time. A late pick- up fee of $10.00 will be charged beginning at 12:05 p.m. An additional fee of $15 will be assessed for every additional 15 minutes the child is in attendance.
VPK early drop off/extended care policies
TUITION AGREEMENT: EARLY DROP OFF/WRAP AROUND EXTENDED CARE
Early Drop Off and Wrap Around Extended Care requires a one-time registration fee of $25.00.
Tuition fee of $95.00 is due every Monday and is payable by credit or debit card only, no cash please. Payment must be received by 6:00 p.m. each Monday.
If payment is not received by Wednesday at 6:00 p.m. an additional late fee of $5.00 will be charged.
If your child's account is past due your child may be suspended from the center until account is brought up to date. Failure to keep account up to date could result in your child being dismissed from our center.
If you would like to pay every two weeks, please let us know. Payments must be made in advance.
You will have four weeks this school year available to you during non-instructional days which you can use as vacation. Fee for non-instructional weeks is $110 per week.
Vacation time does not carry over from year to year and our year runs from August to May.
In the event you want to remove your child from our center, please give us at least two weeks' notice.
