I as the parent or legal guardian of (my child or myself) hereby give permission for my child to participate in all activities with Studio H Dance Company (SHDC). I agree that we will comply with SHDC rules, policies, and procedures as they exist from time to time. I understand that dance instruction, performance, and related activities involve some risk of injury or harm. I assume all liability (whether for injuries or otherwise) arising out of my child’s participation in any such activities. I agree to indemnify and to hold SHDC and its owners and employees harmless from and against any and all liability, damages, costs, claims, and expenses incurred by any of them in connection with my child’s participation in any SHDC activity. If SHDC determines that an in jury suffered by my child requires medical attention, I understand that SHDC will make a reasonable effort to contact me. I hereby give permission for emergency medical treatment to be given to my child by providers chosen by SHDC, however, if that effort is unsuccessful. By agreeing below, I further bind myself, my child, and our respective heirs, executors, administrators, successors, and assigns.
All tuition is due on the 1st of each month of the dance year, regardless of what day of the week the ?rst falls. This includes weekends and holidays when the school is closed. Statements will be sent for overdue accounts only.
After the 5th of the month, tuition payments are considered late and a $35 late fee will be incurred.
If an account becomes 60 days overdue, the account holder will be noti?ed that the student cannot participate until the account has been paid in full or a payment plan has been agreed upon with our office.If an account becomes 90 days past due it will be sent to a local collections company to be collected.
All payments received are applied to outstanding charges in the order in which they were billed. No exceptions.
The tuition rate is for the entire season (August - May.) It is not based on the number of classes during a month, but on the entire duration of our dance season. Some months will have more/fewer classes, but tuition charges remain the same.
WITHDRAWAL FROM CLASSES:
If for any reason it becomes necessary to withdraw a child from a class, a drop form must be ?lled out in the office. The drop form must be submitted before the ?rst day of your child's last month. FOR EXAMPLE: If a drop for is submitted on September 4th, October will be the student's last month. September and October tuition will be due. If the drop form is not ?lled out, the person responsible for payment will continue to be billed the regular tuition amount and will be responsible for payment. Non-attendance is not considered withdrawal from class. Studio H Dance Company does not require a contract. Therefore, if an account is closed, August and May tuition (paid at registration) will not be refunded. There will be a $35 charge for all checks returned from your bank and late fees will apply. After two returned checks, we will no longer accept checks as a method of payment. Payment methods include online payment, Visa, Mastercard, Discover, cash, and check. We do not accept payments over the phone or through email. ALL FEES, TUITION, COMPETITION FEES, CONVENTION FEES, CHOREOGRAPHY FEES, COSTUMES, etc. are NON REFUNDABLE.
