Students and parents/guardians are responsible for making sure all medical conditions and allergies are fully described on the student's tab in the Parent Portal and kept up to date. This includes a description of any required emergency care medication or device (such as an "epi-pen," inhaler, etc.) which the student may need and specific instructions for when/under what circumstances the medication/device should be used, and how it should be used.
In addition, all students with an emergency medication/device must comply with the following requirements:
• The medication/device must be placed in a closed plastic baggie clearly labeled with the student's name and easy to understand written instructions concerning when/under what circumstances the medication/device should be used, and how it should be used. The instructions in the bag also must include the name and telephone number for two emergency medical contacts.
• Students with a medication/device are required to bring the bag to and from CSTC activities and, at all times while at CSTC, to keep the bag in a gear bag on their person or, during classes, in the classroom.
Notwithstanding the above, by checking below, you acknowledge and understand that CSTC does not render medical care, including medical diagnosis or treatment, to students or others, and that any assistance that CSTC staff may offer to a student will be limited to responding to an emergency.
The undersigned City Springs Theatre Conservatory student (and his or her parents or guardian if the student is a minor) hereby unconditionally releases and forever discharges the City Springs Theatre Company, and its officers, board members, employees, and agents from any and all damages, cost liabilities, obligations and claims whether arising from negligence or otherwise and of other injury of any kind, which may be sustained during or by reason of enrollment for study at the Conservatory from this date through August 2020.
Some of the activities engaged in at the Conservatory are physically strenuous and only students who are medically able to participate should do so. Regardless of the cause, the student (or parents or guardian) assumes full responsibility for all risks of accident and personal injury that may result from participation in Conservatory activities. The Conservatory assumes no responsibility or liability with respect to the student's injuries which are sustained as the result of deficiencies in the student's physical health.
City Springs Theatre Conservatory Staff reserves the right to dismiss or otherwise discipline any person whose behavior is not acceptable or endangers the safety of themselves or others. Unacceptable behavior includes verbal harassment, physical assault or sexual abuse, and the use or possession of alcohol. The use, purchase or sale of any illegal substances at the Conservatory is strictly forbidden and will not be tolerated. If anyone is involved in any activity associated with these substances or devices, the appropriate law enforcement agency will be contacted to deal with the situation. No refunds will be made in the event of such disciplinary action. Costs incurred by City Springs Theatre Conservatory in a disciplinary action will be the responsibility of the participant, Parent or Guardian.
The Undersigned has read the Release and attached rules, fully understands it and agrees to be legally bound by it and the rules of City Springs Theatre Conservatory.
