As the legal parent or guardian, I understand that my child's participation in music lessons or off site events with Class Act Music Academy may expose him/her to unforeseen risk of personal injury, death or property damage. I hereby acknowledge that I am voluntarily allowing my child to participate in all lessons and events and agree to assume any such risks.
I hereby release, discharge and agree not to sue Class Act Music Academy, its workers, directors, agents, faculty or employees for any injury, death or damage to or loss of personal property arising out of or in connection with my child's participation in any lessons or events from whatever cause, including the active or passive negligence of Class Act Music Academy owners, staff or any other participants of Class Act Music Academy.
In consideration for being permitted to participate in the School, I hereby agree, for my child, my heirs, administrators, executors and assigns, that I shall indemnify and hold harmless Class Act Music Academy from any and all claims, demands, actions or suits arising out of our in connection with my child's participation in any of the School's events.
I give Class Act Music Academy permission to authorize emergency medical personnel to carry out such emergency diagnostic and therapeutic procedure as may be necessary for my child, and also permit such treatment procedures to be carried out at, and by local hospitals for my child int he event of an emergency. I understand that any medical expenses will be billed directly to me or my insurance company.
I affirm that I have read, understand and agree to be bound by the term and conditions of this contract for music instruction through Class Act Music Academy. I fully understand that failure to abide by this contact will result in termination of this agreement and membership in the school will be revoked immediate. I understand that I can also review these policies any time online at fortemusicschool.com.
I HAVE CAREFULLY READ THIS RELEASE, HOLD HARMLESS, AGREEMENT NOT TO SUE, AND FULLY UNDERSTAND ITS CONTENTS. BY CHECKING THE BOX I AM ACKNOWLEDGING AND AGREEING TO A FULL RELEASE OF ALL LIABILITY AND I SIGN IT FREELY.
LESSON CANCELLATION POLICY
Cancellation of a lesson must be made AT LEAST 24 hours prior to the scheduled lesson time in order to receive a make-up lesson (for definition of a make-up lesson, please see below). This includes leaving messages on the answering machine or email.
1. No shows or failure to notify the school/teacher within 24 hours is considered a forfeited lesson. Illness may be cancelled same-day. Please see student illness policy.
2. No more than 2 cancellations per month will be rescheduled.
3. If you know of dates ahead of time where you/your child cannot make a lesson, please notify the school when booking/paying for your lessons.
4. There are NO refunds on unused or cancelled lessons.
5. Should you choose to take a break, unused lessons can be banked for you for a period of 2 months. Unused lessons after that time will be considered forfeited.
6. Credits CANNOT be transferred from teacher to teacher.
MAKE UP LESSONS
Any lessons cancelled at least 24 hours ahead of the scheduled lesson will receive a make-up lesson at a time convenient to both the teacher and the student. Typically, make-up lessons take place at an alternate time/day within the week. In some cases, the make-up lesson will be added after/before the original lesson time, enabling the student to have an hour lesson.
Make-up lessons not used after 2 months will be considered forfeited. A make-up lesson can be booked with your teacher directly.
No students should attend lessons when sick (ie fever, nausea, vomiting, etc). Class Act Music Academy reserves the right to dismiss a student from a scheduled lessons if the student arrives at that lesson exhibiting possible symptoms of illness. In this case, a makeup lesson will not be offered.
If the student is exhibiting symptoms of illness the day of, one cancellation per 3 months will receive a make-up lesson. If there are 2 or more day of cancellations due to illness within a 3 month period, the cancelled lesson will be considered forfeited, and any make-up lesson will be left to the discretion of the teacher.
Class Act Music Academy complies with all laws prohibiting discrimination in the conduct of its operations and programs. Class Act Music Academy admits students of any religion, gender, race, color, sexual orientation, economic status, national and ethnic origin to all rights, privileges, programs and activities generally accorded or made available to students at the academy. It does not discriminate on the basis of religion, race, color, sexual orientation, economic status, national and ethnic origin in the administration of its educational policies or programs.
Tuition and Registration
Tuition is paid monthly; it is not calculated by the weekly lesson. Your monthly tuition rate is the same each month even though some months will have as many as 5 lessons and others as few as 3. Holiday closures have been calculated into the monthly tuition rate.
Tuition is based on enrollment. Not attendance.
Class Act Music Academy offers open enrollment so you may register for classes at any time. If a student begins lessons in the middle of the month we will prorate for the number of lessons remaining before the beginning of the first full month. For first time students, payment will be charged on the first day of lessons. All remaining payments will be charged on the 2nd day of the month thereafter.
There is a one-time registration fee of $25 for each student and $10 for each additional family member.
The tuition and registration fees are non-refundable and non-transferable.
We accept Visa, MasterCard, Discover, American Express, and Checks. For your convenience we offer automatic billing through Square Reader. This is a proven safe, secure and convenient way to pay ongoing fees while enrolled at the school. Upon registering, you will be billed for the registration fee and for the number of lessons remaining in the month. Your credit card, debit card will then be billed on the second day of the month thereafter.
We have a late payment fee policy of $25 per student. If tuition payments are not received by the seventh day of the month it is considered late and the fee will automatically be applied, no exceptions. It is the students/parents responsibility to ensure we have been provided a valid credit/debit card if enrolled in automatic billing.
Declined Payment Fee
There is a $30 fee for any returned checks, in addition to any additional bank charges we incur.
Class Act Music Academy reserves the right to suspend or discontinue instruction of any student who is delinquent in fulfilling his/her financial obligations at any time.
