Safety is Good Times School of Music's number one priority. We have internal policies and procedures that will help us in maintain safe standards. However, there are times when we may not have control of certain risks and hazards and because of this we have this release of liability for you to read, acknowledge and agree to.
I, the undersigned parent/guardian of the student of Good Times School of Music, desire to have my child participate in the lessons and events held by Good Times School of Music (GTSOM).
I fully understand and acknowledge the dangers, hazards, and risks that may arise from engaging in the program. I also understand and acknowledge that these dangers, hazards, and risks can result in serious injury and/or impairment to my or my child's body, general health and/or well-being.
Knowing the dangers, hazards, and risks of the GTSOM program and in consideration of being permitted to participate in the activities of GTSOM, on behalf of myself, my family, spouse, heirs, and personal representative(s) (the "Releasors"), I agree to assume all the risks and responsibilities surrounding my/my child's participation in all activities of the program. On behalf of myself and the Releasors I hereby covenant not to enter into litigation with Good Times School of Music, or its trustees, officers, representatives, and employees ("Releasees"), and I hereby release, waive, and forever discharge the releasees from and against any and all liability for any harm, injury, damage, claims, demands, actions, causes of action, costs, and expenses of any nature that I may have or that may hereafter accrue to me or a Releasor, arising out of, or related to, the Activity, whether caused by the negligence or carelessness of the Releasees or otherwise.
I further agree to indemnify and hold harmless the Releases from and against any loss, liability, damage or cost, including court costs and attorneys' fees, that may arise due to my participation in the activities. It is my expressed intent that this Liability Release and Waiver shall bind me, the members of my family and spouse, if I am alive, and my estate, family, heirs, administrators, personal representatives, or assignee, if I am deceased, and shall be deemed as a legally binding release, waiver, discharge and covenant not to sue the Releasees.
I agree to conduct myself in accordance with the rules of conduct and standards of behavior that are expected of me as a parent and my child will as a student of GTSOM and to abide by the various instructions and guidance we am given.
I do not suffer from a physical or mental impairment that would limit my ability to participate in activities. I understand, agree and hereby grant Releases permission to authorize emergency medical treatment for me/my child, if necessary and arising out of the activities, and that such action by Releases shall be subject to the terms of this Agreement. I understand and agree that Releases assume no responsibility for any injury or damage which might arise out of or in connection with such authorized emergency medical treatment.
THIS IS A LEGAL AGREEMENT AND INCLUDES A RELEASE OF LEGAL RIGHTS. READ AND BE CERTAIN YOU UNDERSTAND IT BEFORE AGREEING.
I represent that I am the parent or guardian of the minor listed below and that I have the full legal capacity and right to execute this release. By selecting "agree", I hereby acknowledge that I have completely read and fully understand the Good Times School of Music Release of Liability. I also acknowledge that I agree to abide by the provisions set forth in the Good Times School of Music Liability Policy.
Tuition Rate
$250/quarter (3 months)
$200/quarter after first quarter (if enrolling consecutively)
Tuition Policy
Payment for the entire quarter is due by the first lesson of each month. That payment reserves a certain lesson time for you for the entire quarter. If absent from a lesson or performance, the cost will remain the same regardless of the reason. That tuition pays for a reserved spot for each student on a quarterly basis, whether or not the student attends each lesson.
Missed Lesson Policies
It is expected that you will make music lessons a priority and schedule other activities around your lesson time. No refunds or credits are given for missed lessons. Other than an illness, please provide at least a 24-hour notice if you will be unable to make the lesson. No make-up lessons will be available for group lessons.
Lessons are not taught on the following Federal holidays:
January 1 (New Years Day)
3rd Monday of January (Martin Luther King Jr. Day)
3rd Monday of February (Presidents Day)
Last Monday of May(Memorial Day)
July 4 (Independence Day)
July 24 (Pioneer Day)
1st Monday of September (Labor Day)
4th Thursday of November (Thanksgiving Day)
December 24 (Christmas Eve)
December 25 (Christmas Day)
December 31 (New Years Eve)
If one of those holidays falls on your lesson day, your tuition will be prorated for school closure days. This provision applies only to the actual holidays, not to other days surrounding the holidays.
In rare cases when a teacher misses a scheduled lesson for any reason (illness, family emergency, etc.), the lesson will be made-up. Good Times School of Music reserves the right to find a substitute instructor if necessary, however, if a substitute teacher isn't available, a credit will be issued towards the family account, to be used in the next billing cycle.
If you know you will miss a lesson for any reason, please inform us as soon as possible, as a matter of courtesy. There will be no tuition refund or credit, nor will a makeup be carried forward to a future week.
Tuition may be prorated for a quarter in which you intend to stop taking lessons. To qualify for prorated tuition, you must inform us of the date that you intend to stop taking lessons, 30 days in advance.
Tuition Payment Terms
Payment for each quarters tuition is due at the beginning of the quarter, by the date of the first lesson for that month. We accept payment in cash, checks, and credit card. A credit card is required to register online. If payment is not made by the first day of lesson, this credit card will be drafted the tuition amount.
DISCONTINUING / RESTARTING LESSONS
If you need to discontinue lessons for any reason, your lesson time and spot in the band you are working with will not be held for you. These bands all depend on having concrete members and will need to move forward. If you would like to restart in the program, we cannot guarantee that we can get you into the same group/band. Because of this, we highly recommend that you maintain membership.

By selecting "agree", I hereby acknowledge that I have completely read and fully understand the Good Times School of Music Tuition Policy and Rates Agreement. I also acknowledge that I agree to abide by the provisions set forth in the Good Times School of Music policies.
I represent that I am the parent or guardian of the minor child listed below and hereby give my consent to the Good Times School of Music (GTSOM) to take or use photographs, digital images, and or video/audio recordings of my child for use in marketing and/or educational materials as follows: printed publications or materials, posters, brochures, greeting cards, calendars, electronic publications, or Web sites.
I agree that my child's name and identity: may be revealed in descriptive text or commentary in connection with the image(s). I authorize the use of these images without compensation to me. All negatives, prints, and digital reproductions shall be the property of GTSOM.
I agree that GTSOM may use, edit or reproduce such photographs and videos or share them with others for any purposes related to the GTSOM.
I agree that I will not post to social media anything inappropriate in nature of lessons or events held by GTSOM, and that I will take care when posting photos, audio and video of any of the students of GTSOM.
I hereby waive, release and forever discharge the Good Time School of Music from any and all claims or liability arising from the use of my child's photograph for the above purposes. This authorization is continuous and may only be withdrawn by my specific rescission of this authorization.
