For Emergencies only, I give my consent for my child(ren) to be transported and supervised by the Elite Learning employees.
Media / Video Release
I give / do not give permission to Elite Learning to videotape, photograph or audiotape of my child(ren) based on my response in "Permission to release media?" above in the "FAMILY INFORMATION" section
Medical Authorization & Permission to Treat
For emergency medical care, if I cannot be reached to make arrangements, I authorize the Elite Learning employees to take my child(ren) to the Physician and Emergency Medical Care Facility provided on this form. If your child needs any medication such as Epi-pen and/or inhalers, a separate medication must be provided to Elite Learning for use during our After School program.
Health & Safety
Elite Learning requires all health and immunization records be kept current. Parents have the responsibility of providing information regarding allergies, dietary restrictions, and anything else that could help prevent medical emergencies.
For the safety of all the children enrolled in our program, we will not provide care to a child who is displaying any of these symptoms:
> A fever of 100.4° Fahrenheit or more
> Consecutive vomiting or diarrhea
> A persistent cough
> Too fatigued to participate in regular activities
> Signs of an infectious disease: rashes or hives, live lice, or atypical eye, ear, or nasal discharge
If your child develops any of these symptoms while on campus, we will ask that you pick them up immediately. Your child will not be permitted back in the program before they have been symptom-free for 24 hours.
Additionally, if your child was absent from the school during the day due to an illness, he/she cannot check in with our program in the afternoon, either.
Records
I acknowledge that the immunizations and/or tuberculosis tests are current and my child(ren)'s immunization, vision and hearing records are on his/her school campus files.
Admission Requirement
The following documents must be submitted If your child(ren) did not attend pre-kindergarten or a child care facility within one week of admission.
1. Doctor's Statement: I have examined the above-named child(ren) within the one year period and confirm that the child(ren) are eligible to register to the Elite Learning after school program and its activities.
2. A signed and dated copy of a health care professional's state is attached.
3. Medical diagnosis and treatment conflict with the tenets and practices of a recognized religious organization, which I adhere to or am a member of; I have attached a signed and dated affidavit stating this.
4. My child has been examined within the past year by a health care professional and is able to participate in the daycare program. Within 12 months of admission, I will obtain a health care professional's signed statement and will submit it to the child-care operation.
Enrollment & Withdrawal
Before enrollment, parents must complete all enrollment agreements prior to approval, whether the child has special needs or not. Elite Learning does not have access to the school specific documents that parents submit to schools; such as medical records, custody legal documents, etc, and therefore, we must be notified by the parents of special accommodations for each child.
Enrollment forms can be filled out online at our Elite Learning website. Registration is required at least 24 hours prior to the start date.
You must inform Elite Learning at least 10 business days prior to withdrawal from the afterschool program in order to avoid additional charges to your account. Notifications must be in writing and verbal notifications will not be accepted. If you do not notify our office with a withdrawal request in time, your account will be charged half of the next monthly payment. There is no refund for withdrawing from the program during the current month.
Pick up Policy
Parents are required to list those who are authorized to pick up the child. Elite Learning will verify the identification of the person picking up the child and will not release the child to anyone who is not listed in the system.
In the case that an unlisted person attempts to pick up the child, Elite Learning will contact the parent or legal guardian and discuss the verification options. Elite Learning will require written authorization for releasing the child to the unlisted person.
Each person on the designated pick-up list is assigned a unique pin number to sign out the child. Pin number information will be provided upon acceptance to the program. For the safety of your child, we ask that parents not share their pin number with anyone and that they not use each other's pin.
Our staff members are instructed to identify anyone they do not recognize. Please inform the person picking up your child that they will be asked to present a form of identification.
Once a child is checked out from our program, the same child will not be able to check in the program until the following school day.
If the person picking up the child, authorized or not, seems intoxicated or gives the Elite Learning staff a reason to fear for the child's safety, Elite Learning reserves the right to refuse releasing the child to that person. We will instead contact someone on file to find an alternative pick up arrangement and we will call the local police if needed.
Custody Issues
Anyone listed on the Child Enrollment and Health Information page of the registration form will be allowed to pick up their child anytime Elite Learning is in session. They are also permitted to designate other people to pick their child up.
This is the case unless Elite Learning is given a copy of a court order or decree that authorizes the restriction of parental rights and the Program Director is apprised of the court order or decree. Parents are responsible for providing Elite Learning with current and updated court documents as they change since the schools do not share custodial information with Elite Learning.
If only one parent is the legal custodian of a child, only the legal custodian will be permitted to designate others to pick up the child. This will be the case so long as a court order or decree provided to Elite Learning supports such a determination.
In the event of an emergency, the custodial parent will be contacted first. If that custodial parent is unavailable, we will reach out to the other parent.
Behavior Guidance and Management Policy
Our child guidance and management policy is set in place for all parties involved in the program throughout its duration, including students, teachers, and parents. Our main objective is for Elite Learning students to learn independence and self-discipline in order to implement the social skills they'll develop. Self-control is refined through daily practical activities and social exercises.
Positive behavior will be rewarded with verbal reinforcement and other incentives. Inappropriate behavior will be addressed, and the child will be promptly redirected to another activity. Depending on the severity of the situation, other consequences may be put into effect. Parents will be notified, and teachers will work to help prevent a pattern of misbehavior. When a child requires extra care and attention, a special behavior management program may need to be created with the parents' assistance.
Inappropriate Behavior
Aggression among classmates and repeated misbehavior that includes bullying, threats,
physical altercations, and property damage will result in an incident report. The purpose of an incident report is to notify parents or legal guardians of the child's behavior and to find positive solutions that will prevent this behavior in the future. Parents are asked to sign the report in acknowledgement and are encouraged to constructively discuss the incident with the child. One copy will go to the Elite Learning office and another copy will be available for parents to take home. We ask that all issues be settled in a respectful and positive manner.
If any damage to Elite Learning-owned or leased property takes place due to a child's misbehavior, Elite Learning reserves the right to assess the parent or legal guardian for any and all amounts necessary to repair or replace damaged items or property.
Suspension
For more serious incidents or repeated misbehavior, we may ask parents to pick up their child. After communicating with all parties present, the student may be suspended. Before the child can return to the program, a parent-teacher conference is required.
Payment will be due for the time a child is suspended, which will not exceed five school days. If the child continues their misbehavior after suspension or continues to be a threat to the wellbeing of other students and/or themselves, Elite Learning reserves the right to discontinue their enrollment.
Bullying
Elite Learning does not tolerate bullying under any circumstances. We define bullying as a repeated and intentional act that causes mental or physical harm and creates an abusive environment.
All repeat bullies will undergo disciplinary action which includes consequences such as restrictions, conferences, suspensions, and/or disenrollment.
Disenrollment
If a child displays a consistent inability to meet the standards of the program or if they are creating an unsafe or disruptive environment for the other children and/or Elite Learning staff, Elite Learning will reserve the right to remove the child from the program immediately.
Emergencies and Inclement Weather
Emergencies, Delays, Dismissals, Etc.
During emergency situations such as inclement weather, unforeseen facility issues, etc., the Elite Learning program will not be offered if the school is closed for the day. The Elite Learning before-school program will not take place if the school day is delayed.
When the Elite Learning program is not offered for any reason, parents will be notified as far in advance as possible to arrange for alternate care. Please monitor local news media outlets and the district website for more information in these situations.
Inclement Weather Policy
If weather conditions indicate a school day delay or cancellation, parents will be notified by the school district and/or by Elite Learning Program Director.
When a school campus cancels its own afterschool activities, Elite Learning program at the same campus may also get canceled. If the Elite Learning program is canceled due to inclement weather, parents will be notified via phone or email. For the safety of the staff and students, we request that parents pick up their children as soon as possible on days that we have to finish our program earlier due to inclement weather, facility issues, or any other unforseen situations.
Elite Learning will not be able to offer childcare in the event of school closure during the school day.
Other
Personal Items
We ask that children refrain from bringing toys, electronic devices, or any nonessential personal item that may be a distraction to their activities. Phones and any other personal electronic devices are not allowed during the Elite Learning program. If these items disrupt any activities, they may be confiscated by staff members and released only to the parents.
Personal belongings must be taken home from all areas at the end of each day. Anything left behind will be placed in the lost and found box. Our staff members will try their best to help students locate their lost property but cannot be held responsible for lost items.
School Attire
Children must abide by normal school dress codes. Shoes must be worn at all times. If a student is not wearing proper footwear, they may be asked to sit out during physical gym activities or games. We do not allow children to participate with bare feet.
Elite Learning
1. All parents/guardians are required to pay registration fees and the first month's tuition (prorated if applicable). If you leave Elite Learning (E.L.) and wish to return, you will be considered a new applicant and must register again and pay the registration fee. Accounts will automatically be charged for the aforementioned fees, but payments must be made via Parent Portal. Registration Fees are $50 per child or $75 for an entire family (2 or more kids); registration fees are non-refundable/non-transferable.
2. Parents are responsible for all tuition and fees outlined below or applicable to their accounts, and for keeping their information current. Parents must select from only one of our two payment options at the time of registration. All payments whether online or mailed must be received by the 1st of each month.
3. Options for Payment
a. Auto-Payment by debit or credit card. When registering online, parents input their card information. The account is automatically deducted each month on the 1st.
b. Payments by paper check or money order. This choice requires an initial deposit of one-month tuition, plus the registration fee and the first month's tuition (prorated if applicable). The deposit will be used as payment for the child's last month in the program.
Paper checks or money orders must be mailed to the main office.

4. Discounts: Discounts cannot be combined.

• School employees may have special discounts based on specific agreements with the campuses/districts.
• Documentation may be requested to verify any and all discounts.
5. The parent/guardian will be required to enter the building when picking up their child. For your child's protection, parents are required to sign out via our secure electronic attendance system. Only authorized adults with a valid ID will be allowed to pick up your child.
6. The parent/guardian agrees to pay a $1 per minute late pick-up fee after program scheduled pick up time. The time is determined by the clock at The After School Program location. Late pick-up fees are added to the account and must be cleared by the next time tuition is due. Unpaid late pick up fees and excessive number of late pick ups may result in removal from the program.
7. Additional Fees: Tuition is due on the 1st of each month, regardless of when the 1st day falls on the calendar. A $30 late payment fee will be charged to accounts that hold a balance on the 5th of each month. NSF (non-sufficient fund) Charge: A $35 fee is charged for all non-sufficient funds/returned checks/credit card chargebacks. In the event of a payment return, a parent will be required to pay online manually or pay by money order by the 10th of each month or their child may be dropped from services for non-payment. I certify that I have read the above tuition information and I will abide by all the requirements listed under Parent Agreement regarding Late Fees, Late Pick-up Fees, Non-Sufficient Fund Charge and Withdraw/Drop Services Policy.
8. Payments not received by the 10th of the month will result in termination of enrollment. Parents who have not contacted Elite Learning main office for payment arrangements will be responsible for making arrangements to pick up their student at school dismissal time.
9. Both my child and I agree that when he or she is participating in the Program, that he or she will cooperate promptly and fully with all directions of E.L.'s personnel. Participation in the program may be terminated for inappropriate student behavior. Any of the following may be grounds for termination: Defiance of authority; disregard for the safety of oneself or others; foul language/gestures; use of derogatory language with respect to race, ethnicity, or origin, and other offenses. Participation in the program may be terminated if parent(s) displays similar inappropriate behaviors, use threatening language or behavior toward staff, or fail to follow sign-in and sign-out procedure.
10. If I intend to withdraw my child(ren) from the program, the Withdrawal Form must be completed a minimum of 10 business days (M-F) before the student's official final day. The monthly tuition is not refundable for withdrawals occurring with less than 10 business days before the student's final day. If I choose to re-enter the program after terminating this agreement, I understand that I must pay all outstanding fees and balances before my child(ren) is placed in the program again.
11. I will make other arrangements for emergency closings, delays, and dismissals. There is no tuition discount for absences, holidays, or other days on which E.L. is closed. If schools are closed due to inclement weather or other special circumstances, the After School Program will also be closed.
12. While every attempt will be made to protect your child's personal property, The After School Program will not be responsible for any articles brought to the program.
13. I will complete all necessary forms before my child attends the E.L. Program. I will notify E.L. of any changes in registration information (e.g. address, phone number, email, etc.). I will pay all outstanding balances or unpaid returned checks from the previous semester or school year prior to my child's registration.
14. I understand that it is my responsibility to provide E.L. with information regarding any special accommodation that my child requires.
15. LIABILITY WAIVER: I agree to hold harmless the Elite Learning, and its employees from any and all loss, damage, liability, cost, or expenses arising out of or resulting from the use of equipment or property owned by or under the control of the E.L. I also understand that the E.L. does not carry insurance to cover participants in the activities in which I, or my child is participating in. I understand there are risks associated with these activities, and I assume the risk of any injuries that I, or my child may sustain during any of the activities. I forever discharge the E.L., and its employees from any lawsuits, damages, claims, or judgments resulting from any personal injuries or property damages that I or my child may sustain while using E.L. property or equipment, or while participating in an activity sponsored by the E.L.
Homework Policy
Elite Learning is committed to furthering your child's educational development, but our afterschool program is not an extension of the academic school day. We encourage all students to independently finish their homework during our afterschool program by ensuring quality quiet time. If a child refuses to work on his/her homework, our staff cannot force him/her do so. Our instructors may provide assistance -however, our staff are not equipped to offer extensive one-to-one homework time assistance and therefore Elite Learning is not responsible for any inaccuracies or mistakes in your child's work.
