Please have your dancer to class on time every week since regular attendance is necessary for good training.
Follow drop off & pick up instructions. Dancers will not be allowed out the doors until you pull up in the pick up line or come inside to pick up your dancers. If everyone follows our map the line moves much more efficiently.
Pick up your child on time after class, dancer are not allowed to stay more than 15 minutes once their class is over.
Label dance shoes and gear with student's full name, we can easily return items with names. Please do not allow your child to wear dance shoes outside, you pay a lot of money for dance shoes, they will last longer. We suggest a dance bag with name & number for all your dancer's gear.
Please make sure that your child is properly dressed for class, this includes shoes, clothes and hair. When dancers are not dressed properly it is a distraction, dancers will be given a warning, dancers excessively not dress properly will not be allowed to participate. We do understand unexpected circumstances arrises, please communicate with us.
If your dancer is going to miss several classes please let us know, we worry when dancers miss several classes.
Please encourage practice at home.
Please limit food and drink brought in to the studio, food will only be allowed in the cafe area. We expect dancers to clean up after themselves, please remind your dancer to do so. Dancers will not be allowed to take food in to the dance room, closed bottles of water are okay.
Please watch your other children who are waiting in the lobby with you; keep them safe and under control in the lobby.
If you wish to speak to your child's instructor, please set a time for them to call you. Many times they need to start their next class.
Students and parents should be respectful in the lobby area, using their "inside voice" and keeping voices down. Please do not move furniture.
Please keep students' jewelry and valuables at home.
Please make sure there are two good phone numbers on your dancers account.
PLEASE check your email for updates and make sure you are signed up for remind text messages text @companyd to 81010 or enter @companyd in your remind app. Please understand if you choose not to sign up for remind messages you will miss important Company D news and information.
Check the lost and found box frequently, lost & found items will be donated to charity once a month.
Please pay accounts on time. Late fees are issued on overdue accounts.
Please review our student expectations with your dancer to help them understand why it is important for everyone to follow the studio rules.
I understand my dancers will be required to wear the proper dance attire to all dance classes
I understand classes times may change in preparation for recital.
I understand Company D has the right to change or update these policies as needed.
RELEASE OF LIABILITY AND INDEMNITY AGREEMENT?
FOR AND IN CONSIDERATION of the mutual covenants contained herein and the provision of dance instruction to my child or children, it is agreed by the signatory hereto, acting by and on behalf of myself and my child that COMPANY D DANCE CENTER LLC, its agents, servants, and/or its employees shall never be legally liable or responsible for accident, injury, property damage or death sustained on COMPANY D DANCE CENTER LLC premises or in connection with COMPANY D DANCE CENTER LLC activity, whether due to the sole, joint or concurrent negligence of COMPANY D DANCE CENTER LLC, its agents, servants and/or employees from any claims, demand and/or cause of action relating to injury and/or death and/or property damage, whether sounding in contact and/or tort, at law or in equity, by statute or at common law and whether or not due to the sole, joint or concurrent negligence or violation of law, by COMPANY D DANCE CENTER LLC.
By enrolling your child with Company D Dance Academy you are agreeing to the following
I understand Pre-Registration, Registration Fee, Recital Fees & costume fees are non-refundable
I understand Recital fee will be due in September, the fee $45 (returning dancer) $55 (new dancer) per dancer
I understand Company D accepts payments for tuition and costume in the form of Cash, Credit Card, Auto Draft via Credit or Debit Card, no Checks will be accepted. ALL ACCOUNTS WILL BE REQUIRED TO HAVE A CREDIT CARD ON FILE.
I understand tuition is due on or before the 4th of the month. A Late fee of $15 will be charged on the 5th of the month regardless of what day of the week is the 5th. If tuition payments are not received prior to the 20th of the month another late fee of $25 will charged to my account.
I understand that funds applied to an account will be applied to the oldest due date first.
I understand all accounts are considered active and fees will be charged until Drop form is filled out and submitted to the front desk In order to avoid being charged for the next month's tuition classes must be dropped prior to the 20th of the previous month. If class is dropped on the day tuition is due the account holder will still be responsible for that months tuition.
I understand Costumes will not be ordered if your costume deposit is not received by October 25, 2019
I understand I will be responsible for costume balances if a dancer drop class/classes after Dec. 1, 2019
I understand if my account is two month delinquent my dancer/dancers will not be able to attend class/classes until the account is paid.
Company D does not have an excused absence policy. Total monthly tuition is due even if your dancer is absent.
Tuition is based on a minimum of 30 class over the entire dance season.
I understand that by enrolling my dancer I am the financially responsible party and my dancer's enrollment with not be complete without all required information, Parent Portal sign up and the Enrollment Agreement, Forfeiture of these terms could affect your credit score.
