I understand that for the safety of all staff and students all gym policies and staff instructions must be followed or the student may be asked to leave the class.
There is NO gum, food, or drinks (other than water) allowed in the gym area. All personal effects should be stored in the provided cubbies during class sessions.
Please drop-off your student no earlier than 5 minutes prior to their class time, and be available to pick them up promptly when class is over. Students who cannot go to the bathroom independently must have a parent (or responsible party) present during their class session.
Parents are welcome to view classes anytime, but spectators must stay in the viewing area for the safety of all. The exception: an adult must accompany each child in the Playful Parenting class, and in doing so they accept the "Assumption of Risk" below. Adults (other than instructors) are never allowed on the gymnastics equipment, and adults must be enrolled in an appropriate class to utilize other equipment.
To prevent injury and/or damage to the equipment, there should be NO buttons, zippers, snaps, skirts, or strings on clothing, and NO jewelry in the gym area. Midriffs and cleavage should be concealed. Clothing should be well-fitting, not baggy, to avoid catching on equipment or hindering movement. Long hair should be tied back.
Athletic Performance Students
Compression shorts under athletic shorts are recommended to maintain modesty. Athletic shoes should be clean. No cleats unless specifically requested by the class instructor.
We prefer leotards for girls, with snug shorts or pants recommended. Boys can wear shorts or pants with a t-shirt that can be tucked in. Gymnastics students will enter the gym area barefoot.
I hereby give consent for Boundless Movement Academy, LLC to use each student's photograph and likeness, including videos taken during class sessions, to be used in its publications, including its website and all social media outlets.
As a legal adult participant and/or as the legal guardian of my designated student(s), I hereby consent to my own and/or my student(s) participating in this facility's program(s). I recognize that potentially severe injuries, including permanent paralysis or death can occur in any activity involving height or motion, including tumbling, gymnastics, strength, agility, and power training, related activities, and physical activity in general. I understand that it is the express intent of all staff and personnel to provide for the safety and protection of all students and, in consideration for allowing myself and/or my student(s) to use these facilities, I will not hold Boundless Movement Academy, LLC and/or its employee(s) liable if injury occurs to myself or my child while participating in activities connected with Boundless Movement Academy, LLC.
To the extent permissible by law, I hereby release, discharge and/or otherwise hold harmless and indemnify Boundless Movement Academy, LLC, its owners, officers, directors, employees and associated personnel, from and against any and all demands, claims and causes of action arising, directly or indirectly, from the student's participation in its programs.
THIS RELEASE SPECIFICALLY INCLUDES ANY DEMANDS, CLAIMS AND CAUSES OF ACTION ARISING OUT OF THE PAST OR FUTURE NEGLIGENT ACTS AND/OR OMISSIONS OF BOUNDLESS MOVEMENT ACADEMY, LLC, ITS OWNERS, OFFICERS, DIRECTORS, EMPLOYEES AND ASSOCIATED PERSONNEL.
In the event of an accident or illness, I hereby give my permission to trained medical professionals to administer medical treatment to the student (myself or my child) if I am incapacitated or not present.
Registration Fee: Each student is charged an annual, non-refundable, registration fee. Multiple students in one family receive a discount on this fee.
Tuition Back Guarantee: If after the student's first month of classes, you are not satisfied for any reason, we will refund 100% of your tuition for the entire month without hassle.
Tuition:Tuition amounts are charged monthly and are automatically deducted through our automatic payment system. Payment for the first month will be charged when you sign up for the class to reserve your spot. For each subsequent month, families will incur charges on their autopay accounts on the 1st business day of each month for that month's classes. If you opt to use an alternative means of payment, payments are due on the 21st of each month for the next's month's classes. A $7 late fee is charged to all payments made after the 21st due date (the late fee is not added to autopay accounts).
Failed Transactions: In the event that a payment is returned as NSF your family account will be charged a fee of $35.
Drop Procedure: You must notify the office (by email, phone, in person, or through the portal) to drop a student from a class. *social media communication is not a sufficient means of notification
Please note: You are responsible for payment for your student's classes WHETHER OR NOT YOUR STUDENT ATTENDS CLASS until the time you notify the office. Do not rely on your student or their coach to verbally let us know that he/she will no longer be attending classes.
I represent and warrant that if I am purchasing something or paying for a service from this facility or from other merchants through this facility that (i) any credit card or bank account draft (ACH/Bank Draft) information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.
I hereby authorize this facility to charge my ACH/Bank draft, or credit card account for monthly tuition and any other outstanding fees, i.e. registration fees, camp fees, merchandise balances, etc. I understand that notice must be given by the 20th of the month to terminate billing, and I am responsible for payment whether or not my student attends classes until I notify this facility to drop my student from class(es).
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.
