Policies, Terms and Conditions
Tuition and Registration: The annual registration fee is $25.00 for each student. There is a $10.00 material fee per quarter and a recital fee of $35.00 for each student. Recitals are scheduled in June and December yearly. The tuition fee is based on enrollment, not attendance.
Dandan Performing Arts Center offers open enrollment for private lessons; therefore, you may register for classes at any time. If a student begins lessons in the middle of the quarter, we will pro-rate for the number of lessons remaining before the end of the current quarter.
The tuition and registration fees are non-refundable and non-transferable.
Cancellation fee: $ 100.00
Payments: Full payment is due at the beginning of the quarter. We offer class roll-over and automatic billing.
Late fees: We have a late payment fee policy of $15.00 per student. It is the parent's responsibility to ensure we have been provided a valid credit, debit card, or ACH information if enrolled in automatic billing.
Delinquent Accounts: The Dandan Performing Arts Center reserves the right to suspend or discontinue instruction of any student who is delinquent in fulfilling his/her financial obligations at any time.
Refunds and returns: There are no refunds on tuition fees, material fees and registration, no returns or refunds on retail merchandise.
Attendance policy: Regular and punctual attendance is one of the best investments students can have in their music education. Communication about presence is an essential part of the teacher-student relationship if a student is going to be absent for a lesson; he or she must notify the studio 24-hours before their lesson by sending an email to absent@dmajorarts.org. Teaching artists are not required to make up student absences.
The majority of the Dandan Performing Arts Center's teaching artists are independent contractors. If the student doesn't show up for their lesson, the teacher is still committed to the school for your appointment time. Hence, lessons are paid even when the students don't show up. All our teaching artists are dedicated and committed to the success of your child.
Student Illness Guidelines: While we encourage students to attend all of their lessons, classes, and ensembles, an ill student will not perform well and may expose others to their illnesses. If a student is sick enough to stay at home from school, please do not send your child to their music lessons.
Tardy Arrival: Students arriving late will not have their lesson time extended. Students arriving late by more than half of their lesson period will be considered absent, and their teaching artist will be dismissed. If the teaching artist is late or absent, a substitute teaching artist may be provided, or a makeup lesson will be scheduled.
Missed lessons: Students pay tuition regardless of their attendance. Students must send an email to absent@dmajorarts.org before any planned or unplanned absence, but this does not exempt the student from payment. Teaching artists are not required to make up missed lessons caused by student absences. Teaching artists may make-up lessons caused by their absences, or a lesson credit will be applied to the student's account. The lesson credit has to be used within 30 days.
Discontinuation: The Dandan Performing Arts Center faculty reserves the right to discontinue a student's enrollment at any time for reasons including, but not limited to, poor attendance, non-payment of fees, lack of interest, or disruptive behavior. If a student is dropped for non-payment of fees, the student might be able to continue the quarter by paying the unpaid balance.
Termination of lessons: Tuition is NON-REFUNDABLE. If you terminate your lesson prior to a 30 day notice, you will still be charged for the remainder of the lessons. We require that you notify us in writing 30 days prior to the termination of the student's lesson.
Proficiency and Performance: Performance opportunities are scheduled throughout the year as a chance to share the student's hard work and accomplishments with family and friends. As performing is a vital part of music education, recital, rehearsal attendance and involvement is strongly recommended
In addition to the feedback, students regularly receive from their teachers; it is crucial that they get input and suggestions from other music professionals as well. For this purpose, Dandan Performing Arts Center schedules proficiency evaluation once a year.
Dismissal: The Dandan Performing Arts Center reserves the right to dismiss any student due to frequent absences, disciplinary problems, overdue tuition payments, or parental noncompliance with policies.
Inclement weather: The studio may close due to weather or unforeseen events, students are invited for special make-ups, which could include private or group lessons.
Holiday Closures: The holiday and break closures is posted on the website calendar. No makeup lessons are given for holiday closures. You will be notified in advance of any exceptions.
Student Drop-off: When bringing your child to music lessons, make sure you have signed in, and your child makes it to their lesson, with the teacher present. Do not drop your child off in the parking lot or leave them unattended in the waiting room. Neither the Dandan Performing Arts Center nor its teachers are responsible for unsupervised students.
Child Safety: For your child's safety, children will not be allowed to wait in the reception area unattended. It is expected that everyone waits quietly as not to disturb ongoing lessons. Please do not drop off students more than 10 minutes before lesson time and pick them up promptly. The Dandan Performing Arts Center faculty and staff do not supervise children waiting in the reception area before and after classes/lessons. No food or drinks are permitted during the lesson. No roughhousing is permitted. Behavior that is in any way, unruly or disruptive to other students is unacceptable.
Release: Any photographs, audio, or videos of students or families on the Dandan Performing Arts Center facilities and events may be used in promotional publications, websites, or press releases. There will be no compensation for the release.
Non-Discrimination Policies: The Dandan Performing Arts Center complies with all laws prohibiting discrimination in the conduct of its operations and programs. Dandan Performing Arts Center admits students of any religion, gender, race, color, sexual orientation, socioeconomic standing, national and ethnic origin to all rights, privileges, programs, and activities generally accorded or made available to students at the studio.
Policy changes: The Dandan Performing Arts Center reserves the right to change policies, fees, or waivers at any time.
I acknowledge this guide as a current studio policy and agree to everything listed above. If any questions arise, all parties will communicate with one another to resolve any problems and work hard to become polished musicians.

Media Release Form
I grant permission to Dandan Performing Arts Center (herein referred to as the "DPAC") to publish any materials provided by me or taken by a representative on behalf of Dandan Performing Arts Center, including images, artwork, videos, recordings, presentation materials, or interviews and quotes (herein referred to as "materials"). I understand that DPAC may use these materials to help disseminate its mission and programs to the public. I understand that any materials may be published in digital and print form, including but not limited to brochures, flyers, web pages, videos, PowerPoint presentations, and ads. I waive any right to review or approve the finished products.
I acknowledge the DPAC's right to treat the material at its own discretion and understand that DPAC may choose to use the content now or at a later date. I release all copyrights to the materials and revoke my rights to any compensation. I hereby hold harmless and release DPAC from all claims, demands, and causes of action, which I may have against DPAC by reason of this authorization.
Individual Waiver Form
Assumption of Risks, Release of Claims, Indemnification and Hold Harmless Agreement(2020 Individual Waiver Form)
The parties to this Agreement are Participant, Participant 's parents or legal guardian, if Participant is under 18, all referred to hereafter jointly and severally as "Participant") and the Board Members of Dandan Performing Arts Center, its officers, trustees, faculty, agents, representatives, volunteers, students and employees ("DPAC") for the ("Class") ("DPAC" and "Class" are collectively referred to hereafter as "Released Parties").
Participant is a voluntary participant in this class. Participant understands and agrees that such activities may be dangerous, may involve travel (local, domestic and/or international) and that neither the class nor DPAC can guarantee the safety of Participant. Participant shall not engage in inappropriate conduct including the use of physical force or verbal abuse, abuse of the customs or mores of the community, and unauthorized absences from classes or other activities. Participant is apprised that Released Parties shall not be subject to claims or suit to be made by or on behalf of Participant or Participant's heirs, representatives or assigns as a consequence of Participant's participation in the class.
Assumption of Risk. Participant expressly understands and agrees that the class presents risks to Participant and her/his property. These risks can include, among others (by way of example and without limitation): dangers associated with swimming and drowning, manmade and natural jumps; dangers of collision with pedestrians, vehicles, and fixed or moving objects; the dangers arising from surface hazards, including pot holes or other ground or pavement depressions or height differentials, equipment failure, teammates' or co-participants' negligent or wrongful conduct, inadequate safety equipment or training, use of equipment or materials provided by the class organizer, host and others; unfamiliar or different terrain; customs, climate, food and drink; civil unrest, terrorism, kidnapping and assault; laws; social and sexual mores; personal safety; driving practices; sports practices, rules and regulations; communications; criminal and law enforcement activities; disability access; driving practices, road, premises conditions and/or maintenance; disease risks; inadequate health care; injury to the head, neck or spine; injury to the muscular or skeletal systems; injury to internal organs; scratches, bruises, strains, sprains, contusions, falls, fractures; physical violence; verbal abuse; sexual abuse or other forms of harassment by co-participants; loss or damage to sight, teeth; other body parts or hearing; paralysis; concussions; brain damage; long and/or short-term disability; loss of income and/or career and earning opportunities; minor or serious injury and/or death or other risks, including risks related to explosion, fire, chemical exposure and/or risks associated with field trips. Participant is responsible for researching and evaluating the risks he/she may face and is responsible for his/her actions. Any activities that Participant may take part in, whether as a component of the Class or separate from it, will be considered to have been undertaken with Participant's approval and understanding of any and all risks involved. This includes, but is not limited to, risks associated with the consumption of alcoholic beverages and/or drugs or other intoxicants (whether legal or illegal), property loss, injury to person or property, or death arising out of traffic accidents, assault, and theft or other activities. It is Participant's intention that this assumption of all risks shall be legally binding and a complete bar to Participant, Participant's heirs, personal representatives, relatives and assigns. This assumption of risk applies to all activities arising out of, associated with or resulting directly or indirectly from Participant's participation in the Class, including but not limited to those risks listed above. Participant further recognizes, understands and agrees that the Released Parties assume no responsibility for any liability as regards damage or injury that may be caused by Participant's negligence or willful acts committed prior to, during or after participation in the Class, or any liability, damage or injury caused by others, including other participants.
Adherence to Standards. Participant understands and agrees to abide by all DPAC policies, rules, and regulations and to all other laws, rules and regulations, including all office, laboratory and/or general rules and regulations.
Release of Claims. In consideration of being accepted into and/or participating in the Class, Participant agrees for Participant and on behalf of Participant's heirs, executors, administrators, employers, agents, representatives, insurers, and attorneys, to release and discharge DPAC of and from any and all claims which may arise from any cause whatsoever, including any negligent act or omission by DPAC or others. Participant further releases and discharges DPAC from liability for any accident, illness, injury, loss or damage to personal property, or any other consequences arising or resulting directly or indirectly from Participant's participation in the class. The Participant 2020 Individual Waiver acknowledges and agrees that the Released Parties assumes no responsibility for any liability, damage, or injury that may be caused by Participant's negligent or intentional acts or omissions committed prior to, during, or after participation in the Class, or for any liability, damage, or injury caused by the intentional or negligent acts or omissions of others, including other participants. Participant intends that both the assumption of risk and the release of claims be complete defenses to any and all actions, claims or demands that Participant, Participant's heirs or legal representatives have or may have for injuries to person or property, including death, as a result of activities for which the participant has assumed risks and/or released and/or waived claims.
Indemnification and Hold Harmless. Participant hereby agrees to indemnify, defend, and hold harmless Released Parties from any injury, loss or liability whatsoever including reasonable attorneys' fees and/or any other associated costs, from any action, claim, or demand that Participant, Participant's heirs or legal representatives, has or may have for any and all personal injuries Participant may suffer or sustain, regardless of cause or fault as a result of, arising out of, associated with, or resulting directly or indirectly from Participant's voluntary participation in or decision to participate in the Class, travel to and from the Class and any and all related activities, on or off of DPAC's campus. This Indemnification and Hold Harmless Agreement is intended to be all encompassing. Physical Condition and Insurance. Participant attests that he/she is physically and mentally capable of participating and has no known health restrictions that might jeopardize his/her safety or health or the safety or health of others during their participation in the Class. Participant gives permission for DPAC or its representative to provide immediate and reasonable emergency care should it be required. Participant agrees to be solely responsible for payment in full of all costs of medical care he/she may receive.
Activities Outside the Class. Should Participant choose to remain at the Class location or elsewhere either before or after participation in the Class then this Agreement shall remain in full force and effect.
Class Modification and Cancellation. DPAC reserves the right to cancel or modify the Class before or during its operation for any reason, including emergencies, low enrollment, or unavailability of facilities or personnel.
