Sarah McLachlan School of Music - COVID-19 Pre-Screening + Operating guidelines

Parents/Guardians must read and agree to the information contained in this agreement text prior to their student/s entering the Sarah McLachlan School of Music (SoM).

As parent / guardian I understand that I will not send my child to SoM if they are experiencing any of the following symptoms:

Fever, Chills, Cough or worsening of chronic cough, Shortness of breath, Sore throat, Runny nose, Loss of sense of smell or taste, Headache, Fatigue, Diarrhea, Loss of appetite, Nausea and vomiting, Muscle aches, Stuffy nose, Conjunctivitis (pink eye), Dizziness, confusion, Abdominal pain, Skin rashes or discolouration of fingers or toes.

I understand SoM staff reserve the right to send students home if they are not respecting safety protocol, or exhibit any Covid-19 symptoms. A parent/guardian will pick-up any student in these situations if possible.

I acknowledge that SoM and its employees will not be liable for contraction of COVID-19 arising from participation in classes, private lessons, workshops, or general use of the school.

At the Sarah McLachlan School of Music (SoM), we believe in building a community where every individual feels safe and secure to be themselves and to explore who they are through the study of music. Each student must recognize a personal responsibility to learn and follow the SoM values at all times. Failure to follow these values may result in dismissal from our program.

I will try my best to attend ALL of my SoM classes and be sure to call/text/email the front desk to let them know about any absences. I understand that if I have 3 or more unexcused absences I may be asked to attend a meeting with my instructor to discuss my commitment.

As the parent/legal guardian of my child (outlined on this form), I hereby grant to Sarah McLachlan School of Music and any employees, licensees, and assigns of Sarah McLachlan School of Music, for the purpose of advertising and promoting Sarah McLachlan School of Music, the right and permission to use and re-use, in whole or in part, my or my child's first name, voice, image and likeness, as well as any recording, music produced, soundtrack, artwork, film or footage in which my child's name, voice, image or likeness appear, modified in any way, without payment or notice to me. I agree that photographic and video images may be taken by SoM of my child participating in SoM activities during the course of their involvement in SoM and I hereby agree that such images may be used by SoM in SoM's sole discretion to advertise and promote SoM's programs and to raise awareness of SoM's programs.

In addition to attendance at the Sarah McLachlan School of Music's program facility that may require transportation, throughout the program year students of the Sarah McLachlan School of Music may also participate in a number of performances. Once signed, this waiver gives your consent and allows your son/daughter to be transported by bus, taxicab or passenger van to all performances at offsite locations inside the Lower Mainland during the program year. Separate permission slips will be issued for those performances that require travel outside the Lower Mainland, or that are scheduled during school hours.

WHEREAS:
a) The Sarah McLachlan School of Music is a charitable organization offering music programs to children in the Lower Mainland.
b) The Sarah McLachlan School of Music's facility is located at #200-138 East 7th Avenue Vancouver, B.C. ("Facility").
c) From time to time, attendance at the Sarah McLachlan School of Music's programs both at the Facility and elsewhere requires transportation of children by vans or vehicles that are leased by a third party transportation company ("Vehicles").
d) The undersigned is desirous of having their children participate in the Sarah McLachlan School of Music's programs that require transportation by the Vehicles.
e) The Sarah McLachlan School of Music's ability to compensate the undersigned or the undersigned's child or children for any loss, cost, damage, expense or injury incurred while being transported in the Vehicles is limited to the insurance coverage available to the Sarah McLachlan School of Music for such purpose ("Insurance").

THE PARTIES AGREE AS FOLLOWS:
1) The Sarah McLachlan School of Music agrees to transport the undersigned's child or children from time to time in the Vehicles to and from Sarah McLachlan School of Music programs at the Facility or elsewhere, as may be required by the child's or children's attendance at the Sarah McLachlan School of Music's programs.
2) The undersigned on their own behalf and on behalf of their child or children who might be transported from time to time in the Vehicles hereby waives and hereby agrees to hold harmless the Sarah McLachlan School of Music from and against any and all claim or claims, action or actions, cause or causes of actions, demands, costs, expenses and damages of every nature and kind whatsoever which the undersigned or the undersigned's child or children may have against the Sarah McLachlan School of Music or its directors, officers, servants, agents or employees for and in respect of or in any way arising out of the transportation of the undersigned's child or children in the Vehicles, save and except such liability of the Sarah McLachlan School of Music, its directors, officers, agents, or employees as may be covered by the Insurance.
3) It is understood and agreed that this Waiver and Hold Harmless Agreement is applicable even in the event of the negligence of the Sarah McLachlan School of Music or any of its directors, officers, employees, servants or agents.
