COVID 19
Due to the COVID-19 pandemic, Dance To Life is taking full precautions and will follow the CDC guidelines for dance studios. We ask that if your child is displaying any of the COVID-19 symptoms PLEASE keep them home. To ensure the safety of the campers and all stakeholders, each person entering in the studio will be
1. Temperature Checked : Thermometer reading of 100 degrees or higher WILL NOT be permitted into the studio until the temperature reads satisfactory.
2. Parents MUST complete the CDC Guidelines Questionnaire daily upon drop off.
3. Hands will be sanitized upon entering the studio.
4. Students MUST have a clean mask.
5. Hand washing will be frequent to avoid the spread of germs. We are asking that each camper has his/her personal hand sanitizer.
6. The studio will frequently sanitize all common areas throughout the day.
Monthly Tuition
*Monthly tuition will be AUTOMATICALLY WITHDRAWN on fifth (5) of each month through the JackRabbit Parent Portal.
*Tuition is the same each month regardless of holidays.
*I agree and understand that payments will be withdrawn from my financial institution every Monday for the 8 week session.
*I agree to pay any and all fees associated with the collection of any outstanding balances on my account.
*I understand that all tuition payments are non-refundable and non-transferable.
Payment of Tuition
*The 2020/2021 dance season registration fee is is waived when you register.
* A VALID credit or debit card must be on file to PERMIT AUTOMATIC WITHDRAWLS .
*Your first month's tuition will be withdrawn on September 5th, 2020., J
*If funds are not available by the end of day, A late fee of $10 is charged.
* Students will not be permitted to attend class whose outstanding tuition is not paid by 15th day of the month.
* I understand, after Wednesday, any overdue balances will result in a NO PARTICIPATION notification.
Dress Code
Dancer's Dress Code The dress code will be STRICTLY ENFORCED. Youth dancers out of dress code will not be permitted to participate. ADULT STUDENTS are also required to be in dance attire. Sweat pants and oversize t-shirts, are STRONGLY discouraged.
Ballet Attire
EGGPLANT Leotards and FLESH TONE (ballet skirts are optional but must be in the color list above) Shoes: ballet shoes required.
Jazz/ Lyrical/Liturgical Attire
EGGPLANT Leotards and FLESH TONE with all black dance pants or shorts Shoes: jazz and lyrical shoes required.
Tap Attire
EGGPLANT Leotards and FLESH TONE with all black dance pants or shorts Shoes: Black tap shoes required.
Hip Hop Attire
EGGPLANT Leotards and FLESH TONE with all black dance pants or shorts. Shoes: Hip Hop sneakers, or clean bottom gym shoe
Preschool ONLY
Ballet Attire
LAVENDAR Leotards and FLESH TONE (ballet skirts are optional but must be in the color list above) Shoes: ballet shoes required.
Dirty and/ or torn dance attire will not be tolerated and will result in the dancer not participating in class.
Hair
FOR BALLET hair must be secured in a ponytail chignon or bun. All other classes hair must secured away from the face preferably in a ponytail.
Technology
ALL DANCERS all electronic devises must be placed on silence and stored. If observed not stored devices will be confiscated until the end of classes.
Media Release
I hereby give permission to Dance To Life, LLC to take photographs and/or videos of students listed above that will become permanent/sole property of Dance To Life, LLC. I consent to the use of such materials for promotional purposes by Dance To Life, LLC.
Behavior
I understand that disruptive behavior (rude comments towards students, parents and Dance To Life, LLC instructors, physical contact with other students, the lack of discipline during class, etc.) may require the instructor to dismiss a student from class. I agree to abide by all Dance To LIfe, LLC policies and will direct any comments or suggestions directly to the owner.
Before/After Class Supervision
Parents whose child arrives before and/or remain after their schedule dance time will be assessed a Supervision Fee of $10 per hour, beginning after the initial 15-minute grace period.
Dancer Drop Off / Pick Up
Due to limited space and potential distractions students will not be permitted to enter the facility until 5 minutes before class starts. Drop off will be through the front entrance Pick-up will be through the rear exit.
Additional Cost
I understand RECITAL and COSTUME fees are separate from monthly tuition . These fees are required if participating in the end of year recital. I further understand, I have the OPTION to pay these fees in my monthly tuition.
Withdrawing Policy
I understand a formal written letter must be submitted 30 DAYS PRIOR to the new billing cycle if withdrawing from classes. I further understand, If I do not submit a formal letter within the aforementioned time I will be REQUIRED TO PAY THE TUITION incurred for the new billing cycle.
Participant Injury
I, the undersigned, for myself, my heirs, administrators, and executors, herby waive and release Daneen and Jamie Brown individually and Dance To Life, LLC and directors, employees, instructors, guest instructors and volunteers from any and all claims, cause of action, suits or liability, costs and expenses (including, without limitation, reasonable attorneys' fees) arising out of, or in connection with, any loss, personal injury, death, or other damage that I may incur or suffer by reason or as a result of my enrollment or participation in classes, events, performances, and private lessons with Dance To Life, LLC. I expressly assume the risk associated with dancing and choreography and all over events and programs and performances sponsored by Dance To Life, LLC. I understand that risk of injury is inherent in any physical activity and I knowingly and voluntarily accept that risk. I certify that I am in proper physical condition to participate in the exercise/dance program and that I have been examined by a licensed physician and found to be in proper physical condition to participate in said program I, the undersigned, do herby authorize Daneen and Jamie Brown or their designated agents (being teachers or administrators employed by Dance To Life, LLC) to obtain medical treatment for myself in emergency situations if needed. I understand that I am responsible for any medical expenses and that the absence of health insurance does not make Dance To Life, LLC responsible for payment of medical expenses. This authority includes the power to authorize any and all treatment deemed necessary under the circumstances by a licensed physician. This power is in essence a power of attorney and shall remain in effect for one year from the date signed below.
