Please read this document carefully. It must be signed by all Mount Hermon After School Adventure Participants. Since the participant is a minor, at least one parent or guardian must also sign as evidence of their agreement to these terms and conditions on their own behalf and on behalf of the minor.
1. I acknowledge that I have voluntarily applied to participate in the Mount Hermon After School Adventure program operated by Mount Hermon Association, Inc. which can be a physically demanding and/or challenging program. Students may participate in standard After School Adventure activities with manageable medical conditions.
2. ACKNOWLEDGEMENT OF RISKS
I understand that the Mount Hermon After School Adventure at Mount Hermon may expose participants to certain risks. The activities require moderate physical exertion and may be conducted at heights up to 40 feet (climbing wall, tree climbing) Among the hazards and risks of the activities and use of the premises and equipment are the following: falls; collisions; abrupt and possibly harmful contact with structures, objects and persons; anxieties and fears associated with heights; close contact with other people; coordination and misjudgments on the part of participants; the failure of structures or equipment; and the unpredictable forces of nature. Participants may experience increased heart rate and other symptoms of anxiety and stress due to physical exertion, reliance on other participants, a fear of height, or of unprotected falling, loss of balance, coordination and misjudgments, including failure to follow procedures and instructions, physical or mental or psychological stress, fatigue, chill and /or dizziness which may diminish reaction time and increase the risk of an accident. Injuries associated with participation may include breaks, sprains, bruises, and in extreme cases, emotional upset, anxiety and even death. Participants acknowledge that the description of risks is not complete and that other unknown or unanticipated risks may result in injury, illness or death. Participants acknowledge that this program is purely voluntary, and with full knowledge of the inherent risks of the activities.
3. ASSUMPTION OF RISKS
I understand that participation in the Mount Hermon After School Adventure program entails certain risks. I am voluntarily participating in this program with knowledge of the risks involved. I hereby accept any and all risks of injury or death to myself or any minor children for which I am responsible, arising out of or in any way connected with the use of the program, the Mount Hermon Adventure facilities, and/or one of affiliated organizations of Mount Hermon Association, Inc.
4. RELEASE AND INDEMNITY
As consideration for being permitted to participate in the Mount Hermon After School Adventure program, I hereby agree that I, my assignees, heirs, and/or as the parent/guardian of a minor participant, will release and hold harmless and not bring any claim or legal suit against Mount Hermon Association, Inc., its directors, managers, officers, agents, employees and volunteers or its affiliated organizations or the supplier of any of the equipment used in the activity ("Released Parties"), for any and all claims of injury, disability, death or other loss or damage to person or property suffered by me or my minor child arising in whole or in part from participation in this program, both foreseeable or unforeseeable.
In addition, I agree TO INDEMNIFY (that is, defend and satisfy by payment or reimbursement, including costs and attorney's fees) Released Parties from any claim of loss, injury or death, brought on by myself or my child against another co-participant. These agreements of release and indemnity include loss or damage caused or claimed in whole or in part by the negligence of a Released Party, but not intentional wrongs or the gross negligence of a Released Party.
I HAVE CAREFULLY READ THIS VOLUNTARY PARTICIPATION AGREEMENT FORM AND PARTICIPANT REQUIREMENTS AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY IN WHICH I AM GIVING UP IMPORTANT LEGAL RIGHTS AND A CONTRACT BETWEEN MYSELF AND MOUNT HERMON ASSOCIATION, INC. AND/ OR ITS AFFILIATED ORGANIZATIONS, AND SIGN IT OF MY OWN FREE WILL.

Program Day Options
We offer 3 different schedule options: Wednesday Only, 2 Day Program or 5 Day Program. We do not offer bi-weekly or rotating schedules. 5 Day Program registrants have first priority.
Pricing is per month and per child. Tuition is due monthly on the first of the month. Payments are accepted as "on time" on or before the 5th. We accept cash, check, visa, MasterCard, and ACH.
Payments made after the 5th are considered late and will be charged a late fee of $50.00. If the payment is not made by the 20th of the month, your spot will be dropped and filled by the waitlist.
If the 5th falls on a weekend of holiday, payment is due the Friday (or last day program is open) before the 5th.

Drop In Services
We do not offer any drop in services as a part of our program.
Make- Up Days
We do not offer make-up days. After School Adventures operates on a monthly budget with expenses that stay the same whether or not an individual child is present. We cannot give payment credits due to illness, holidays or vacations.
We offer 3 different discounts to our community members.
1. Sibling Discount: 10% off monthly rate for the 2nd child enrolled from one family.
2. Mount Hermon Employee Discount: Those receiving MHE discount are not eligible for additional discounts.
3. Mount Hermon Resident Discount. Those receiving MHR discount are not eligible for additional discounts.
4. Financial Assistance: Those with need for financial assistance are given a discounted rate based on income and household size. This information is kept confidential. We ask that you please inquire with 831-334-0092 EMAIL if you have need. We follow the same sliding scale offered through the PG&E Care program. The following documents will be required: your last year's state or federal tax forms and a current pay stub for at least one month.
Late Pick Up Policy and Fee
Our program ends at 6:00 pm and we ask the all children are picked up no later than 6:15 pm. Picking up your child late from program will result in a $25.00 Late Pick Up Fee. After 5 minutes an additional $2.00 will be charged per minute.
If you know you will (or even might!) be late, please arrange for someone from your authorized pick-up list to pick up your child before end of program and call our office to let us know of the change. If we haven't heard from you by 6:20 pm, we will take the liberty of calling people on your authorized pick-up list to pick up your child.
We will give written notice a month in advance of any tuition/rate changes. Families should expect an increase in rates each fall.
Once registered, your child is enrolled in our program until the end of the school year. We require 2 weeks-notice for cancellation. You must re-register your child(ren) each fall when After School Adventures program begins again.
An annual registration fee is charged at $75 per family. Registration fees cover administrative costs and are non-refundable.
Your child may attend only once the required forms are submitted and your child has been scheduled into the program. Outstanding balances will prevent registration for the following school year until all payments have been made in full.
A two-week notice is required to implement any changes to your tuition and schedule.
Mount Hermon maintains a waiting list for After School Adventures. Admission will be granted based on availability. If you withdraw your child from the program, that spot will be filled by a child on the waiting list.
