Operational Policies & Procedures
The undersigned hereby acknowledges and agrees that they will abide by and agree to the Club Busy Bee Operational Policies & Procedures located at http://www.clubbusybee.org at the bottom of the page.
Assignment of PIN
The undersigned hereby agrees to and understands that he/she may NOT give their PIN to any other person to pick up or drop off their child(ren) for any reason. This is a security risk and all authorized drop off and pick up personnel must have their own PIN assigned by our automated system through the Parent Portal. I hereby indemnify and release Club Busy Bee and Jamsteld West, Inc. or its Agents from any liability should I compromise my assigned PIN.
Immunizations
I hereby acknowledge that my child's immunization records (or exemption) are on file with the elementary school and address listed on my child's enrollment record. I give permission for the records to be examined and/or released to qualified Club Busy Bee personnel for licensing inspection purposes.
COVID-19
I understand and agree that I will not send my child(ren) to Club Busy Bee with the knowledge of any COVID-19 or other communicable disease symptoms or positive test results of a communicable disease. Any willful neglect will be grounds for immediate expulsion from Club Busy Bee at the Director's discretion. I also acknowledge and understand that I will be required to pick my child(ren) up immediately upon notification of COVID-19 symptoms.
Chargebacks
I understand and agree that if I dispute a legitimate charge (My child was in care and proven with attendance records) with my credit card company/bank that I will not only be responsible for the fees owed but also the chargeback fees and any other associated fees due to the fraudulent dispute/chargeback.
Payment of Fees
All fees for CLUB BUSY BEE are due upon successful registration. Fees for your requested session(s) will be automatically charged to the payment method used on file during the registration process.
FEES WILL BE DEDUCTED WEEKLY FROM YOUR SAVED PAYMENT METHOD. YOUR PAYMENT METHOD WILL BE CHARGED NO LATER THAN THE SUNDAY PRIOR TO THE NEXT WEEK OF SERVICE.
DEBIT/CREDIT CARDS TRANSACTIONS WILL INCUR A 3% SURCHARGE. USING A CHECKING ACCOUNT TO PAY FOR SERVICES IS FREE OF CHARGE.
YOU MAY ALSO ELECT TO PAY BY CASH OR CHECK BUT MUST DO SO PRIOR TO CARD PROCESSING FOR THE NEXT WEEK. IF YOU WISH TO DISCONTINUE SERVICE, YOU MUST DO SO IN ACCORDANCE WITH THE POLICIES AND PROCEDURES MANUAL.
Release of Liability
The undersigned also hereby agrees to INDEMNIFY, DEFEND AND HOLD Club Busy Bee HARMLESS from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities including, but not limited to, attorney's fees, arising from, or in any way related to, Participant's participation in Club Busy Bee, except for those arising out of the willful misconduct, gross negligence or intentional torts of Club Busy Bee, as applicable
Annual Registration Fees
There is a NON-REFUNDABLE annual Registration Fee per child. A discounted registration fee is offered to families with more than one (1) child. The registration fees are as follows: (1st Child - $50, 2nd Child $40, 3rd and additional children $35 each)
Medical Emergencies
I understand and agree that Club Busy Bee will not administer medication to the children in our care. See exceptions below. I agree to and authorize Club Busy Bee personnel to arrange emergency medical treatment and transportation should the need arise. All families musts provide the name and phone number of your child's physician or emergency care facility. If no physician or emergency care facility is named, Club Busy Bee, by default will use Texas Children's Hospital emergency care located at 18200 Katy Fwy, Houston, TX 77094 as the Emergency Care facility and I agree to this facility.
Exception: If a child has a recurring medical condition, such as asthma or allergic reactions, the child's parent or health care provider may sign a medication authorization form allowing Club Busy Bee to administer the medication when symptoms occur for up to a six month period. The authorization must include information on symptoms to watch for.
Exception: Texas DFPS Rule § 744.2653 (d) of Minimum Standards for School-Age and Before or After School Programs: Parent Authorization is not required if you administer medication to a child in a medical emergency to prevent the death or serious bodily injury of the child, provided that you administer the medication as prescribed, directed, or intended.
Photo & Video Release
I hereby grant permission to Club Busy Bee and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of me, or members of my family, for the purpose of publication, promotion, illustration, advertising, or trade.
If you are not in agreement with this clause, please indicate as such in the comments box below with the following:
"I am not not in agreement with the Photo & Video Release clause and do not give permission for photos or videos of me or members of my family to used in any manner without my expressed written permission."
Assumption of Risk
The undersigned hereby acknowledges and agrees that they understand the nature of Club Busy Bee; that Participant(s) is in good health, and in proper physical condition to participate therein; that there are certain inherent risks and dangers associated with Club Busy Bee; and that, except as expressly set forth herein, they, knowingly and voluntarily, accept, and assume responsibility for, each of these risks and dangers, and all other risks and dangers that could arise out of, or occur during, Participant's participation in Club Busy Bee.
