Participant Liability Waiver and Hold Harmless Agreement
I understand by allowing my child to participate in any classes, rehearsals, performances or workshops (in person or virtual) there is possibility physical injury. I hold harmless, Studio J Dance Academy, all employees and representatives from all liability, suits, and claims arising from participation in or observation of any Studio J Dance Academy activities or events. I understand that Studio J Dance Academy is not responsible for any lost, stolen, or damaged property. I also recognize that Studio J Dance Academy is not responsible for students when they are outside the classroom.
Insurance
Studio J Dance Academy offers classes that require physical activity with risk of injury. The studio and its instructors are not responsible for any injury occurring while attempting these activities. Studio J Dance Academy doesn't carry medical insurance for its students, all students are required to be covered be their own family insurance policies.
COVID-19 Participation Release
Thank you for your continued support and trust in Studio J Dance Academy. As with the transmission of any communicable disease like a cold or the flu, you may be exposed to COVID-19, also known as the "Coronavirus", at any time or in any place. Be assured that we have always followed state and federal regulations to limit transmission of all diseases in our studio and continue to do so.
Despite our careful attention to sterilization, disinfection, and use of personal barriers, there is still a chance that you could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Social distancing has reduced the transmission of Coronavirus. Although we have taken several measures to provide social distancing and contactless participation, please know that participation in Studio J Dance Academy instructional classes is at your own risk.
I accept the risks and consent to allow my child participate. I also agree to adhere to all of the Studio J Dance Academy safety guidelines and measures to help stop the transmission of COVID-19.
I HEREBY WAIVE, RELEASE, AND DISCHARGE ?Studio J Dance Academy? and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during my childs/children's participation in and/or instruction of camps, intensives, private instruction, choreography, rehearsals, recitals, competitions, or any activity they may participate in with ?Studio J Dance Academy.
Registration, Tuition, & Payment Policy
Registration Policy
An annual, non-refundable, fee is required upon registration.
Once this fee is paid, and a parent/guardian has selected a payment plan for the year, registration is confirmed and the students place is held in the class.

$25.00 per student
$20.00 per sibling
Tuition Information
Tuition is based on a full season which runs from September-June.
There are three payment options available, upon registration you will be able to choose the best option suited for you.
The first (1st) tuition payment is due at registration.
Tuition is due the first (1st) of the month.
By the 5th of the month if we have not received payment a $25 late fee will be added to your account and the card on file will automatically be charged the balance.
Any student with an outstanding balance will not be allowed to participate in the recital.?
Although the number of classes per month varies, tuition remains the same each month. Students with a remaining tuition balance will not be allowed to participate in the recital.
All tuition payments are non-refundable and non-transferable.
Tuition Payment
An active credit card is required to be kept on file, in addition to a pre-authorization contract, in the event that Studio J Dance Academy does not receive your payment by the applicable due date.
The credit card number will be encrypted into the computer system, and there will be no permanent written documentation of the credit card number.
Payments that are paid in full by the first (1st) class will receive a 10% discount on their total tuition bill. ?
If you choose our 3 installment payment plan, payments can be made by cash, check, or credit card (Visa, Master Card, Discover, American Express).
There will be a $35.00 charge for all returned checks. After the second (2nd) returned check, checks will no longer be accepted.
The first (1st) tuition installment is due at registration, The second (2nd) installment is due January 1st, and the third (3rd) installment is due April 1st.
If you choose the 10 monthly installment plan an active credit card (Visa, Master Card, Discover, or American Express) must be kept on file and will be charged automatically on the first (1st) of every month (September-May). The last two payments (May and June) are due together May 1st.
In the event that the credit card on file becomes inactive, the parent/guardian must place an active credit card on file with the studio on or before the next billing date. Should Studio J be unable to charge a credit card by the fifth (5th) of the month, there will be a $25 late fee added to your account.
Should the studio be unable to hold classes in person due to situations out of our control (example: pandemics, government closures, etc.) classes will be offered on an online platform and tuition will remain the same.
Private Lessons Policy
All private lessons will be billed in full at the beginning of the month. All lessons and fees are non-refundable. If a student needs to miss their private lesson the studio must have 24 hours notice in order to make-up missed instructional time. Students have one-month from the missed lesson to be make-up the time, subject to the instructor's discretion. In the event of an emergency, and the notice is under 24 hours, please contact the studio so arrangements can be made.
Withdrawal and Refund Policy
You must contact the studio in writing one month prior to your upcoming tuition payment in order to discontinue.
If you choose to withdraw, you are still responsible for the tuition payment that is due within 30 days of your written withdrawal.
The payment that is due within the 30-days of your written withdrawal will automatically be charged to the card on file once the withdrawal is finalized.
If a student withdrawals after February First (1st) the account on file will immediately be charged for the remaining season's tuition.
No refunds will be given for studio closings or missed classes.
Registration and tuition payments are non-refundable and non-transferable.
Photo Release Policy
By registering with Studio J Dance Academy, I give permission for instructors and staff to take and publish photos/videos of my child.
I acknowledge that if I do not wish for my child to be photographed/recorded and/or posted I must email a request to info@sjdanceacademy.com.
Recital and Costume Policy
Costume Deposit & Fee
A recital costume deposit of $80.00 will be charged to your account on November First (1st). If your costume deposit is not paid your costume will not be ordered. If you pay your costume deposit after costumes have been ordered we will place another order and try and get you the costume. In this case you will be charged for shipping. If there is a monetary difference in the amount of the deposit and the actual cost of the costume(s) your account will either be credited or charged. If there is a costume balance, the balance must be paid prior to receiving the costume. If you withdraw prior to the recital you will not receive your costume(s) and you will forfeit your costume deposit. All costume deposits are non-refundable and non-transferrable.
Recital
Studio J Dance Academy holds an annual recital at the end of every season. All performers are automatically registered for the recital. If a student will not be participating in the recital, the studio must have written notice prior to November first (1st) in order to avoid costume(s) fees. If the studio is notified after November First (1st) your account will be charged for the costume(s). Once the costume(s) is charged to your account, there is no refund. No student or class (competition or recreational) is permitted to perform any choreography or use costumes in any venue without written permission from Studio J Dance Academy.
Attendance Policy
• Students should arrive five (5) minutes prior to their scheduled class and should arrange to be picked up at the completion of class.
• Studio J Dance Academy is not responsible for students once class concludes.
• Students should stay in the studio until a parent/guardian is present.
• A parent/guardian should notify the studio if they intend on being early or late for student drop-off and/or pick-up.
• If a student will not be attending class, please notify the studio, by phone or email, 24-hours prior to the start of class.
• Students are allowed three (3) unexcused class absences. (Unexcused is defined as not giving the studio the required prior notice.)
• Students missing more than three (3) classes without notification are required to make up the time by scheduling a private with the instructor or by attending a similar class according to their level and age.
• Classes missed due to inclement weather will be rescheduled.
• Students who feel ill, have contagious symptoms, or have a prolonged fever should stay home from class.
o Students should not return to class until they are fever-free for 24 hours.
• Students who are unable to dance, but feel well enough to attend class, may participate by observing.
• In the event of a prolonged absence please notify the studio.
Studio J Dance Academy recognizes that class attendance is a direct correlation to student success. Instructors will be responsible for maintaining accurate class records and attendance. Students who have perfect attendance will be recognized at the recital. Please make every effort to have your child in class.
Inclement Weather
Studio J Dance Academy follows the Montville school system with regards to studio closings and closings due to inclement weather. An email will be sent out if the studio is closed for holidays or inclement weather. You can also check out Facebook and Instagram page for updates.
Class Observation
Parent/Guardian observation will take place two (2) times per school year. Each student will be allowed two (2) tickets. Observation dates will be announced at the beginning of the year. Photos and videos are permitted during this time.
Classroom Policy & Procedures
• No student is allowed in the studio or classroom without an instructor present.
• Students should respect instructors, staff, and classmates.
• Students should help keep the studio workspace neat and tidy.
o No hanging on the barres or touching the mirrors.
• Students first and last names are to be marked on the inside of all shoes, dance bags, and other personal items.
o Studio J Dance Academy is not responsible for lost or stolen items.
• Students should not wear dance shoes outside of the studio.
o See dress code for complete list of appropriate dance footwear.
o No street shoes are allowed on the dance floor.
• Students should come to class fully prepared in the proper class attire.

• No jewelry is allowed in class.
• No food, drinks, candy, or gum is allowed in class.
o Water breaks will be given at the teacher's discretion, students may take a water break between classes.
• Students should keep talking to a minimum during class time.
• Instructors reserve the right to dismiss any student from class for continual disruption. If multiple instances occur a meeting will be held between the instructor, parent/guardian, and student.
• No cell phones are allowed in class.
• Students should use the restroom before class. Students may use the restroom during class at the discretion of the instructor.
o Parents of students of ages 3-6 are encouraged to stay in the waiting area while class is being conducted for "emergency" bathroom situations.
