The purpose of this release and waiver (the "Waiver") is to openly communicate the risks of practicing dance together, either in person or online, and have you release Centerstage Starz of any liability. Please be aware that if you do not sign this Waiver and agree to its terms, we will not let you participate in our classes.
If you are under the Age of Majority in Colorado (18 years old), your legal guardian must also sign this Waiver on your behalf.
PLEASE READ CAREFULLY, UNDERSTAND FULLY, AND ASK QUESTIONS IF ANYTHING IS UNCLEAR. WE ARE HERE TO SUPPORT YOU. MAKE SURE YOU UNDERSTAND THIS WAIVER. BY SIGNING THIS YOU AGREE YOU ARE SIGNING AWAY YOUR LEGAL RIGHTS AND YOU AGREE TO BE BOUND BY ALL THE TERMS OF THIS AGREEMENT.
Parties. We will refer to Triple Threat Management, LLC, a Limited Liability Company registered in the State of Colorado, as "Centerstage Starz", "us" or "we" and we'll refer to you, the undersigned (electronically or by hand) or person who has clicked "I Agree" to this Waiver, as "you" or "your".
The Activities. You may participate in dance and exercise classes, sessions, workshops, programs, competitions and more with Centerstage Starz.
We offer knowledgeable dance and exercise classes, including tap, ballet, jazz, lyrical, hip hop, pointe, musical theater, contemporary, ballroom, technique, tumbling and more, all of which may involve but are not limited to the following activities (the "Activities"):
Dance and creative movement, flexibility training, body weight exercises, locomotor movements, stretching and resistance exercises, strength exercises, mobility, range of motion exercises, breathing exercises.The constant in all of these classes is that the Activities involve moving in full ranges of motion, strength training, and postural training. The Activities may involve receiving adjustments from instructors, so please advise in advance if you do not want adjustments.
The Activities also apply to any dance and exercise classes which are made available for online streaming and participation or that take place outside of Centerstage Starz's facilities, including outdoors. As these classes will be taking place outside of Centerstage Starz's facilities, we need you to acknowledge that you are responsible for the safe facilitation of the Activities.
Equipment. In the course of the Activities, you will use a variety of equipment, which may include but is not limited to: ballet barre, stage props, poms, resistance bands, foam rollers, massage balls, ankle weights, and suspension bands (the "Equipment"). Please do not use any equipment not instructed by Centerstage Starz.
Inherent Risks. You understand that participating in the Activities poses inherent risks, some more obvious/serious than others. These risks can result in serious harm and injuries that could change your quality of life and, in very rare and extreme circumstances, may even result in death.
Injuries include but are not limited to things like muscle tears, strains and other musculoskeletal injuries, sprains, broken bones, cardiovascular complications, dehydration, dizziness and fainting.
Exposure to and contraction of COVID-19 or other communicable diseases passed on via other participants and use of shared space, surfaces, or equipment.
There are additional risks posed by participating in the Activities online, as there is no in-person supervision or space provided for you, and you will therefore need to ensure the safety of the Activities, using your judgment how to best practice them, not pushing yourself too far or attempting anything you feel unsure how to perform.
Affirmation of Health. By participating in any Activities at Centerstage Starz, you affirm that you have sought medical advice regarding your fitness to practice dance. If you have not sought such advice, you must be certain that your medical and fitness levels are sufficient to participate in advanced level dance, including dance in heated conditions.
COVID-19 / Infectious Disease. Amidst the current developments surrounding communicable diseases, you acknowledge that every time you participate in the Activities you are affirming that you are healthy and that you do not present an increased risk due to COVID-19 or other infectious diseases. If you are suffering from symptoms of a communicable disease or are unsure of your health and ability to practice the Activities, please consult with your doctor and we urge you not to visit our studio or participate in our Activities. We are all in this together and it is important that we follow guidelines on social distancing, hygiene practices, and act with kindness and responsibility towards each other in order to ensure everyone's safety.
Voluntary Assumption of Risk. You have read this Waiver and understand the risks of participating in the Activities with Centerstage Starz. Your signature below, electronic signature or clicking 'I Agree', and your participation in the classes at Centerstage Starz illustrates your voluntary engagement and assumption of the risks of the Activities.
Release, Waiver and Indemnity. You hereby release Triple Threat Management, LLC, its members, directors, officers, contractors, employees, volunteers, agents, executors, administrators, successors, family members and assigns (the "Released Parties") from any liability and damages arising from death or personal injuries, including the contraction of COVID-19 or other communicable diseases, however caused including as a result of Centerstage Starz's negligence, during your participation in the Activities at Centerstage Starz. You are releasing the Released Parties at your own risk and you agree to forfeit any and all forms of legal recourse which may be available to you, including but not limited to any form of damages, as a result of your participation in the Activities. You agree that these provisions above apply to you, your family, heirs, executors or anyone else who may be able to bring a legal action on your behalf in the future.
Media Release. We think you're awesome and want to show you off! By being a student at our studio, you agree to grant us the irrevocable right to use your image, likeness, photographs, video content, audio recordings of you captured in our studio, other venues such as, recital, competitions/conventions, outside performances or that you share with us online (via your own or others posting of you) as part of our online streaming, marketing and sales throughout the world and in perpetuity. You also release us from all claims you may have relating to the use mentioned in this section. Please let us know If you ever want us to stop using an image of you.
Continued Agreement. Agreement to this Waiver will act as your continued agreement to all ensuing classes, sessions and/or workshops whether in person, online, or via video conferencing tool.
General Legal Provisions. Jurisdiction. This Waiver will be governed exclusively by the laws of the State of Colorado. Severability. If any provisions of this Waiver are invalid or unenforceable, the other provisions in the Waiver will remain in full force and effect. Entire Agreement. This Waiver constitutes the entire agreement between the parties and replaces any prior agreements. Headings. The headings used in this Waiver are for stylistic purposes only and none of the content in the headings are intended to be legally binding. Online Agreement. We agree that this Agreement may be signed electronically or agreed to by having you click "I Agree", the effect of which will be the same as signing by hand and the intention of which is that both parties desire to be bound by all the terms of the Agreement.
You agree that you have read this Waiver and fully understand its contents and voluntarily agree to be bound to all of its terms.
Tuition Information
All Centerstage families are asked to set up an automatic monthly payment program using a credit or debit card. Tuition will be run the 1st of the month for the entire Session.
• Our classes are structured around Sessions. Fall Session runs September - December (4 monthly payments), Spring Session runs January - May (5 monthly payments), and Summer Session runs July - August (1 payment). We also have an annual registration fee.
• Families are welcome to pay the entire session, for which there is a discount, by cash, check, or credit. All monthly payment accounts must use a credit or debit card.
• Students will automatically be enrolled from Fall to Spring classes unless we are informed otherwise. If a student plans on changing classes from Fall to Spring Session, please inform us by December 1st. If your student is not continuing for the Spring Session, please notify us by the 1st of December.
• Tuition will only be pro-rated if a student starts class after the start of the session.
• On accounts where the credit card expires or declines, a late fee of $15 will be assessed after the 7th of the month. Accounts not paid by the 15th of the month will result in a student not being allowed to take the class until the balance and fees are resolved. All accounts more than 30 days past due will be turned over to our collections agency.
• Tuition will always be the same whether there are 3, 4, or 5 weeks. The longer months and extra rehearsals compensate for the shorter months. We do not charge by the class but rather by the session, which takes into account all scheduled holidays and breaks.
• 10% off each additional sibling.
• Absolutely no refunds! No financial credits for missed classes. No exceptions will be made. If a child misses a class, he/she is welcome to make up in another class. Please schedule make up classes with our Office Manager.
• If a student is withdrawing from a class, we require written notice via our Class Withdrawal and Automatic Payment Cancellation form. This form must be turned in and its receipt acknowledged by a Centerstage representative at least 30 days prior to our next billing cycle. Many of our classes are on a waitlist and we need to notify the people on
the waitlist to fill vacant spots. If we are not notified of withdrawal 30 days prior, you will still be held financially responsible and you will receive an invoice for the appropriate amount as well as any late fees that have accrued. Students withdrawing early from the Session will not be offered a refund or credit for tuition.
• All exceptions must be made in writing by the parent/guardian and approved by the owner in writing.

Studio Guidelines
Please remember these guidelines are to ensure the safety of students, maintain a clean environment, and help the studio function in an efficient manner, and above all, provide students with unparalleled performing arts education!
• Observation Week is strictly restricted to the first week of every month. Restricting observation time eliminates excess noise and allows the students to focus and learn more quickly, ensuring a high quality class (especially with younger students). There are observation windows in all of the studios for observation week. Please keep the hallways clear and allow all parents a chance to observe. Please do not tap on the glass or enter the room for any reason. If you have an emergency please notify the front desk. Parents/friends are allowed to wait in the lobby area. The dressing rooms are for dancers only, please respect their privacy.
• All studio news and information will be sent by email, posted on our website at www.centerstagestarz.com and on our Facebook page. Please make sure info@centerstagestarz.com is on your safe-sender list and not on your spam list. Please reference the website often for the most up-to-date news, upcoming dates, etc.
• It is very important that students attend all classes for which they are registered. Consistency is an integral part of the learning process and a key to improvement and success.
Please call the studio for any absence, for an extended period or just one class.
• Questions or concerns regarding any aspect of your child's education/accounting at Centerstage Starz should be addressed by appointment with the appropriate staff member/ director. During teaching hours, the focus should be on students so please do not disrupt class time to address questions. Please call the studio during regular office hours
or to arrange a meeting. Please address questions to the staff instead of another parent as we have the most up to date and accurate information to help you. We will only discuss issues that affect your child individually.
• Students are expected to show respect for themselves, teachers, parents, and classmates. We understand that students form friendships and are encouraged to, however, we ask that excess talking and such be taken care of before or after class to limit distractions in class.
• In the case of staff illness/absence: We do not cancel the class. Another staff member will step in and substitute teach in the absent staff member's absence.
• In case of inclement weather: Tune to radio or TV stations for Cherry Creek, Littleton, and Douglas County School closures or cancellation of any after school closings. Centerstage Starz will be canceled on days that these districts are closed due to bad weather. If the studio must be closed due to circumstances beyond our control (power outages, inclement weather, etc.) there will be no refunds or make-ups scheduled.
• Proper dance attire and dance shoes are required for all classes. Unprepared students will not be allowed to participate in class.
• Ballet: Girls need to wear a solid color leotard and tights with solid tight-fitting shorts or ballet skirt over and pink ballet shoes. Boys need to wear sweats or dance pants with men's black ballet shoes.
• Jazz/Poms/Lyrical/Tap/Hip Hop, etc.: Dance students need to wear exercise clothing. Inappropriate clothing, including khakis, jeans, socks without shoes, and large baggy t- shirts, restrict movement and can cause an injury.
• Black jazz shoes are required for Lyrical, Poms, Jazz, Turns, and Progressions. Black tap shoes, preferably tap oxfords, are required for Tap class. Hip hop sneakers need to be shoes that are not worn outside at any time. Street shoes are absolutely not allowed on the dance floors!
• No gum or food during classes! We do not want anyone choking. Food will only be allowed in the front lobby. Under no circumstance will food be allowed in the dance rooms or locker rooms.
• Always bring a water bottle to class. The studio does not have a water fountain. Bring it with you to your class.
• Students and parents are responsible for cleaning up after themselves. Please make sure trash makes it into the trash cans. Please respect the property of Centerstage Starz including all common areas, floors, barres, tables, chairs, walls, etc.
• Centerstage Starz is not responsible for lost or stolen items. Students must have their name on all personal property, shoes, bags, jackets, etc. Please do not bring valuables into the studio. All personal effects should be kept in a dressing room locker or a cubby.
• Students are to arrive to class no more than 10 minutes before class starts and must be picked up promptly after class. Every student should arrive in enough time to change clothing, eat, and use the restroom, etc. before class starts. Parents will be charged a $20.00 babysitting fee if a dancer is picked up more than 20 minutes after their class has ended. Call us if you are running late for any reason.
• Centerstage Starz is not responsible for students and/or other children left unattended outside of regular class time including any older children who leave the premises for any reason.
• We highly encourage parents to walk students in and out of the studio. No student should leave the studio without a parent. Students should not wait outside!
• We do one large dance recital production and one theatre production in the spring. We will do a smaller performance at the studio of scenes, dances, and songs after Fall Session.
• In lieu of a recital fee, we sell tickets to our Spring shows. Ticket prices will range from $20-$25 per ticket.
• A costume fee will be charged for each dance class in which a student performs. Dancers will keep these costumes. Costumes will range from $60-$95 per class. Dancers will also need to purchase the appropriate tights for their costumes, which range from $10-16 if you order them through the studio. Parents will be responsible for selecting the costume size we will order. Parents are responsible for any necessary alterations.
• We at Centerstage Starz are dedicated to keeping a positive and encouraging environment for all students, parents, and staff. We will not tolerate any negativity, gossip, or otherwise disrespectful behavior. Poor behavior by any parent or student may result in dismissal from our studio. Thank you for helping us continue to provide a fun and healthy place for your children to grow.
• Centerstage Starz reserves the right to refuse any further service to a parent or dancer if poor or inappropriate behavior occurs. If termination occurs, any fees paid to Centerstage Starz will not be refunded.

Basic Concepts:
• Be aware of COVID-19 symptoms (Appendix A) and check the CDC website for updates. https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html
• Dancers and staff should only be at the studios if feeling completely well with no symptoms of illness.
• Temperature checks using a no-touch forehead thermometer must be conducted daily for every individual before entering the studio.
• If a dancer or staff member has had COVID-19, they will need a clearance note from a doctor to return to the studios.
• Please do not send you dancer to the studio if they have a fever of 100.4 or above, cough, shortness of breath, chills, muscle aches, soar throat, or loss of taster or smell.
• Any possible exposure to someone with any Covid-19 symptoms or someone who has tested positive for Covid-19 (close contact for exposure is typically defined as within 6 feet for at least 10 minutes) Will need to quarantine 14 days before returning to the studio.
• Social distancing recommendations in general become significantly greater when dancers and athletes are exercising indoors.
• All dancers and staff need to behave as if they are an asymptomatic carrier of COVID-19 to help reduce risk that asymptomatic individuals are infecting an enclosed environment.
• Our staff will be cleaning all touch areas such as door handles, ballet barres, light switches, door knobs, and bathrooms. We have cleaning supplies, hand sanitizer and antibacterial soap to make sure we maintain a safe working environment with cleanings multiple times/day to adhere with safety guidelines.
Returning to Centerstage Starz:
• All staff will be required to wear a mask
• All Instructors will be in a designated area in the class room and will not have any physical contact with any of the students
• Every dancer will be getting a temperature check as they enter the building
• Dancers take off their street shoes at the studio entrance and will place them in a single file row in the hallway outside the studio or in the front lobby.
• We are highly recommending all dancers to wear a mask. If a dancer has to remove their mask, they will only be allowed to remove it when they are in the dance studio standing in their assigned square.
• Dancers in each studio will remain 6 feet apart, the studio floors will have square markings to indicate where to stand.
• Dancers with multiple classes will not change studios they will remain in the same studio and keep their assigned square through all of their classes
• Hand sanitizer will be in each studio
• We have several specific entrances and exits with signage to keep you safe
• Studio doors will be propped open to help with the air flow
• Parents are to drop off and pick up at a designated exit and not permitted in the studio. Any questions or information can be done through email, text, or a phone call.
• Dancer must bring their own water bottles that are full!
• The water fountain /Ice machine will not be in use
• If necessary or required by an instructor the dancers will be required to bring their own equipment (yoga mats, etc.)
• All locker rooms will be closed so dancers will need to bring their dance bags and they will be placed in a designated area in the studio.
• We are zooming all of our live-in studio classes for those not ready to return
• One person at a time in the restrooms
• Lobby is closed no waiting area available
• Dancers are encouraged to have personal hygiene kit in their dance bags that has hand sanitizer, a mask, and wipes
• Dress code is to be followed but for dancers who take multiple dance classes in one evening, they will only be allowed to wear one outfit. We suggest for dancers with several classes to wear: girls- black bra top and booty shorts or black leotard and or black leggings, boys- black t-shirt and black pants
• No make-up classes will be available during our summer session due to the limited number of students we are allowed to have in each class
