COVID-19 Waiver for the participation of the student.
Dance IQ and its staff are doing everything to mitigate the risks associated with the COVID-19 pandemic. We are taking measures to protect the public, our workers and volunteers to the extent that our activities permit. As with any physical activities, we can avoid proximity as much as possible, but at times, it may be impossible. Since all our students are children, their involuntary behavior at times may not be possible to control. To the extent possible, we will be following the Center for Disease Control (CDC) and local health department guidelines regarding social distancing practices to reduce the spread of Novel Coronavirus or COVID-19.
These are the steps and procedures we will be taking:
1. We will require our staff and volunteers to maintain six (6) feet of distance between ourselves and you, the client, as much as possible. We will also be requiring the same for client-to-client contact to limit the exposure to all individuals.
2. Only one parent will be allowed in the reception area at a time.
3. Masks must be worn by anyone coming in the studio other than teachers and students during classes (it is not feasible to wear masks during activities).
4. Parents must pick up their children as soon as CAMP/Class(s) ends. The staff will be cleaning up for the next class therefore, children are not allowed to "hangout" in the dance room/lobby. Failure to do so may cause termination of the student, and no refunds will be given.
5. Parents shall keep at least six (6) feet from each other when dropping their children off.
6. We will be taking each child's temperature upon arrival. Any child with a temperature above 100.4o (CDC's standard for defining a fever) will not be allowed to take the class. If a class is missed there are NO makeups for summer camp. After-noon classes may be made up within 2 months of absence. The temperature will be taken with an infrared thermometer, which is noninvasive and does not touch the child.
7. We have acquired a hospital-grade disinfectant that will be used on floors, barres and restroom area after each class. For information on such disinfectant, please check HALT.
8. We have split the studio floor into 6' spaces. To the extent possible, we will have the children distanced from each other and not leaving their areas.
9. The instructors will plan their classes with the separation in mind. The exercises and dances shall be planned to keep children apart from each other.
10. We are providing hand sanitizers.
11. Students will leave their street shoes at the front door cubby holes. Everyone will sanitize their hands prior to entering the street lobby door entrance. Everyone will either wash or sanitize their hands after using the restroom, sneezing, coughing, and before eating meals.
It is important to note that precautionary measures are sometimes hard to enforce when dealing with children. These are mitigating procedures.
We welcome you to our facility. We understand that each one of us has a different opinion on all that is going on. In respect for others and to keep us all safe and businesses open, we ask for your understanding and cooperation.

Failure to comply with these written instructions or verbal instructions from staff or volunteers may result in your removal from the premises.

GENERAL INFORMATION

We will communicate upcoming information to parents by email and/or with classroom handouts. Information will also be kept current on the website.
Parent Visitation Week:
Parent Week is held for one week in December and one week in May. During that week you are invited into the class and may take pictures or videos. Please check our Calendar and Events page for this year's dates.
Teacher Conferences:
Our teachers have the best interest of your child as their priority. If you would like to discuss your child, the director will be happy to speak to you on the phone or meet with you in person. In fairness to all children in the class and to ensure that our classes start and end on time please do not attempt to discuss situations with the teacher before or after class.
Privacy Policy:
Your personal and payment information will not be shared. It is for Dance IQ's use only. Security cameras are placed in lobby, dance floor and back cubby hole area for your security.
Trial Classes:
Prior to committing to a season enrollment, purchasing a class package or Homeschool program, a student is entitled to take 1 (one) free class as a trial.

Indemify
PARTICIPATION AGREEMENT, PERSONAL CONSENT, ASSUMPTION OF RISK, LEASE OF LIABILITY
Participation Agreement- I FULLY UNDERSTAND THE NATURE OF POTENTIAL INJURIES ASSOCIATED WITH ENTERING THE FACILITY AND PARTICIPATING IN A DANCE CLASS. I ASSUME ALL RISK, LIABILITY, AND MEDICAL COSTS FOR ALL INJURIES REGARDLESS OF WHO IS DETERMINED TO BE NEGLIGENT. I state that we are in good health and physical condition and without any previous injury, surgery, or limitations to safely participate in all activities. I understand that dance involves a likely risk of bodily injury which may be caused by our own actions or inactions, action of others or by other risks either not known to us or not readily foreseeable at the time and may even by the negligence of employees at the studio and I state I HAVE FREELY CHOSEN TO ENTER THE STUDIO AND PARTICIPATE IN A DANCE CLASS KNOWING OF INJURY RISKS AND FULLY ACCEPT AND ASSUME ALL PERSONAL RISKS AND ALL RESPONSIBILITIES, ESPECIALLY FOR MY MINOR CHILD. I will assume all costs or other damages that are a result of our participation in any and all activities at, in, and outside of the Dance IQ facility.
I ALSO AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS all involved with Dance IQ thereby releasing all others from liability, personal claims, and even negligent occurrences at or by Dance IQ. I declare we have personal health care insurance or a personal bank account, which will be used in the event of any costs involved in a personal injury.
Dance IQ has an emotional service dog (hypoallergenic) on the property to help our students with learning differences.
Membership OR Monthly Tuition & Reg. Fees

Membership UNLIMITED MONTHLY CLASSES:
12 Months (Aug.-Aug).
Renewal of membership is AUTO RENEWAL. A 3 month (90 day) warning is mandatory to terminate your auto renewal of the following year. The $50 registration fee will be due if enrollment is interrupted and the family will go back to non-membership prices.
For Credit Card payments there will be an extra 2.5% fee. For auto debit from a bank account there is no extra fee.
If first term is prorated your next membership term will need commitment by June 15th of the following year.
Credit Card or Automatic debit charges will occur on the 1st of every month.
A current Credit Card MUST be kept on file if credit card payment is the option.
If governmental closure is mandatory, your account will be frozen at NO CHARGE and credit will be given when studio resumes classes. If we offer classes though Zoom you may choose to continue classes.
Non-Membership:
Sibling discounts apply (5%) non-membership only
An Automatic debit will occur the 1st of every month using your bank account or CC.
NON-membership-A 2-week cancelation notice must be given on the 14th of the PRIOR month in order to cancel classes.
No refunds, credits, or transfers. If governmental closure is mandatory due to the pandemic, your account will be frozen at NO CHARGE and credit will be given. For example, closure in September 15th you will receive a credit (2 weeks) for the remainder of the month either towards Zoom or makeups when the studio resumes classes. The remaining installments of the membership program WILL NOT BE CHARGED or owed. This applies to Homeschool families as well. If you paid your membership in a LUMP SUM, you WILL HAVE A PRORATED REFUND OF YOUR TOTAL MEMBERSHIP! ** The prorated refund is valid for summer camp as well.
Membership & Non-Membership:
A one-time non-refundable $50 registration fee (per family) is due upon enrollment.
A RBT specialist may be given upon request or evaluation (extra fee applies).
Makeup classes and class packages are not transferable to another student/sibling.
If you leave the studio or do NOT renew membership in June 15th, a registration fee is due again if you return. Makeups are only allowed for registered students within 2 months of missed class(es).
Tuition is due on the 1st of the month and considered late on the 10th. A $15.00 late charge fee will be added to accounts not paid by the 10th.
If you enroll in classes IN THE MIDDLE OF THE MONTH, you will be charged tuition at a prorated rate.
Etiquette
Students are to arrive on time for class (missed warm-up can result in in injury) and picked up on time. Dance IQ employees are not responsible for personal property.
No gum, food, drinks or smoking is allowed. Water is permitted and encouraged.
Please no hanging on the barres and touching mirrors.
Child's parent or guardian must supervise child/children not in dance class. Office staff will not be responsible for unattended children. Please restrain from entering into the studio to discipline your child or accommodate them with water or clothing issues. Water, dance shoes etc. should be kept in the dance room to avoid students and parents from going in and out of the studio area.
Non-members as a moral and ethical request, please advise the studio 2 weeks prior to the 1st of the month billing if you decide to terminate your enrollment. Dance IQ offers the freedom of having a "NO CONTRACT AGREEMENT" however, if this curtesy is abused and you choose to return to the studio a FULL/Prorated academic year payment will be enforced. In order to plan finances we need to know our enrollment. If you simply decide to leave and not pay after the 1st of the month please be known that this is unethical. Thank you in advance for your honesty.
Attendance
Does Not Apply to Distance-Learning
Regular class attendance is imperative for consistent progress and to insure that the dancer receives the maximum benefit from the class. When you enroll in a class a spot is reserved for you and there are no refunds for absences. Make-up classes are available if offered in the schedule. Makeup classes can be scheduled within 2 months.
A student MUST BE ENROLLED to makeup classes. If a student must miss a class please call the studio at 954-294-5503 prior to scheduled class time so we can notify the teacher.
Makeups are NOT transferrable to siblings. If a student does a makeup, the makeup needs to be of the same genre. Class Packages are NOT transferable to siblings or other students.
Dance IQ has a twelve dance season. Dance IQ follows Broward county schools for Holidays and school closures. Dance IQ will stay open on school conference days and school half days (except for the Wednesday before Thanksgiving). The studio will close for Halloween, Oct. 31 at 6 P.M. Classes before 6 P.M. will be active.
Observation/Visitors
Does Not Apply to Distance-Learning
No parents or siblings are allowed in the classrooms unless invited by the teacher. If you need to cross the dance floor to use the back room areas, please do so discreetly as to NOT disrupt the class, students and teacher. Please DO NOT enter the class to fix your child's clothes or pamper. Please bring water into the studio area so children are NOT running in and out of the class. This causes a huge disruption.
Children behave differently when parents are in the room and tend to cling to you instead of focusing on the teacher and experiencing the class. Please observe the class from the observation windows.
Canceling Classes and Substitute Teachers
Class may be cancelled due to events beyond our control such as power outages or Hurricanes and Hurricane Watches. There are no refunds or credits for missed classes. Dance IQ will leave a message on the answering system letting you know whether or not classes are cancelled. There are no refunds or credits given for classes missed due to illness, holiday, vacation or when the studio is closed due to weather. Make up classes are a courtesy, and if available, must be taken within the 2 month period of the class(s) missed. Applies to students who have private classes:
If a student needs to cancel a private session you MUST call at least 24 hours in advance prior to the scheduled private session. If the student does not show up to the previously scheduled session without giving a prior notice you will be charged for the class.
Summer Camp REFUNDS ONLY! If the studio is forced to close because of government/Pandemic, you will be refunded for the week/days your child did not attend. This refund is ONLY for government mandated closures.
The studio reserves the right to provide a substitute teacher if the regularly scheduled teacher is sick or otherwise unable to teach. If a teacher is ill and we cannot arrange for a substitute, class will be cancelled and a make-up class arranged.
Photo Release
Photos and or video may be taken in class for use on our website, yearbook, promotion, social media or advertising. If you do not wish your child to be photographed, please indicate in the box above under the family information section (Photo? Yes or No)
