Tuition Policies
Tuition is due on the 1st of every month. A Late Fee of $20 will be assessed on the 2nd of each month. A fee of $20 will be assessed for all returned checks or declined credit cards. Automatic payments will be drafted on the 1st of the month. Prepaid sessions are non-refundable.
Registration Fee
A Registration Fee of $45 per student or $65 per family is due at the time of registration for monthly classes. This fee is non-refundable and non-transferable.
Annual Fee
An Annual Membership Fee of $45 per child or $65 per family is billed on September 1st of each year and due no later than October 1st for all students who are actively enrolled in a class. This fee is non-refundable and non-transferable.
Make-Up Classes
In the event that a student misses a class due to illness or travel, one make-up class is permitted per month. Students must be paid in full and all make-up classes must be scheduled through the front office. Make-up classes are not permitted for regular gym closures. Missed classes may not be transferred to other students, credited, or refunded. Make-up classes may not be available for all classes due to limits in class size and class availability.
Withdrawals
Upon enrollment your child will automatically be placed on the following months' roll. Should you choose to withdraw your child, a WRITTEN NOTICE PRIOR TO THE 15th of the month proceeding is required. (Ex: To drop for January, Withdrawal Forms must be received by December 15th.) If notice is not received, you will be responsible for payment of one month's class fees at the time of cancellation. NO VERBAL or ELECTRONIC WITHDRAWAL WILL BE ACCEPTED. Withdrawal forms are located at the front desk.
Photography
I understand that my child's photograph may be used for promotional purposes in print or electronic advertising. I agree to notify the front desk if I do NOT want my child's image used for these purposes.
Assumption of Risk & Release of Liability
As legal guardian and/or one of the above named persons and/or participants, I recognize that potentially severe injuries, including permanent paralysis or death can occur in sports or activities involving gymnastics, ninja, or tumbling, included but not limited to gymnastics/ninja related activities, tumbling, clinics, camps, private lessons, birthday parties, parent night out events, bring a friend events, field trips, competitions, trampoline, running, conditioning, personal training, and preschool activities. Being fully aware of these dangers, I voluntarily consent to my dependent(s) and/or other participants on my account participating in any and all programs at FAC and I ACCEPT ALL RISKS associated with that participation. In consideration for allowing my child and/or myself to use this facility, I, on my own behalf and the behalf of my child and our respective heirs, administrators, executors, and successors, hereby CONVENENT NOT TO SUE and FOREVER RELEASE Fusion (FAC), its owners, officers, directors, employees or other representatives whether paid or volunteer, from all liability for any and all damages or injuries suffered by myself or my child while under the instruction, supervision or control of FAC. I also understand that it is the responsibility of the legal guardian and/or the above named persons to warn the participant and/or be aware of the dangers of injury. The guardian is aware and should warn the participant according to what the guardian feels is appropriate. Fusion Athletic Center will only warn the participant thru safety messages and our teaching style and progressions.
I further acknowledge, understand, appreciate and agree that my participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of FAC or others, and assume full responsibility for my participation and exposure.
I further agree that FAC, and the sponsor of any FAC event, along with the owners, officers, directors, employees, and other representatives shall not be liable for any losses or damages occurring as a result of me or my child participating in an FAC event or traveling to or from the event, except where such loss or damage is the result of intentional or reckless conduct of one of the organizations or individuals listed above.
Medical Emergencies
I fully understand that Fusion Athletic Center (FAC) staff members are not physicians or medical practitioners of any kind: With that in mind, In the event of injury while participating in Fusion Athletic Center classes, clinics, and/or events, I hereby give my consent for Fusion Athletic Center to obtain emergency medical treatment for me and/or my dependents. I also agree that I will be responsible for all medical treatment and expenses for myself and/or my dependents that result from any injury attained at Fusion Athletic Center.
Student's Rules & Reminders
I agree to review these rules with my child(ren) and do my part in helping them to adhere to the following class rules:
• Students must dress in appropriate attire. Girls should wear leotards. Shorts or leggings may be allowed for certain classes and during certain times of the year. Ninja Zone students must wear approved Ninja Zone uniforms.
• For Gymnastics & Kindergym, Socks & shoes should be left in parents' care or placed neatly inside a cubby.
• For Ninja classes, camps, birthdays, and other ninja-related activities, all students must wear socks and shoes to participate on warped wall and/or rig training.
• For the safety of all athletes, safety rules must be obeyed and children will be required to sit out if safety rules are violated. This includes but is not limited to: Standing on top of warped wall, using a forward decent on warped wall, running across gym floor and/or rig lanes, misusing mats or equipment, violating jump rules, and other safety rules as the instructor advises.
• Hair must be pulled back from face in a ponytail or secured with clips.
• No food, drinks, or gum are allowed in the gym area at any time.
• All students should use the restroom prior to entering class.
• Only registered students are allowed in the main gym and the preschool gym, and admittance is only allowed during their scheduled class time.
Parent Rules & Expectations
• I agree to observe from the parent room only. I understand that I am not permitted on the gym floor for any reason without the accompaniment of a Fusion Athletic Center employee.
• I agree that I will not communicate with my child from the viewing area. Each child should focus on the instruction they are given in class, as any distraction from outside of class could pose a safety risk.
• I understand that I may not leave my child unattended in any area of the gym unless he/she is in class, and in which case I will notify the Front Desk if I will be late picking up my child.
• I understand that gymnastics and ninja training are sports that require strength, flexibility, and skill. At times classes may not look like "work", but I understand that every game that they play, every exercise that they do, and every muscle that they stretch is preparing their bodies to be great athletes.
