Release, Assumption of Risk and Waiver of Liability
I AGREE and will comply with all policies set forth in the DASSdance Student Handbook and DASSdance reserves the right to change policies at any time for any reason.
I AGREE and understand that there are NO refunds or prorates for half or full year tuition paid in advance or monthly tuition. If I am enrolled in monthly auto pay, I may withdraw from classes if I provide a 30 day written through hard copy or email notice by the 1st of any given month.
I AGREE and understand that absences eligible for a makeup class require 24 hour advance notice of absence or the makeup is not eligible. Makeups must be scheduled in advance. See DASSdance Handbook for more information.
I AGREE that I will assume all risks associated with COVID-19 (Novel Coronavirus), which may occur during participation in classes, workshops, rehearsals, performances and other DASSdance activities. I understand and agree that while DASSdance will take reasonable precautions to clean and sanitize the DASSdance premises, the nature of dance requires that people be in relatively close contact with each other, and DASSdance has no way of monitoring the related health of fellow dancers, third parties, teachers and staff. I understand that COVID-19 (Novel Coronavirus) may go undetected for periods of time and may cause serious injury, death and other DAMAGES (as that term is defined below).
I AGREE that the student will refrain from participating in classes, workshops, rehearsals, performances, and other DASSdance activities if the student or close family member or roommate of the student has symptoms of COVID-19 (Novel Coronavirus) or if the student has other wise been in a high-risk environment or been exposed to others with COVID-19 (Novel Coronavirus).
Assumption of Risk
If you are under 18 years of age, your parent or legal guardian's signature is required.
By signing below, I agree to the following waiver and release. I understand that by being a participant in DASSdance school activities that:
1 I will have the opportunity to participate in various activities, including dance classes, workshops, rehearsals and performances held at DASSdance. I will receive information and instruction about dance and other related activities. I understand that by participating in any of these activates that I may be subjected to a variety of hazards and risks, foreseen and unforeseen, which may cause me to suffer property damage, injury or death ("DAMAGES"). I know that these DAMAGES can occur due to natural causes, the active or passive negligence of DASSdance, or the negligent or intentional acts of third parties and or fellow participants. I understand that I should therefore exercise extra care for my own person. I choose to participate in DASSdance school activities agree to pay the required costs therefore, and voluntarily assume the risks of such DAMAGES occurring while I am participating in DASSdance activities.
2 DASSdance may at times deliver participants to various third parties who are not DASSdance employees, who will conduct, supervise, guide or instruct participants in various activities. DASSdance assumes no duty to certify, monitor or verify the qualifications of any third parties involved in these activities. Participant's concerns regarding the qualifications of any third parties conducting those activities should be directed to the third parties. Participant agrees to release, indemnify and hold harmless DASSdance for the liability for DAMAGES arising out of negligence of such third parties.
3 I recognize that dance and all related activities require intense physical exertion that may be strenuous and may cause physical injury and therefore involve unique risks of DAMAGES, and I am fully aware of the risks and hazards involved. I understand the nature of Marley floors, mobile floor barres and other dance equipment, and the consequences of wearing ballet slippers, pointe shoes, socks or bare feet and other related dance footwear, and I accept full responsibility for my own choice of footwear. I am satisfied with the condition of DASSdance facilities and hereby assume all the risks inherent in the activities there. I voluntarily assume the risks of any and all damages I may incur at DASSdance, unless such DAMAGES are directly related to the negligent operation of the facilities by DASSdance.
4 I (we), the undersigned, hereby waive and release any and all we may have regarding ownership or use of video or photographs bearing my (our) image while at DASSdance facilities or off site DASS events, and grant permission to said DASSdance to use and display said video or photographs for commercial or non-commercial use.
RELEASE AND WAIVER OF LIABILITY
In consideration of being accepted by DASSdance school to participate in any activity, I hereby agree to hold DASSdance, it's employees, agents, contractors or owners, harmless from any and all liability, action, causes of action, debts, claims and demands of every kind and nature whatsoever which I now have or which may arise from or in connection with my DASSdance activity. The terms of this agreement shall also serve as release and assumption of risk from any heirs, executers and administrators for all members of my family.
I further agree that this RELEASE and ASSUMPTION OF RISK agreement and WAIVER OF LIABILITY is intended to be as broad as permitted under law, and that if any portion thereof is held invalid, it is agreed that all other portions shall continue in full legal force and effect.

Artistic Director
Daniel Wilkins is available Monday-Thursday from 1:00- 2:00p.m. Or anytime via E-mail. Please contact Daniel with any concerns that you may be having. If you need to speak to an instructor directly, inquire with Daniel to schedule an appointment for a private meeting.
Practice Wear
Practice wear is required for all classes. If a student is not dressed appropriately they will be asked to sit and watch class.
Student's hair must be tied back and secure for class. No gum or jewelry.
Practice Wear can be purchased online at Discount Dance Supply or at "Centerstage" in the U District, Seattle.
Attire for All Classes: Any style or color of leotard and tights, leggings or bike shorts.
Ballet Classes: Shoes ~ Pink professional quality ballet shoes. (Bloch, Sansha or Capezio shoes are recommended.)
Jazz: bare feet or jazz shoes
Contemporary: bare feet
Note- Dance is a disciplined and structured art form. The appropriate attire will allow the Instructor to accurately correct a dancer's placement.
Hip Hop Classes: Clean tennis shoes & Athletic wear. No big sweatshirts.
Boyz Class: Athletic wear and bare feet or appropriate dance shoes.
Snow or Bad Weather
The studio will usually follow the Seattle Public Schools decision on class cancellation for bad weather. DASSdance reserves the right to cancel classes at our discretion. Any classes that are missed or canceled can be made-up by attending another class close to the same level.
Holidays
The studio will be closed for the following Holidays: Thanksgiving (Nov. 23-24), Holiday Break ( Dec. 18 - Jan.1), Mid-Winter Break (Feb. 19 - 24), and Spring Break (April 9-14). Our monthly rates factor in Holidays.
Attendance and Tardiness
If a student is more than 7 minutes late they will not be allowed to participate in class that day. Students coming in late cause too much distraction, delay, and will not be properly warmed-up for class. Good attendance is imperative, as absences and tardiness can hold back an entire class; and the studio cannot jeopardize its responsibilities to the rest of the class for one student. If more than 3 classes are missed during a rehearsal period the dancer WILL NOT be allowed to participate in the up-coming performance. Please make every effort to attend every class. Missed classes may not be deducted from tuition payments. Make-up classes should be attended.
Make-Up Policy
If for any reason your child cannot attend class, they are encouraged to attend a make-up class within the month. (No extra charge for make-up.) See Daniel for class placement.
Dropping off and picking up dancers
Parents are welcome to drop their child off for dance class. Parents may want to stay for young children in classes. Please be on time to pick up your child after class.
Communicating with Instructors
We work very hard to keep you informed by e-mailing newsletters and updating our Website and via our Facebook group. However, it is your responsibility to keep up to date by reading newsletters, e-mails, and checking the information board and Website. If you have a question please do not ask instructors during or in-between classes. Please contact Daniel with any questions. If you need to talk personally to the director or an instructor please schedule an appointment so that the instructors can give you their full attention.
Performances Dress rehearsals for performances are the week leading up to the shows. Please reserve these dates now because your child could perform any one of these days. The dress rehearsals are mandatory for anyone wanting to perform in the shows. (Dancers who do not attend the dress rehearsal WILL NOT be allowed to perform in the shows.
Production Fee
Production Fees will run $80 for Alice in Wonderland, $80 for All 4 1 dancepalooza and $100 for Mini Nutcracker. Production Fees cover rehearsal time with Choreographers, Theatre and Costume costs. The Production fee is due 1 month before the Performances. You may pay money towards your Production fee at anytime before it is due.
Payments
There is a $36 fee for ALL returned checks and automatic payments.
Tuition is set to automatic debit from checking, savings, and credit card accounts. (1st of each month)
Tuition is based on a Fall Session - September - December and Spring Session: January - June. Payments are broken up into 10 monthly payments for your convenience. Tuition is non- refundable and is paid to save your child's place in class. If your child has decided to stop attending class you must send an email to the director at least 2 weeks before payment is due. If we are not contacted we will continue to bill you for saving your child's place in class. You are not charged for Holiday Break, Mid-Winter Break, or Spring Break.
Registration
All dance class registration is done online. This offers parents more convenience and access to their account 24 hours a day. You may choose for your automatic payments to come out of your checking account or credit card account. Go to www.DASSdance.org to register online.

If I am enrolled in monthly auto pay, I may withdraw from classes if I provide an email notification between the 1st and 5th of any given month prior to the month you wish to withdraw.
Example: You want to withdraw your child starting in February. You must notify the office by email between the 1st and 5th of January.
