LIABILITY
While Simply Ballet, in Port Coquitlam, British Columbia, takes all precautions to ensure the safety of its students and teachers, it cannot accept any responsibility for accidents or injuries during class or on its property. By acknowledging below, I agree to hold Simply Ballet harmless for any injury or accident that may take place during class or on its property.
TUITION
Tuition is based on the amount of registered hours for the total number of classes in the season from September to June. This is then divided into 10 equal payments, payable on the 1st of every month, beginning September. The first and last month's tuition payment (September & June) installment is due at the time of registration. June Tuition is a deposit fee that becomes non-refundable as of November 1st. Fees can be paid monthly by pre-authorized debit (10 equal installments) or in full by cheque, Direct Debit, Visa or Mastercard. A 3% fee on each transaction will be added to all credit card payments to offset the processing fee Simply Ballet must pay to the credit card processor. All other payment methods will carry no additional charge.
Tuition fee is payable even in times of virtual classes provided on zoom. Classes will run as schedules with lessons provided in a virtual platform. There is no withdrawal policy for classes due to virtual transition. All classes will transition from in studio to online if deemed necessary by the legal authorities or Simply Ballet.
Once the tuition payment has been processed it is non-refundable. A NSF Fee of $25 will be charged on all returned cheques, insufficient funds or declined payments. After the 15th of the month, tuition fees not paid are considered past due and will automatically incur a $25 late fee. Simply Ballet reserves the right to remove the student from participating in classes until full payment has been received. All other fees - costumes, private lessons, exam fees, competition fees, etc. will be invoiced as incurred. Please note: No tuition adjustments or credits are given for missed classes. Only those classes cancelled by Simply Ballet will be offered a make-up lesson or credit.
COSTUMES
All dancers are required to purchase a costume for each performance/competitive class. A costume deposit is due at the time of registration or Sept 1st. Please note - this is a deposit only. The total cost of the completed costume may be between $75-$150. If the total price of the costume is less than the deposit, the remaining amount will be returned to you. Costume prices include the outfit, accessories, and props only. Tights, dance shoes and some hair pieces are not included in costume prices. All costume deposits become non-refundable as of November 1st.
Each dancer is responsible for taking care of their costume. Should a costume piece become damaged, the student may not be able to perform in competition or the year end recital. If time permits, Simply Ballet will try and order a new costume. The full cost of the original costume, the replacement costume and any shipping charges will be the responsibility of the parent/guardian of the student.
COMPETITIONS
Entry fees are required for each competition and festival. These fees are set separately by every competition. Simply Ballet will collect a competition entry fee deposit at registration or Sept 1st For Solos, the entry fees range from $40-$150, Duos/Trios $30-$70, and Groups $35-$60. A surcharge will be added to each entry by Simply Ballet to cover administrative paperwork & postage. Solo/Duo/Trio Competition Entry Fees will be due October 15th. Group Competition Entry Fees will be due November 15th.
Competition Entry Fees are non-refundable once paid out to the competition. Should a dancer become injured and unable to perform, the competition requires written documentation from their doctor in order to refund entry fees if applicable.
WITHDRAW POLICY
If for any reason a student chooses to discontinue classes before the end of the dance school term, it is understood and agreed that the parent/guardian will fill out a withdrawal form and submit it to Simply Ballet prior to the 20th of the month. If not received by the 20th of the month, tuition fees for the following month will be processed. June 2019 tuition fees are non-refundable as of November 1st. Failure to attend class does not constitute withdrawal. Students in Full Time Training, Level 2 and up have an expectation to continue from Sept - June. There is no withdraw policy for these levels. Class schedules become fixed after October 15th, tuition will be based on hours of training based from that point forward. It is understood that there will be no refund for any payments previously made to Simply Ballet if a student decides to withdraw at any point in the season, for any reason. If enrollment of the student at Simply Ballet is dismissed for any reason, by decision of the Artistic Director, it will be understood that there will be no refund on any fees previously paid to Simply Ballet, and the parent/guardian of the student will be required to pay for any competition fees or costume fees in the coming months that have been previously paid for.
STUDIO POLICIES
Every Simply Ballet Student is expected to behave in a disciplined, responsible and courteous manner when on the studio premises or representing Simply Ballet. The Director of Simply Ballet reserve the right to suspend or expel any student whose attitude, class attendance, work habits, teamwork or overall conduct is judged unsatisfactory. All fees previously paid to Simply Ballet on behalf of the discharged student will be forfeited by the student. Participation in upcoming festivals, competitions and exams, entered by the studio on the student's behalf will also be forfeited, without refund. Students and parents are expected to be polite to one another as well as all Simply Ballet faculty and staff. Any rude, aggressive or harassing behavior from students or parents will not be tolerated. Any use of offensive language or harassment to employees of Simply Ballet will result in immediate dismissal without refund. Simply Ballet reserves the right to change the faculty/teacher/choreographer of any class throughout the year for any reason. Simply Ballet will strive to supply its clients with an equally qualified instructor to uphold the studios standard of high quality dance education.
NOTICE OF NON-DISCRIMINATION
Simply Ballet admits students of any race, colour, sexual orientation, national and ethnic origin to all the rights, privileges, programs and activities generally accorded or made available to students at the School. It does not discriminate on the basis of race, colour, sexual orientation, national and ethnic origin in administration of its education policies, admission policies, scholarship programs or any
other school-administered programs.
PHOTO/VIDEO
As the parent/guardian of the participating student named above (the "Student"), I grant Simply Ballet permission to record, take and use photographs, video footage, voice recordings or other audio and visual recordings of the Student, including the Student's name, age, image, voice and likeness, including any altered or new versions or derivative works of any of the foregoing (the "Recordings"). I understand that the Recordings may be used and distributed for any purpose including educational and promotional materials, in any format, via any technology, now known or hereafter invented. I hereby waive any right that I may have to inspect or approve the Recordings. I understand that the Recordings may be used or altered in part or in whole at the sole discretion of Simply Ballet and will be used without compensation to myself. I release Simply Ballet from any claim that I may have for any use of the recordings.
