I hereby agree to the Rules and policies stated herein, and I recognize that programs at Acrobatic Rock-n-Roll Academy of Boston, LLC (hereinafter "ARRA") require physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand that it is my responsibility to consult with a physician prior to and regarding my, or my child's participation in the programs of the ARRA. I represent and warrant that I am/ my child is physically fit and there are no medical condition that would prevent full participation in the programs. In consideration of being permitted to participate in the programs of the ARRA, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the programs, or anywhere in the facility or on parking lot. I knowingly, voluntarily and expressly waive any claim I/ my child may have against the Acrobatic Rock-n-Roll Academy of Boston, AKA Acrobatic Rock-n-Roll Academy of Boston, LLC and its employees and instructors for injury or damages that me or my child may sustain as a result of participating in the program, or anywhere in the facility or on parking lot. I, my heirs or legal representatives forever release waive, discharge and covenant not to sue Acrobatic Rock-n-Roll Academy of Boston, LLC and its employees and instructors for any injury or death caused by their negligence or other acts.
(For Parents: As the Parent or Legal Guardian of the student(s), I hereby consent to their participation in the programs offered by Acrobatic Rock-n-Roll Academy of Boston, LLC, including and not limited to dance, gymnastics, tumbling, acrobatic Rock-n-roll, fitness, birthday parties, special events & activities including inflatables, camps and any and all other programs offered by Acrobatic Rock-n-Roll Academy of Boston.)
Myself, and or as the Parent or legal Guardian of the above named child/children, I hereby agree to individually protect for the possible future medical expenses which may be incurred by me, and or my child/children as a result of any injury sustained while participating of any program of instruction, recreation, or competition at, for or under the direction of Acrobatic Rock-n-Roll Academy of Boston, LLC, or for any injury sustained anywhere in the facility, or outdoor or parking lot. In any event of an accident or emergency, I hereby authorize myself, and or my child/children to be transported to a hospital for medical treatment and I hold Acrobatic Rock-n-Roll Academy of Boston, LLC and its representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses that may be incurred by me or my child or children as a result of any injury sustained participating at or for Acrobatic Rock-n-Roll Academy of Boston, LLC.
**Each Student/Family is required to have a credit card on file for payments.
Non-refundable, non-prorated Annual Registration fee is $50.00 per child or $60.00 per family (September 1st through August 31st) and due at the time of Registration to classes, camps.
(A) Recreational & Competitive TEAM Classes:
Prices are based on the cost for the total season and divided into 10 equal monthly payments, and already factor in all holidays and studio closures, due by 1st day of month. Payment determined by the number of hours of class taken each week. The fee cannot be adjusted due to absences, cancellations, holidays, shorter/longer months. Tuition is for your child's entire academic year, not for the month itself. Tuition installment can be paid via cash, or check, credit card, debit card, or through your Customer Portal. You will have a grace period of 5 days from the Payment Due Date to pay your child's tuition. ** If your child's tuition installment is not paid before the 6th day from the Payment Due Date, your credit card on file will be charged. This will occur on the 6th day from the Payment Due Date. $10 late fee will be charged, if the tuition installment is not paid after 6th day after the Due Date. Failure to pay by the Due Date may result in removal of your child from class.
(B) Competitive ARRA Team (Acrobatic Rock-n-Roll Couples, Formations, Duos):
There are Minimum Skill Requirements for Acceptance into Competitive/Performance Team. Head Coach Approval is required. Minimum of 3 hours of training are required to participate in the Team. Commitment to at least two Acrobatic Rock-n-Roll competitions per academic year 2017-2018 is mandatory: Winter Cup on 12/10/2017 & USA Championship on 05/27/2018. Other performances and Regional Dance competitions are recommended. Please refer to the Studio Schedule and Important Dates for closures and competitions. Appropriate jazz shoes are mandatory for each rock-n-roller. A costume will be required for each routine. By signing up your child to a Competitive Team you agree to pay Competition Fees and Costume Fees, which are separate from Tuition, and due no later than 1 month before the scheduled competition, to avoid any late fees.
By signing up your child for classes at ARRA you agree to the above payment policy.
I hereby authorize ARRA to charge credit card provided on my account. I understand and agree that if I write a check with insufficient funds, or it is returned for other reasons, I will be charged $30 fee, or if I am late with payments, I will be charged $10 late fee.
ARRA Camp must comply with regulations of the Massachusetts Department of Public Health and be licensed by the local board of health.(105 CMR 430.000). I agree to complete & sign the Summer Camp Registration Form Summer Camp Registration Form
including release form, emergency numbers and to provide up-to-date health records, and sign ARRA Camp Policy. These forms MUST be returned prior to attending ARRA Camp.
