By signing this release, I understand that dance is a physical activity that may result in injury or illness occurring during or after participation, and Move shall not be responsible for any injury suffered by the student. The release extends to Move, its owner, officers, instructors, employees and locations of classes now or in the future. The Release extends to every claim, demand or liability of any kind based on any injury or damage described below.
I understand that dance activities as conducted and taught at the studio have inherent risks of injury. These risks include muscle pain and pulls, broken bones, ankle injuries and other personal injury. I recognize that the student is exposing himself/herself to such risks when undertaking dance activities. I understand that these risks cannot be fully eliminated without jeopardizing the essential qualities of the activity. The student and I assume and accept all risks of injury or damage resulting from such dance activities. The student's participation in this activity is purely voluntary, and the student elects to participate, and I join in that election, in spite of the risks.
I further understand that I have been advised of the need for the student to be covered by adequate insurance to cover any injury or damage that may be suffered while participating and I have obtained such insurance or have agreed to bear the costs of any such injury or damage myself.
I understand the terms of this Release and have had the opportunity to consider and discuss it with such individuals and advisors as I deem appropriate. By signing and dating this Release, I confirm that I have read the Release in full, that I understand its terms, and that I agree with those terms. I further confirm that by signing this Release, I acknowledge that if the student is hurt during participation in this activity, I may be found in a court of law to have waived my right and any right of the student to maintain a lawsuit against the studio on the basis of any claim which is released hereby. In consideration of the student's being permitted by the studio to participate in the activities provided by the studio, I further agree to hold harmless and indemnify the studio for any and all claims which are brought by or on behalf of any student who is a minor and which are in any way connected with the activities performed at the studio by any such minor student.
I understand that Move requires parents/guardians of students with a serious or life threatening illness, disability, or allergy to be present at Move and Move events with their child.
I understand that in the case of an emergency, "Guardian 1" will be notified immediately. If "Guardian 1" cannot be reached, "Guardian 2" will be notified immediately. If "Guardian 1" and "Guardian 2" cannot be reached, I hereby authorize Move, its owner, officers, instructors, and employees to take any steps deemed necessary to make medical attention available, including but not limited to ambulance, physician, hospital, or any medical services and the cost of such medical services shall be born exclusively by myself.
I understand that I am responsible for picking up my registered student on time. If student is not picked up within 10 minutes after the last class of the day, a fee of $2 per minute until pick-up will be applied and charged the same day.
I understand that Move instructors will sometimes find it necessary to use appropriate physical contact to make corrections or aid dancers in their training. This includes but is not limited to helpings students stretch, adjusting posture, correcting alignment, and identifying correct placement.
I hereby give Move, its owner, employees, and any other persons and entities acting with its permission, or upon its authority, the absolute right and permission to take, copyright, use, and publish any photographs or videos of or concerning my child for the purpose of any Move advertising, promotion, or other purpose consistent with the Move mission. I agree that any such photography or videos is the exclusive property of Move, and I hereby waive all rights thereto. I further waive any and all rights to inspect and/or approve any printed or electronic material that may be used in conjunction with the photographs or video, or to approve the use to which the photographs or video may be applied.
I understand that Move, its owners, officers, instructors, employees and locations of classes, are not responsible for any property loss or damage suffered by a student, parent/relative, or guest, which results upon entry into or presence in the studio or its surrounding property.
I understand that I am responsible for the cost of any damages made against Move equipment or facilities, or any damages made at a venue during a Move event.
Non-registered children must be monitored by a guardian at all times.
Move reserves the right to refuse service.
Students are not permitted to take class if he or she has experienced fever, diarrhea, vomiting, chickenpox, measles, hand, foot and mouth disease, strep throat, influenza, or lice in the last 24 hours. Students who are continuously coughing, experiencing difficulty breathing through their mouth or nose, or seem too lethargic or ill to perform safety may be asked to leave or sit during class.
Phone numbers, email addresses, medical information, and billing information must remain current. MDA is not responsible for failure to provide and/or receive information due to inaccurate contact information, nor responsible for costs that may arise due to inaccurate, outdated or compromised billing information.
No animals, except for certified service animals are permitted inside of MDA.
Choreography provided by MDA is property of MDA and cannot be performed, used, or shared without explicit consent from the instructor/choreographer and studio owner.
Classes are prohibited from being videotaped in entirety or majority.
Students, parents, guests, or spectators found as hinderance to maintaining a positive, nurturing, and safe environment inside of MDA and/or at MDA events will be immediately removed from the studio without any refunds.
The use of profanity inside of MDA is strictly prohibited.
Parents/guests/spectators are strictly prohibited from entering, interrupting, or distracting classes in session. If you would like to speak with your dancer's instructor, please leave a memo with the front desk to schedule a meeting and/or phone call.
Instructors reserve the right to send a student out of class due to behavior deemed inappropriate, disrespectful, and/or a hinderance to the function of the class.
Instructors reserve the right to close viewing windows for any reason.
Studios are closed until an instructor is present. Dancers are not permitted inside of studios alone.
Students may not attend class if more than 15 minutes tardy (1 hour class duration) or 10 minutes tardy (30 minute class duration). Dancers must wait for an appropriate time and/or instructor's approval to enter a class late.
MDA reserves the right to cancel classes and/or end classes early without notice and without refunds.
In the event that no registered student is present for a class within 10 minutes of the start time, the class will be cancelled.
In the event that only 1 or 2 registered students are present for a class, the instructor reserved the right to end class at 30 minutes (1 hour class duration) or at 20 minutes (30 minute class duration).
The "student break area" is for students only. This area is prohibited from being used as a lobby or class viewing area.
Dancers may not walk in and out of class.
No food and/or drinks other than water are permitted inside of studios.
Dancers must meet the class age requirement unless given approval to enroll by the instructor.
Dancer's levels are determined by MDA instructors. Classes level 2 and 3 must have instructor's approval prior to enrollment.
Dancers in Ballet 2 and up are required to take at least two Ballet classes to maintain their level.
The studio lost and found will be discarded every two weeks.
Annual registration fee of $25.00 (add $10.00 per sibling) is due at the time of registration and August 1, 2020.
If you withdraw and choose to register again, the annual registration fee will be due again.
Tuition is automatically deducted from the credit card on file on the 1st of every month (no cash or checks accepted).
Tuition payments made after the 3rd of the month are regarded as late and subject to a $20.00 late fee.
Declined or insufficient fund credit/debit card notices will be subject to a $10.00 reprocessing fee.
Declined or insufficient fund credit/debit cards will be reprocessed until the transaction is complete.
Students with a tuition balance after the 7th of the month are prohibited from taking class until tuition and late fee is paid.
Pricing is based off of an average of 4 lessons per month. Prices do not increase if there are more than 4 lessons in the month nor do prices decrease if there are less than 4 lessons in the month.
Past due balances will be processed with every tuition transaction. Oldest fees must be settled first.
Private lessons must be scheduled through the front desk. Private lessons are charged to the card on file the same day the lesson takes place. Failure to provide at least 24 hour cancellation notice for a private lesson will result in 50% of the private lesson fee due and processed the same day.
No refunds are granted under any circumstances for tuition fees, registration fees, recital fees, private lessons, absences or missed class time, failure to attend/complete the month paid for, studio closures, observed holidays, combined classes, shortened classes, canceled classes, and/or failure to give timely notice of withdrawal.
To withdraw from Move and stop automatic tuition payment deductions, written notice ("drop form") must be provided and confirmed with Move at least 14 DAYS before the 1st of the month that you wish to end enrollment. Enrollment is ongoing until notice is provided.
Lack of attendance will not automatically remove your student's enrollment or stop automatic tuition payment deductions.
