I understand and agree that in participating in any dance class there is a possibility of physical injury and/or transmission of COVID19, despite any and all safeguards which can be implemented by DANCE FORCE. I agree to release and hold harmless DANCE FORCE, including its teachers, dancers/family members, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold DANCE FORCE liable for any personal injury, specifically including the transmission of COVID19 to my student or family members, which may occur on the premises before, during or after classes. Furthermore, I understand that I should be aware of my, my family members, and/or my child's exposure to COVID19 and agree to notify the studio immediately if any of my family members, including my child, are exposed to and/or test positive for COVID19. Upon notification, a decision will be made by the Directors as to whether my child can continue live instruction or online instruction. Dance Force directors have the absolute right and authority to ask my child to continue instruction online and/or reserve the right to close the studio if necessary, to meet health mandates.
Further I understand and acknowledge that because of the physical nature of DANCE, there may be physical contact between directors, employees, instructors, and students during dance instruction. I understand that at times, for proper instruction and safety, physical contact is required and necessary, but due to the ongoing safety concerns related to the transmission of COVID19, contact will be limited by instructors and completely discouraged between students. Further I understand and acknowledge that I will instruct my dancer to abide by all the additional temporary rules and regulations imposed as a result of the COVID19 health crisis. These regulations may be amended from time to time as the health crisis requires and are necessary to meet all state and federal safety requirements, as well as the safety of our dancers and their families.
Dance Force Policies 1
1. Tuition Policy
Tuition Fees are due on the first scheduled lesson of each month. The Fee becomes past due after the 10th of the month. A $25 Late Fee is charged to the account if payment is not received by the 10th of the month. Dance Force strongly encourages you to adapt the habit of paying tuition on the first lesson of the month. A 20% Sibling Discount applies after the child with the highest tuition pays full price. This discount does not apply to boys as they already receive a discounted rate. Tuition is based upon the number of class hours your child participates in each month. The cost of tuition is set forth in the attached Schedule of Tuition Fees.
Tuition is not reimbursed for a partial month if your dancer quits dance. Once paid at the beginning of the month, they are committed for that month.
If the dancer's tuition is not paid in full by the last day of the month, the dancer will not be able to attend class(es) until the account is current, including payment of all assessed late fees. All payments on account for are applied to open tuition first. Tuition must be paid before other related fees.
2. Payments
Payments can be made at the studio by cash, check, debit card, or credit cards (VISA, Discover, & MasterCard) or mailed to PO Box 4060 Clarksville, TN 37043. We offer a discount for payments made by cash or check. We encourage you to take advantage of Automatic Draft from your credit/debit account. We can debit your charge/debit card or checking account the amount you request on the 5th or 10th of each month, September-May. A convenience fee of approximately 3% is charged to all accounts that are paid by credit/debit card. This is a fee that is assessed to us through the credit card companies.

3. Returned Checks/Automatic Drafts
We are charged a fee by our bank for handling a returned check/draft. We must include this fee when we settle the check/automatic draft. Dance Force will assess a $35.00 Fee for any returned check to your Dance Account. You must make arrangement to pay the fee and the returned check amount within five days. A loss of check writing privileges will take effect after a second returned check. Alternate payment must be arranged through cash, money order, credit, debit, or EFT.
4. Class Information
Our classes are based on a minimum of 10 dancers per class. We will review class sizes at the end of October. If any classes have fewer than 10 students, we will have to close that class and move students to a different class. Under such circumstances, parents will be furnished with information regarding other available classes your child may attend which would then be selected by Parent.
5. Dance Force Logo
The Dance Force Logo is not to be used for personal products, including the production of t-shirts or dancewear. Use of the Dance Force Logo, and name "Dance Force" and "Dance Force, LLC" is expressly prohibited without prior written authorization of the Directors.
6. Inclement Weather
Our primary concern is for safety and best interest of our students, their parents, and our teachers. If we decide to close the studio due to inclement weather, we will announce the decision no later than 1:00 pm. Please call the studio and listen to the answering machine (931) 552-2223 or go to our website at www.danceforceclarksville.com to see if we have posted the closing. You may also check our Facebook Page, The Dance Force, for any weather related closings.
NOTE: We do not follow the same inclement weather policy as the Clarksville Montgomery County School System, and we reserve the option to open the studio when Montgomery County schools are closed.
We will make up classes that are missed due to weather. If your child is in class and we have a tornado warning, we will place all students in the safest room in the building. Also, we suggest that you take cover and do not attempt to pick up your child because it might place all of you in greater danger. Sometimes we might close after classes have started if the weather conditions worsen. If this happens, we will contact you as soon as possible. It is imperative that Parents always provide us with up to date contact information.
7. Contact Information
We use the information on file to notify you about important issues, studio closures and in case of emergencies. Please keep your contact information up to date at all times by submitting changes to the Front Desk. It is imperative that Parents always provide us with up to date contact information.
8. Medication/Illness
Parents must fill out a Request for Assisted Self-Administration Medication Form if you want your child to take medication while at dance. Parents must provide the medications, and all such medication must contain a pharmacy label. We encourage dancers to follow the classes from the sideline while sick or injured, especially before the Spring Recital. Watching the class will make it a lot easier to catch up with the rest of the class later. We do ask that a child with a contagious illness or a fever stay at home and not return to class until he/she has been symptom free for 24-hours, the same to be determined by Parent. Please notify the studio by calling 931.552.2223 and leave a message before the class starts if your child will not attend.
9. Injury
Even though our staff is insistent on teaching the correct technique, accidents do happen. If your child should be so unfortunate as to have an injury while in class, we will take the appropriate action including immediate medical attention if the senior staff-member on duty feels it is needed. We will contact the Parents immediately.
10. Lost Items
Each student should label all personal items to include shoes. We maintain a lost and found for lost items turned into our staff. The Dance Force is not responsible for any lost, damaged or stolen property.
11. Recital Costume Deposits and Balances
Recital costume deposits are due on November 15 - the deposit is $50.00 per costume. Costume Deposits for Recital are non-refundable. We will not order your child's costumes without the deposit being paid. Recital costume balances are due when you are notified between March and April. No costume will be delivered to a student who has an unpaid balance for costumes or tuition. Recital is in late Spring.
12. Recital Fee
A Recital Fee of $123.60 if paid by credit/debit card or $120.00 if paid by cash is assessed to the first Recreational student in a family. Each additional student in the family is assessed a Recital Fee of $46.35 if paid by credit/debit card or $45.00 if paid by cash. The Recital Fee will pay for four (4) tickets (per family) to the show and one (1) souvenir T-Shirt to be worn in the Recital Finale as well as a jump drive of the Show. This Recital Fee includes expenses such as the price of admission, rental of facility, and custodial/security staff. Payment of the Recital Fee is due March 15th.
13. Restroom Policy
It is very important that students use the restrooms prior to entering class. Dancers, especially ages 3 - 5, need to be accompanied to and from the classroom. We ask that Parents, when possible, remain in the lobby during your child's class time in case a child must use the restroom during the hour. (This also helps to speed the transition from ballet to tap shoes mid-way through class.) If a student's Parent is not readily available to escort the child to the restroom and back to class, the teacher will need to take all the students in the class to the restroom, they will all remain in the restroom until the child is finished and will return to the classroom together. 14. Referral Fee
If a dance family refers a new student, that pays tuition for 3 consecutive months, they will then be given a $15.00 credit on his/her account. You can refer as many new dancers as you want, but to get the credit a new student must attend and pay for 3 months.
15. Attire-Girls
Ballet: Black leotard, pink tights, pink ballet shoes (no slippers), and hair must be up off face (ponytail or bun)
Jazz: Dancewear, bootie shorts, bra tops, foot undeez/caramel jazz shoes. Hair up off face
Tap: Dancewear, bootie shorts, bra tops, tan tights, caramel tap shoes. Hair up off face
Hip Hop: Long shorts/sweat pants, t-shirt, and tennis shoes
Attire-Boys
Ballet/Jazz: Black dance pants, fitted t-shirt, black ballet shoes
Tap: Ballet attire or basketball shorts, t-shirt, black tap shoes
Hip Hop: Loose shorts t-shirt, tennis shoes

16. Liability/Hold Harmless Disclaimer
In consideration of the services received from Dance Force, LLC, I (expressly on upon behalf of myself, spouse, student/child, family members, heirs/assigns, collectively hereinafter referred as "RELEASORS") hereby release Dance Force, LLC, its owners/members, employees, instructors, and volunteers (collectively hereinafter herein as "RELEASEES") from liability for all actions taken in good faith during any and all attendance as well as any and all functions attended by students and families of Dance Force, and I, individually and behalf of the RELEASORS, hereby waive my/our right to sue Dance Force, LLC, its individual members, employees, instructors and volunteers from any and all claims resulting in any physical injury, illness (including death) or economic loss I/we may realize as a result of RELEASORS' participation in any activities of Dance Force.
RELEASORS are voluntarily participating in the activities of Dance Force, and RELEASORS understand that there are risks of harm associated with the student's participation in activities of Dance Force, such as physical and/or psychological harm, pain, suffering, illness, disfigurement, temporary or permanent disability, death or economic loss. Such injuries or outcome may arise from the student or other's actions, inactions or negligence, or the condition of the facility. Nonetheless, RELEASORS assume all risks of student's participation in any and all activities of Dance Force, whether known or unknown to RELEASORS, including travel to and from scheduled performances, practices, conventions, dress rehearsals, competitions, recitals and all other events incidental to any activity of Dance Force. It is the Parent's responsibility to notify Dance Force if the student suffers from any physical condition, injury, or illness which could cause the student further physical harm by the student's performance in the activities of Dance Force. Parents are recommended to Student examined by a medical professional to verify their physical ability to engage in Dance Force activities. Ultimately, Parent will decide whether student/child is physically able to engage in any activities of Dance Force.
RELEASORS hereby acknowledge that although Dance Force will designate and schedule the locations of the studio's participation in performances, practices, conventions, dress rehearsals, dance competitions, recitals or other events, the student's travel to and from any such events is the sole responsibility and control of Parent/student. It is acknowledged that from time to time, RELEASEES may transport students of Dance Force to and/or from events/activities of Dance Force per authorization of the student's guardian/parent. It is hereby acknowledged and agreed that such transportation is gratuitous, and the execution of this Document shall evidence the undersigned's release and waiver of any claim whatsoever against any RELEASEE or volunteer to the extent any student may sustain physical injury, illness (including death) or economic loss during such gratuitous transport of student to and/or from activities of Dance Force.
RELEASORS agree to defend, indemnify and hold completely harmless Dance Force its owners/members, employees, instructors, and volunteers from any and all liability, loses, costs, fees, damages, injuries, actions, and causes of action of any and every kind of nature or whatever, arising out the activities of Dance Force. In consideration of the services received from Dance Force, LLC, RELEASORS herewith release Dance Force, LLC and their owners/members, employees, instructors, and volunteers from liability for all actions taken in good faith during any and all attendance as well as any and all functions attended by students and families of Dance Force. In addition, permission is given to Dance Force to use any and all photos and or video material deemed appropriate by Dance Force and its employees, servants, and agents.
17. Mutual Duty to Maintain Safety of Premises
Your child's safety is our paramount concern. Dance Force, its owners/members, employees and instructors regularly inspect the interior premises, including the Marley flooring, mirrors, fixtures, dance equipment of the Dance Force studio for defects and/or damaged equipment, and repairs and/or replacement will be promptly made upon notice of such defects/conditions. Each Parent and student herein acknowledges their duty to immediately notify the Directors of any known defects and/or dangerous conditions concerning the interior premises of the studio including any defects concerning the Marley flooring, mirrors, fixtures, and dance equipment.
18. Costume Payment Agreement
I understand that I shall be responsible for payment of the required costume deposit that will be due in November, and that the costume deposit is non-refundable. I understand that once the costumes(s) have been ordered, I am responsible for the balance owed for the costume irrespective of whether my child will no longer be participating in such class. Costumes will be released to Parent/student only when account balances are paid in full. Competition Team Parents and Performance Team Parents understand that if the Competition Team Dancer or Performance Team Dancer resigns from his/her contract, the property of all costumes (regardless of Parent's purchase thereof) become the property of the Dance Force, without any refunds or credits, and must be turned into the studio within one week of the resignation.
19. Tuition Payment Agreement
I understand that Parent is liable for monthly tuition until Parent notifies the Front Desk personnel that student will no longer be participating in the dance classes or classes for which tuition is owed. If my student(s) drops in the middle of the month there will be no refund of tuition. It is imperative that Dance Force be notified of any "class drops" prior to the order of class costumes. I understand that in the event I should default on any terms of these Policies, to the extent Dance Force acts to enforce its rights under this Agreement, I acknowledge and agree that I shall be responsible for the payment of all costs and expenses, including attorney's fees, collection fees, court costs, and any contingent fees assessed by collection agencies of not less than 35%.
20. Parent Gossip
We ask that all parents please be aware of what you say to your child, and other parents and dancers. We know intentions are good; however, sometimes even the smallest things can get taken the wrong way or turned around. All problems or concerns need to be addressed directly to the Directors. We also ask that parents do not critique other children and try to stay positive. Any parents that are non compliant will receive a warning and if the problem persists, the parent will not be allowed come in to the studio. We do not like to do this but we want the studio to be a positive and happy
