WARNING! CATASTROPHIC INJURY, PARALYSIS, OR EVEN DEATH CAN RESULT FROM THE IMPROPER CONDUCT OF THE ACTIVITY.
In consideration of Sparkle City Gymnastics accepting myself and/or my child into participation and/or training in gymnastics, an activity which I hereby acknowledge involves greater than normal risk of injury, I agree, for myself and as my child's guardian to assume responsibility for all risks, costs, or losses sustained by me, my child, or my child's family in connection with participation in gymnastics classes, programs, lessons, meets, birthday parties, open gyms, field trips, or any other activities connected with Sparkle City Gymnastics.
I give my permission to Sparkle City Gymnastics and/or appropriate medical facility to make whatever emergency (first aid, disaster evacuation, etc.) measures as judged necessary for the care and protection of me and/or my child while under the supervision of Sparkle City Gymnastics.
In case of an emergency, I understand that I and/or my child will be transported to an appropriate medical facility by the local emergency unit for treatment if the local emergency resources deem it necessary. Transportation will be at my own expense. It is understood that in some medical situations, the staff will need to contact the local emergency resource before the parent, physician, and/or others acting on behalf of the parent or family can be reached.
Further, I hereby release and agree to hold harmless and to indemnify Sparkle City Gymnastics employees, owners, or volunteers from any claims, losses or expenses incurred or on the behalf of me, my child, or my child's family.
I understand that my child's photograph or video may be taken during the course of class instruction, during a special event at Sparkle City Gymnastics, or at a function sanctioned by Sparkle City Gymnastics. I hereby grant permission to Sparkle City Gymnastics to use my child's photograph or likeness in any publicity or promotional publications (e.g., website, newspaper ads, bulletin boards, newsletters, programs, brochures, public broadcasting releases, etc.) and to allow the news media to film and/or photograph programs and activities for broadcast purposes.
Tuition is due on or before the 1st day of the month and may be paid by cash, check, or credit card. SCG requires a credit card to be placed on file in the event an account becomes past due. If your balance is not paid IN FULL by the 3rd day of the month, the credit card on file will be charged the full balance plus a $25 late fee. If the card is declined and the balance is not settled by the 5th of the month, your child will not be able to participate in the program.
Students are automatically re-enrolled for the same class every month for the duration of the academic year (August - May). Tuition for the year will be the same from month to month. While some months have more than 4 weeks in them, you will not be charged a higher rate for these months. Accordingly, months that are shorter due to holidays or other closures will not be prorated or discounted. Our tuition rate already takes into account observed holidays and breaks. Summer session requires a re-enrollment and is not automatic.
When the office is unattended, please use the tuition box for payments, questions, or concerns. You may also email us at firstname.lastname@example.org at any time. It is the responsibility of the customer to pay tuition by the due date, either through the office, parent portal, or through the mail. A $35 processing fee will be assessed to your account in the event that your check is returned. We reserve the right to charge the credit card on file the full tuition (and any late fees) plus the $35 processing fee if your check is returned.
