CAMPS and EVENTS:
There is NO enrollment fee for CAMPS and EVENTS. Payment is due in full at time of enrollment. I authorize Performance Cheerleading Academy to charge my credit card for the fee listed. Fees will not automatically be charged at time of your registration submission. Performance Cheerleading Academy will process the payment once your registration has been received.
There is a one-time $30 individual enrollment fee, with a $40 family maximum for CLASSES. You understand and authorize that your credit card will be charged on the monthly anniversary of your registration/start date for tuition. Our curriculum is year round and continuous. Your enrollment will be ongoing unless you notify PCA in writing that you would like to cancel your child's account at least 14 days prior to your next payment day. However, if you become inactive (not enrolled in a class) for a period greater than 3 months you will be assessed a re-registration fee of $15.00 per child to re-register for classes. Open gym privileges are suspended during periods of inactivity. Fees will not automatically be charged at the time of your registration submission. PCA will process the payment once your registration has been received.
Class attendance policy: Tuition is based off of 4 classes per pay cycle. There are no make-up classes if students miss their class time. Students may come to an Open Gym time (days/times on class schedule) if they miss their class. An open gym visit must take place within 2 weeks of the missed class. There are no open gym times during the summer schedule. There maybe months where 5 classes fall in your pay cycle.Performance Cheerleading Academy does not charge extra if this occurs.
Credit Card payments are processed at the time of the private lesson. All Private Lessons will start on time. Our schedule does not allow us to go over the allotted time, so please arrive to PCA a few minutes early to allow for changing and preparation for class. IF YOU ARE LATE FOR THE LESSON, YOU WILL STILL HAVE THE SAME TIME ALLOTMENT AND WILL PAY THE SAME AMOUNT. For example; you have an hour private lesson form 4pm -5pm but you came at 4:15pm...you will be charged for the full hour but have a 45min. lesson. No exceptions, even if you misunderstood time or forgot.
Cancellation: If notified 24 hours in advance, via email, no charge will be incurred and coach may reschedule. YOU MUST receive an email confirmation or a call (from coach or staff) that we received your cancellation request. IF student does not show for lesson or you notified PCA in less than the 24 hour period, YOU WILL BE CHARGED HALF OF THE PRIVATE COST, no exceptions.
There is a one-time $75 individual enrollment fee, per family there is a $100 enrollment fee maximum 3 kids. If you are already enrolled with PCA, you are not responsible for the enrollment fee again.
Note: Your card information is not stored at PCA, your encrypted card information is stored in the secure Visa, MC, Discover or Amex vault. Performance Cheerleading Academy will only draft based on the drafting you have agreed to.
As Consideration for being allowed to enter the gym area and/or Participate in any party and/or program at Performance Cheerleading Academy (PCA) the undersigned, on his or her behalf, and on the behalf of the Participant(s) identified below, acknowledges, appreciates, understands, and agrees to the following:
1: I represent that I am the parent or legal guardian of the Participant(s) named above or I have obtained permission from the parent/legal guardian of the Participant(s) named below to execute this agreement on their behalf. 2: I acknowledge and understand that there are known and unknown risks associated with participation in Performance Cheerleading Academy activities and the use of the gym area, inflatables, trampolines, pits and any and all other Performance Cheerleading Academy equipment, including but not limited to Parties and Open Gym, which include but are not limited to: contusions, fractures, scrapes, cuts, bumps, paralysis, or death. 3: I, for myself, family in attendance and the Participant(s) named, willingly assume the risks associated with participation and accept that there are also risks that may arise due to OTHER PARTICIPANTS which I also willingly assume. 4: I agree that the Participant(s) named, and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any activity at PCA. 5: I, for myself, the Participant(s) named, our heirs, assigns, representatives, and next of kin agree to hold harmless, release, waive and indemnify the independent owner of this PCA facility, Performance Cheerleading Academy, their predecessors, parent, subsidiaries and affiliates, officers, and employees from any and all injuries, liabilities or damages from participation, except for those arising from the gross negligence or willful misconduct of Performance Cheerleading Academy. 6: I additionally agree to indemnify the independent owner of this PCA facility, their predecessors, parent, subsidiaries and affiliates, officers, and employees for any defense cost or expense arising from any and all claims, injuries, liabilities or damages arising from participation, except for those arising from the gross negligence or willful misconduct of Performance Cheerleading Academy. 7: I am of physical ability to participate and am legally competent to understand and complete this agreement. I hereby execute this agreement without coercion. 8: I understand that entry, by myself and the participant(s) named, constitutes consent for Performance Cheerleading Academy to use any film, video, or likeness of participants for any purpose whatsoever, without payment to the participant. 9: The invalidity or unenforceability of any provision of this Agreement shall not affect the validity or enforceability of any other provision of this Agreement, which shall remain in full force and effect. 10: Any controversy, dispute, or claim arising out of or related to this Agreement shall be within the State of Georgia.
