I acknowledge that Petite Pas Ballet School, LLC staff and any person affiliated with the company will behave prudently in the instruction of dance choreography and technique skill drilling in an effort to avoid accidents and/or injuries from occurring. I realize that participation in dance endeavors and other activities that my child will be participating in entails the risk of injury to the participants. I accept this risk regardless of the nature of the injury and/or the endeavor in which my child participates. I also waive and absolve Petite Pas Ballet School, LLC and its teachers, staff, volunteers, owners, officers, directors, independent contractors, and landlords or any one or more of them or their executors, administrators, heirs, next of kin, successors or assignee of and from all liability and responsibility for injuries, including death, sickness, accidents and/or natural occurrences during participation in performances and activities with Petite Pas Ballet School, LLC. I understand that each participant is responsible for his/her own personal health, medical, dental, chiropractic, and accident insurance coverage.
I acknowledge that Petite Pas Ballet has specific refund policies on the various programs offered. I agree to those policies as it relates to the program I have selected for my child/ren.
Monthly Tuition:
The monthly tuition structure charges the card on file a monthly rate on the 1st of each month between September - June. If you choose to withdraw from Petite Pas Ballet, you must notify us by emailing info@petitepasballet.com PRIOR TO THE END OF THE MONTH that you wish to discontinue dancing at the end of the current month. Your card will not be charged on the 1st. If you don't notify us before the 1st, your card will be charged automatically and you will NOT receive a refund or credit for that charge. We take holidays and breaks into consideration when determining our monthly tuition rate. You will NOT receive a credit or a make-up class for classes missed due to the studio being closed. If the studio is closed due to inclement weather or illness, we will honor a make-up class.
Full Year Tuition (with bi-annual payments):
You will receive a discounted rate if you choose to commit to the entire Academic Year (September - June) but choose to make a bi-annual payment. This amounts to a $50 annual discount. The first payment is due on September 1st and the second installment will be charged to the card on file on 2/1. If you choose to prepay your tuition for the year and you drop before the year's end we will offer a refund for the unused months however we will back charge the discounted amount for the months used and a $25 processing fee for early cancellation.

Summer Program Refund Policy:
CAMP: We reserve the right to modify themes if needed. We also reserve the right to cancel any scheduled weeks due to enrollment demands. Should we cancel any of the camps, families will receive a 100% refund if tuition has already been paid. Family Cancellation Policy: Payment in full is required for Petite Pas Ballet's Summer Camp. If you cancel your camp enrollment more than 60 days prior to the first day of camp, you will be awarded a 75% refund. If you cancel at least 30 days prior to the first day of camp, you will be awarded a 50% refund. If you cancel at least 15 days prior to the first day of camp, you will be awarded a 25% refund. Cancellation less than 15 days prior will not receive any refund or credit. A great deal of scheduling, pre-planning and ordering of supplies goes into planning our summer programs (based on enrollment commitments) the non-refundable amount will be retained to cover any administrative and supply costs already committed to the program and the loss of income from a potential other camper. This cancellation policy is non-negotiable.
6-WEEK SUMMER PROGRAM: We reserve the right to modify themes and combine or cancel classes if needed. These decisions are solely made based on enrollment numbers. If we cancel a class, we will refund you 100% of your tuition. Otherwise, the flat-fee summer program tuition rate is non-refundable. If you are unable to attend one of the classes, make-ups are always offered and welcomed. Due to the short nature (and commitment) of our summer ballet program, we will not be offering prorated tuition for a late start.
