Acknowledgement
I, the parent/guardian of the participant, a minor, certify by agreeing to these terms that the above information is correct and grant permission for the participant to participate in programs offered by Petite Pas Ballet School.
I also agree that I and the participant and participant's legal guardian(s) will abide by the rules and regulations and code of conduct policies set forth by Petite Pas Ballet School as outlined in the Student Handbook.
Liability Waiver
I acknowledge that Petite Pas Ballet School, LLC staff and any person affiliated with the company will behave prudently in the instruction of dance choreography and technique skill drilling in an effort to avoid accidents and/or injuries from occurring. I realize that participation in dance endeavors and other activities that my child will be participating in entails the risk of injury to the participants. I accept this risk regardless of the nature of the injury and/or the endeavor in which my child participates. I also waive and absolve Petite Pas Ballet School, LLC and its teachers, staff, volunteers, owners, officers, directors, independent contractors, and landlords or any one or more of them or their executors, administrators, heirs, next of kin, successors or assignee of and from all liability and responsibility for injuries, including death, sickness, accidents and/or natural occurrences during participation in performances and activities with Petite Pas Ballet School, LLC. I understand that each participant is responsible for his/her own personal health, medical, dental, chiropractic, and accident insurance coverage.
Photo Release
I authorize and agree that Petite Pas Ballet School, LLC may take and use photographs, videos or likenesses of myself or my child as needed for its record keeping, advertising, social media and/or public relations projects and that I have no rights to the same and will not be compensated for the same.
If your family has a specific desire to have their child's photos kept from publication (as listed above), please contact Petite Pas Ballet at info@petitepasballet.com so that we may ensure that your privacy requests are respected.
We are unable to control the personal photography and video taken by other families at our studio and/or events and how they publish those photographs on social media sites or otherwise.
Tuition Payment and Refund Policy
I acknowledge that Petite Pas Ballet has specific refund policies on the various programs offered. I agree to those policies as it relates to the program I have selected for my child/ren.
Monthly Tuition:
The monthly tuition structure charges the card on file a monthly rate on the 1st of each month between September - June for Petite Pas Ballet (PPBS) and between September - May for Bloom Creative Fine Arts (BCFA).
If you choose to withdraw from Petite Pas Ballet School you must notify us by emailing info@petitepasballet.com PRIOR TO THE END OF THE MONTH that you wish to discontinue dancing at the end of the current month. Your card will not be charged on the 1st for future programming. If you don't notify us before the 1st, your card will be charged automatically and you will NOT receive a refund or credit for that charge. We do not refund registration fees. We take holidays and breaks into consideration when determining our monthly tuition rate. You will NOT receive a credit or a make-up class for classes missed due to the studio being closed. If the studio is closed due to inclement weather or illness, we will honor a make-up class.
For Bloom we require a THIRTY (30) day notice by email. Tuition charges will end after the 30 days. Materials and registration fees are non refundable.
PPBS Full Year Tuition (with bi-annual payments):
You will receive a discounted rate if you choose to make a bi-annual payment. The first payment is due on September 1st and the second installment will be charged to the card on file on February 1st. Early withdrawals are responsible for 25% of remaining tuition (at the full rate), paid from date of departure plus a charge-back of the discount received for months used and a $75 administrative and processing fee.
PPBS Full Year Tuition (one full payment):
You will receive a discounted rate if you choose to commit to the entire Academic Year (September - June) but choose to make a bi-annual payment. The full payment is due on September 1st. Early withdrawals are responsible for 25% of remaining tuition paid (at the full rate), paid from date of departure plus a charge-back of the discount received for months used and plus a $75 administrative and processing fee.
BLOOM Bi-Annual Payment Plan
You will receive a discounted rate if you choose to pay for the Academic Year in our bi-annual payment plan. Enrollment is an annual commitment and therefore early withdrawals are responsible for a charge-back of the discount received for months used and plus a $75 administrative and processing fee.
BLOOM Annual Payment Plan
You will receive a discounted rate if you choose to pay for the Academic Year in our full year payment plan. Enrollment is an annual commitment and therefore early withdrawals are responsible for a charge-back of the discount received for months used and plus a $75 administrative and processing fee.
Summer Program Refund Policy:
CAMP: We reserve the right to modify themes if needed. We also reserve the right to cancel any scheduled weeks due to enrollment demands. Should we cancel any of the camps, families will receive a 100% refund if tuition has already been paid.
Payment in full is required for Petite Pas Ballet's Summer Camp. If you cancel your camp enrollment more than 60 days prior to the first day of camp, you will be awarded a 65% refund in the form of a CREDIT. If you cancel at least 30 days prior to the first day of camp, you will be awarded a 50% refund in the form of a CREDIT. If you cancel at least 15 days prior to the first day of camp, you will be awarded a 10% refund in the form of a CREDIT. Cancellation less than 15 days prior will not receive any refund or credit. A great deal of scheduling, pre-planning and ordering of supplies goes into planning our summer programs (based on enrollment commitments) the non-refundable amount will be retained to cover any administrative and supply costs already committed to the program and the loss of income from a potential other camper. This cancellation policy is non-negotiable.
If cancellations are prompted by Petite Pas Ballet or Bloom Creative Fine Arts, a full refund will be provided.
COVID Waiver
I understand that an inherent risk of exposure to COVID-19 exists in any public place where people are present. I understand that Petite Pas Ballet is operating under recommended safety precautions for sessions in accordance with local and state recommendations. I waive Petite Pas Ballet, it's staff, and affiliates from any & all financial, moral, & legal liability should any member of my party become diagnosed with COVID-19, SARS, the flu, or any other illness after participating in sessions. I understand that my child is not permitted to attend sessions if they are currently diagnosed with COVID-19, have recently been exposed to someone infected with COVID-19, have a fever above 100.4, or are experiencing any other symptoms identified by the CDC. I understand that I have the right to not have my child attend Petite Pas Ballet classes, programs or events.
