Online & Portal Registration
Adding a class to your cart, also enrolls the student/client in the class and adds his/her name to the roster. Therefore, any classes that you or your child is enrolled in, must be paid for. Please take the time to go to your cart and pay the balance due. If you are unsure of your registration, please remove the class from your cart. Any unpaid class registration (classes in your cart), will be processed within 48 hours by Stars Gymnastics staff, to insure that all students/clients on a roster have paid their tuition.
An annual membership fee of $36 is due at the time of registration and will be processed again one year later and every year following. Our Online registration system does not post the annual membership fee. However, students who enroll in ongoing classes (not a trial) will have the fee posted to their account manually and processed with the card you have on file within 3 days of your online registration. You will not receive a notification or reminder prior to processing. Please keep in mind that this will happen. Siblings receive a $5 discount.
Tuition is non-refundable once the session has begun, even if the student does not attend the entire session. Once the student takes a spot on the class roster and the session begins, tuition is due in full and will not be refunded for any reason.
Assumption of Risk
ACKNOWLEDGEMENT OF RISK:
I am the parent and/or legal guardian of the participant named on this document. I recognize that severe injuries, including permanent paralysis or death can occur in sports or activities involving height or motion; those activities include but are not limited to gymnastics, tumbling, trampoline, movement education, dance, and stunting. I also realize that my child(ren) will be performing and training on all gymnastics events plus various training devices, including trampoline. I certify that I have consulted a physician, to the extent that I deem appropriate, concerning my child(ren)s participation in these activities. I represent to Stars Gymnastics LLC (SGLLC), that my child is medically fit to participate. I am also aware that participation in day camps, open gym and competition involves transportation to and from such events and that such transportation could result in injury or death in a vehicular accident. I furthermore recognize that due to increased movement, height, flipping, twisting and inversion, the competitive pursuit of these sports and activities carries a higher degree of risk of catastrophic injury than do the recreational versions.
CONSENT AND ASSUMPTION OF RISK: Being fully aware of these dangers, I hereby give consent for my child(ren) to participate in any and all ;SGLLC programs and activities for which they are registered and I ACCEPT ALL RISKS associated with this participation.
Medical and Release
WAIVER AND RELEASE:
In consideration for my or my child(ren)s participation, I hereby, for myself and my child(ren) and our respective heirs and successors, PROMISE NOT TO SUE AND FOREVER RELEASE AND DISCHARGE SGLLC, its officers, directors, shareholders, employees, contractors, teachers, coaches and volunteers from all liability resulting from damages or injuries incurred as a result of participation in SGLLC programs, including those resulting from acts of negligence. I understand that SGLLC has relied upon this agreement in determining the extent of insurance coverage to be obtained, and that in the absence of this Release, SGLLC would charge considerably higher fees to participants.
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19: The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. SGLLC has put in place preventative measures to reduce the spread of COVID-19; however, SGLLC cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending SGLLC could increase your risk and your child(ren)'s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending SGLLC and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at SGLLC may result from the actions, omissions, or negligence of myself and others, including, but not limited to, SGLLC employees, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at SGLLC or participation in SGLLC programming ("Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless SGLLC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of SGLLC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any SGLLC program.
CONSENT TO MEDICAL TREATMENT: In the event of an accident or emergency, I herby authorize SGLLC and its representatives, including its employees, contractors, teachers, coaches and volunteers, to render first aid to my child(ren) to the extent they deem appropriate, I further authorize SGLLC and its representatives to transport or arrange for transportation, by ambulance if SGLLC deems it appropriate of my child(ren) to a hospital or any other medical or dental treatment for my child(ren). Additionally, I hereby agree to be personally responsible for payment of all medical and dental expenses, including transportation, which may be incurred by myself on behalf of my child(ren) as a result of any injury sustained while participating at or for SGLLC, including future medical and dental expenses related to such injury.
PHOTO AND MEDIA RELEASE: I am aware that individual and group publicity photos and videos are taken from time to time and in consideration for my or my child(ren)s participation, I grant permission for my child(ren)s likeness to be used in SGLLC publicity, media usage including but not limited to social media, training tools and videos and/or advertising.
New Covid-19 Policies
The actions of every person who enters the Stars Gymnastics LLC (SGLLC) facility will impact the safety of others. SGLLC is counting on everyone to help keep us all safe by meeting the physical-distancing and personal hygiene standards recommended by the CDC and the State of California.
The following restrictions have been placed on SGLLC by the Los Angeles County Department of Health and will be enforced.
1. Participants must use the restroom and wash their hands thoroughly before leaving home for SGLLC and should always wash both hands and feet after returning home. Hand sanitizer will be provided throughout the facility and waiting areas.
2. Only one guest/parent may enter the facility with the enrolled participant. Participants may also be dropped off and picked up.
3. Arrive 5-10 minutes early prior to your scheduled class/training time. Everyone must line up outside of the facility upon arrival to receive a quick Covid-19 Health screening prior to entry.
4. Those with any symptoms and/or contact with another person who has symptoms, may not enter the facility.
5. All adults and children entering SGLLC must wear a facemask. Everyone must continue wearing their face mask as long as they are inside.
6. The lobby and entry area will have clearly marked physical-distancing position indicators. Blue stickers are for adults who need customer service. Red stickers are for students waiting for class. If this space starts to fill up kindly move to the bleacher area or wait outside and we will walk your child out to meet you when class is over.
7. Please do not send your sick child or yourself into the SGLLC facility if you are not feeling well. We are temporarily offering unlimited make-ups if the absence is due to Covid-19. If not, we offer 1 make-up class per session.
8. If all viewing space is taken, no further seating will be possible and waiting guests/parents will need to wait outside.
Everyone who enters SGLLC is expected to be respectful of the gym's social-distancing policies, and supporting the amazing teachers, coaches, and customer service staff as they work hard to provide a safe place to develop happy, healthy and confident athletes.
Stars Policies & Guidelines
MEMBERSHIP FEE
Students enrolled in Stars Gymnastics classes have a $36 annual membership fee that is due at the first time of registration and on that anniversary date each year following. SESSIONS/TUITION:
Registration is on a first come first serve basis. Class enrollment is open 15 days prior to the start of the upcoming session. Your space is reserved for the session once your payment is made. Sessions are eight weeks in length. We recommend that you enroll your children as soon as possible, to avoid your favorite class filling up. Tuition is non-refundable once the session has begun. Auto Pay participation provides students with a guaranteed spot in the class of their choice. However, a separate authorization form must be completed and signed to join auto pay.
Each class has a max number of students to coach ratio: Classes Age 1-6: 8:1 Classes Age 7+: 10:1

ASSIGNED CLASS INSTRUCTORS
Each class has an assigned instructor for the full eight-week session. Requests for a class taught by a specific instructor may be submitted at the front office up to two weeks prior to an upcoming session. If available, students may be transferred into another class during the current or for a future session. Substitute instructors will be assigned as needed for a sick instructor.
ADVANCING/CHANGING CLASS LEVELS
Your child will be automatically enrolled in the same class every month unless you notify the office otherwise. If your child advances in levels, it is your responsibility to notify the office as soon as possible to initiate the change in class enrollment and automatic registration for future months.
WHAT TO WEAR
Boys must wear tucked in T-shirts and elastic waist shorts/pants. Girls are required to wear a leotard or unitard. NO chewing gum or dangling jewelry. Stud earrings may be worn. Dangling or hoop earrings may NOT be worn. Bracelets and rings may NOT be worn. Hair should be pulled neatly and securely away from the face so that is stays up for the entire workout, INCLUDING BANGS. Girls should not wear bows or other large hair ornaments that may cause discomfort during activity. All students will be barefoot during class. Personal items should be left in cubby holes or with parents. LEAVE JEWELRY ARTICLES AT HOME. Stars Gymnastics facility's staff will not be responsible for ANY items that may be lost or stolen. Be sure your student's personal items are marked with their name.
DROP OFF/PICKUP
Remember that our students may include young children. Drive slowly and carefully through the parking lot. Walk your child to the line. Do not take a chance on your student running to and from your car. If late pick up happens, there will be a 5 min grace period for children ages 6-7 and a 10 min grace period for children 8yrs or older. After the grace period, Stars Gymnastics will charge $1.00 per minute late pick up fee, which must be paid before future enrollment. There is no grace period for children age 5 and younger.

SPECIAL/CLOSED EVENTS
Events such a camp, TGIF, Parents Night Out, Movie night or anything else that is going on outside of normal business and class hours will require a parent to line up with the child outside of the facility to sign the participant in and/or out. These events will usually require an additional registration fee. Non-Members may participate in many of our special/closed events but are required to have student information forms and waiver completed before enrolling or attending. Children without required documents submitted will not be allowed to participate and may not be able to remain in the facility.
PUNCTUALITY POLICY:
Every class begins with a warmup period. This warmup time is important and necessary for every student. It provides physical and mental preparation for class and when performed completely, can help prevent injuries. Therefore, if your child arrives late and misses any portion of warm up period, he/she WILL NOT BE ALLOWED TO JOIN THE CLASS. You may call for a make-up class if you are running late. MAKEUP POLICY
Each student is allowed ONE make up class per eight week session for classes missed for their own personal reasons. Instructors take attendance at the beginning of every class. Make-Up classes cannot be scheduled prior to the missed class. Students must first be marked absent. Students must enroll in a make-up class, prior to attending. Make up classes must be scheduled within two weeks and taken within three weeks of the missed class or will be forfeited. All make up classes will be arranged by the office (not by instructors). Make up credits may NOT be used to discount tuition. A missed scheduled make up class may not be made up.

Financial Policies
TUITION
Your space is reserved for the session, once your payment is made. Tuition is non-refundable once the session has begun.
CREDIT FOR MISCELLANEOUS REASONS:
There are very few reasons in which a credit may be given to a family account. Long term illness or injury with documentation from a doctor is one of those reasons. Another reason may be an error on our part, by over charging fees or failing to discount appropriately based on our policies. Since there are not refunds, a credit will be given to the family account and must be used within 90 days of receiving the credit. After 90 days, the credit amount is forfeited.
Note: (Auto Pay) You are responsible for payment for your student's classes WHETHER OR NOT YOUR STUDENT ATTENDS CLASS until the time you notify the staff VIA WRITTEN NOTICE. Do not rely on your student to verbally let us know that he/she will no longer be attending classes. If a student stops coming to class without notification, then that student's account will be charged for the additional 30 days. This charge will be for holding the student's place in that class instead of offering that place to another student.
DECLINED PAYMENTS AND RETURNED CHECK FEES:
A $25 fee will apply to all returned checks & declined or invalid payments after one attempt. This fee will be automatically added to your account & will be due immediately to hold a spot in the selected class. Invalid payments will result in the student losing his/her reserves space in class until payment is made in full.
Payment Options
AUTO RE-Enrollment (Auto Pay)
Auto Re-Enrollment (Auto Pay) will keep your child enrolled and your tuition paid. No invoicing or payment reminder will be needed. Once enrolled, your child will be automatically enrolled in the same class each eight-week session until you notify the office otherwise. Participants of Auto pay also receive a 5% discount off total tuition due for each 8-week sessions. Auto-Pay transactions are processed on the 15th of each month with 1/2 of each session tuition (after discount), processed each month. Auto Pay cancellation or suspension must be submitted in writing a minimum of 10 days prior to upcoming processing date. Tuition is non-refundable once the session has begun.
• Enrollment into Auto Pay is done by completing a separate Auto Pay Authorization form. You may not join auto pay by online registration, verbal communication, a text, or an email. Once you have completed the form, your payments and guaranteed enrollment will begin on the upcoming date of the 15th with enrollment in the following month. Until that time, enrollment is first come-first served, and you must take care of your registration by using our online enrollment system or getting assistance from our front office staff.
• PARENTS MUST NOTIFY STARS GYMNASTICS TO DROP A STUDENT FROM CLASS. Only a written notice via email, regular postal mail or hand delivered to our front desk will be acceptable. Automatic registration and enrollment will continue until written notice is received.

• 15 day written notice is required to be removed from autopay. No exceptions. Phone calls, voice mail or verbal notification at the front desk are not acceptable forms of cancelation. Suspension/cancelation forms are available at the front office. Emails are also acceptable. Refunds will not be given if written notification is not received by the 1st of the month, prior to processing on the 15th.
