Welcome & Participation
Welcome! We are so excited that you have decided to become a part of the Ultimate Dance Center. We love dance and our goal is to make everyone love dance! We have designed a state of the art dance studio to give dancers the best quality experience. We offer the most extensive dance classes in all of Polk County. Our 2020 classes for the Fall Session begin on August 10th, 2020 and RECITAL will either be on June 5th or June 12th, 2021. As soon as we have confirmation from the venue we will let you know the exact date.
Please take a moment to read the following carefully!
Always enter the studio with your dancer if they are under 12 years old to ensure their safety & that they know which class & studio they will be taking class in.
UDC is located between Swan Brewing & Central FL Speech & Hearing in the Winn Dixie Plaza off of Lakeland Highlands Road and Edgewood Drive at 2980 Lakeland Highlands Rd, Lakeland, 33803. If you need directions feel free to call. There is adequate parking.
Registration
Registration
-All registration fees are to be paid prior to the start of class attendance.
The $25 registration fee is not refundable & must be paid in full before a student is placed on the class roster and allowed to participate.
-Upon payment of registration fees, all students who never have been a UDC student in the past, will receive a free T-shirt.
Students should visit the studio prior to the start of the session to ensure correct sizing & see our space & learn about our dress code policy* & studio etiquette.

EMAIL & TEXTING POLICY
-When you sign up & provide your email address you will have the ability to access all of your account information including transactions, making payments, updating contact information, & enrolling or adding future classes. Please take advantage of our parent portal to enjoy these features.
-Your email & texting feature is our primary point of contact for all major announcements, holiday & weather closure notices, schedule changes, special event details, due dates for major payments (pictures, costumes, recital fees), promotions, monthly newsletters & ALL RECITAL DETAILS. It is your responsibility to stay current with all thing Ultimate Dance Center by regularly checking these emails. We will send you all information needed & this eliminates calling or stopping at the Front Desk for this important information.
Tuition Rates & Recital Fees
Tuition Rates
Tuition is due the 1st of every month. As classes for the 2020-2021 season start on Monday Aug 10th, 2020, the very 1st months tuition will be withdrawn on August 10th, 2020. As you are aware some months have 4 weeks and some have 5 and so it all balances out. UDC does not give refund on tuition. It is your responsibility to inform us if you are stopping enrollment from the center.
A late fee of $10 will be added to all accounts past due weekly, payments are expected to be on time regardless of attendance in class. Students will not be allowed to participate in class if tuition is more than 30 days late and payment arrangements are not made
$2.00 will be added to your account along with tuition for camera access each month.
UDC Tuition Rates
• 1 Kids Combo / Kids Acro Class per week - $45 per month/45 Minutes (2-3, 2-5yr Ages)
• 1 Class per week - $55 per month 1 hr (4-6, 7-11 & 12+yr Ages)

-Discounts: If you have a discount of a percentage you will not get the $10 off. If you have more than one child you do get the 10 off per child for one class not all classes or 10 off each class the system will determine which discount would be greater for you for tuition. As a paying customer we can not have double dipping of discounts for tuition.
Recital
-All students are expected to participate in recital. Recital is the end goal of the season usually taking place the 1st or 2nd weekend of June, at the RP Funding Center. This is where all UDC students shine & showcase what they have learned in class with a recital piece on stage, cultivating self-confidence with pride! All recital fee deadlines must be paid in full or else the student cannot participate. The cut off for newly registered students to be able to participate in recital is December prior to costume ordering but they are still welcome to join the class & will learn the recital routine as well as class curriculum.
-Costume Fees: A $60 costume fee is issued for child sizes XS-LC, XLC - Adult Sizes owe a $70 Costume Fee. Dancers will be able to keep their costumes after recital, UDC will also handle all sizing measurements & ordering - you will be notified when your dancer can pick up their costume by the class Instructor. Costume fees are due by the end of January or your dancer cannot participate in recital.
-Recital Fees: TBA for recital fee. This recital fee facilitates rental of the performance space & instructor pay for choreography. Recital fees are due by the end of March or your dancer cannot participate in recital.
-There is no recital showcase during the summer months of June & July - if attending Summer Session please disregard costume & recital fees above. We have an open audition for our annual Holiday Production which changes every season - the performance takes place the 1st weekend of December & Auditions are in August, Rehearsals take place on Saturdays from September - December..
Ultimate Dance Center does not issue refunds on tuition, costume or recital fees.
Procedures & Behaviors
Behavior & Procedures
-While in the studio, students are expected to exhibit proper behavior and demonstrate respectful attitude toward fellow students and UDC Faculty.
Instructors reserve the authority to remove a student form class or exclude them from participation should problematic behavior occur! We expect our students to listen & be respectful of the space, other dancers & themselves.
-Though lost or stolen articles are not the responsibility of UDC,
we do keep a Lost & Found Box. Check with reception if something is lost.
-Dress Code Policy: Upon registration you will receive a dress code packet outlining all of our class styles and what dancers need for proper attire. You can also view the dress code online at UDCLakeland.com or ask for a copy at Front Desk. Please review the requirements as all styles vary - from shoes to leotards & etc. In order to learn proper technique & skills we expect dancers to be dressed accordingly.
-CRAZY WEEK - We like to have fun while we learn at Ultimate Dance Center - it's ultimately about the joy of dance - during the 3rd full week of every month we have "Crazy Week" where dancers are encouraged to get wacky with their dancewear - we love seeing costumes, crazy colors, & etc - students may ditch the dress code (no logo no leo & etc) during this week only, all we require is that they have the correct dance shoes for their class (Ballet, Tap, Lyrical, Jazz, Sneakers for Hip Hop) to ensure safety.
-Please provide us with up to date health information such as daily medications, food allergens etc. Absolutely NO medication may be administered by the UDC faculty or staff.
-Students are expected to attend each class in which they are registered. The faculty of UDC will evaluate each student's progress throughout the semester and a change of class level will be suggested if necessary. We have a set curriculum for level & age, please trust we will guide your dancer in the right direction - placement is based on Freddie DeJesus' ( Artistic Director) discretion. UDC students may enjoy the following benefits as registered active students:

Grab instructions from the UDC reception desk, log into the app and see your child's/children's class at any time! *The only time that this is not permitted is approximately 2 months prior to the recital date.

The waiting room is available for the convenience of our students and their families.
There you may relax as you wait for your child to finish her/her class and converse with other parents. We ask that you please respect the activities of others and the classes in session. We allow children who are with their parents to play with toys or other games brought from home. We also welcome students & siblings to do their homework or read & enjoy the lobby space . Check out out after school pick up program to enjoy even more UDC benefits - pickup from car lines, tutoring, & more! Ultimate Dance Center picks up from select schools.
-Food & Drink: *Please only water or tightly covered drinks and dry snacks. If you use our books and magazines, please return them. If your child or dancer makes a mess please tidy after them before you leave. Your courtesy is greatly appreciated!

Drop off and Pickup
-*Always call whenever delayed for class start time or pickup.
-Be sure to arrive at the studio with adequate time to prepare for class, parking, changing, restroom, etc. This means at least 10 min prior to class start time.
-Parents, for your children's safety, do not drop your child off more than 15 min before class begins and please be available in the waiting room to receive students when dismissed from class.
-Curbside drop offs or pickups are prohibited for students under 12yrs. A Parent or guardian must come inside to drop off or pick up a student.
Missing Class & Attendance
If for any reason your child can not make it to class they will get a recording of class to your parent portal. This is a new camera system being installed and that will be make it for not attending class. If class is canceled for any reason you will get an email of a make up date asap.
It is the family & students obligation to inform UDC if any breaks or vacations will interfere with attendance in class. We cannot keep track of absences and will not prorate or refund tuition. In order to have a top notch recital, consistency with practice is key for performance. No more than 2 absences are permitted after the recital fee is due or dancer may lose the right to perform in recital. If at any time you elect to drop a class you need to inform UDC in writing @ info@udclakeland.com. If you withdraw from dance and then decided to come back you will need to enroll again through the parent portal or you Come by the studio. The registration fee is charged for ALL students for each new season . If you enroll in August and then you drop all classes in October but decided to take classes again in December then if you re-enroll then you must pay the registration fee again to secure your student a spot in class. I understand that Ultimate Dance Center, LLC utilizes a mobile online viewing application designed to allow friends or family of the dancers to view the classes on their mobile devices to watch the dancer's progress and participation in class, ensure their safety, and become better informed about the dancer's activities. I understand that in connection with Ultimate Dance Center, LLC's use of this mobile online viewing application that my/the dancer's photographic portraits, video, or photographic likenesses, or images may be taken by the Ultimate Dance Center, LLC, and via the execution of the below Release, consent to the use of my/the dancer's photographic portraits, video, or photographic likenesses or images in connection with the mobile online viewing application and/or in studio promotional materials.
Student Health
As you know the COVID-19 pandemic is still with us. We at UDC think that it will be with us for sometime and so we have changed our policies and protocols to reflect the changing landscape in the country and the world. No dance center, nor any other business entity can guarantee that a student, parent, attendee or customer will NOT acquire COVID-19 nor any other viral illness by coming into a particular business.. The only thing that any business can do is to try and mitigate the possibility of acquiring an infection by having hand sanitizers throughout the facility, checking temps and asking people to wear masks. However, even with ALL of the above policies and procedures, it is entirely possible that a person with COVID-19 who is asymptomatic unknowingly enters a place of business. After reviewing the current information re COVID-19, the following procedures will be in place until further notice.
1. All persons entering and staying at UDC must wear a mask.
2. All dancers must wear a mask even while taking classes. This decision was made after careful consideration of everyone's safety and after reviewing the current(July/Aug 2020) CDC guidelines regarding the definition of close contact. Close contact is defined as being less than 6 feet apart and not wearing a mask for 15 minutes or longer. If everyone wears a mask, then technically there will be no "close contact" We will have masks for the 1st week ( Aug 10th, 2020) for everyone if they do not have masks. If you have your own mask then per the new guidelines, they masks CANNOT have a valve.
3. UDC strongly advises all parents of dancers less than 6 yrs old to wait in their cars once they drop their student off for class.
4. All staff, guests, students entering UDC will have temperature taken.
UDC will follow the current CDC guidelines as listed: a) Any student with confirmed or suspected COVID-19 illness must self-isolate. The isolation can end if it has been 10 days after symptom onset and there is resolution of fever for at least 24 hours, without the use of fever-reducing medications and with improvement of symptoms. A doctor's note is required stating that that the above criteria have been met. b) For any dancer with a positive COVID-19 test but never developed symptoms, isolation and other precautions can be discontinued 10 days after the date of their first positive RT-PCR test for SARS-CoV-2 RNA. A doctor's note is required stating that that the above criteria have been met. c) It is important to remember that anyone who has close{30 min of exposure less than 6 feet away} contact with someone with COVID-19 should stay home for 14 days after exposure based on the time it takes to develop illness.

Release of Liability
I, the dancer, or the parent or legal guardian of the dancer if the dancer is under the age of eighteen, in consideration of the services rendered to the dancer and other good and valuable consideration, the receipt of which is acknowledged, grant to Ultimate Dance Center, LLC, a Florida limited liability company, with its principal place of business at 2980 Lakeland Highlands Road, Lakeland, FL, 33803, its successors and assigns, the right to use and publish for advertising purposes and/or for use in its mobile online viewing application, video and photographic portraits, video, or photographic likenesses or images of me/the dancer, and disclaim any rights I/the dancer may have to my/their portraits, video, photographic likeness, or images, or any profits or income generated by the same.
I/the dancer may be included in the portraits, videos, photographic likenesses, or images in whole or in part, in composite or distorted form, in conjunction with my own or a fictitious name, or reproductions of the same, or in color or otherwise, made through any medium.
The portraits, videos, photographic likenesses, or images are to be taken at the date time and place of my classes at Ultimate Dance Center, LLC, time within the studio, or at an Ultimate Dance Center, LLC affiliated event.
I/the dancer waive(s) any right that I/the dancer may have to inspect or approve the finished product or the advertising or other copy that may be used in connection with the product or the use to which it may be applied.
I/the dancer release and discharge such photographer or videographer, grantee and its owners, agents, and employees, their successors and assigns and all persons acting under their permission or authority, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of the pictures, or in any processing tending toward the completion of the finished product, unless it can be shown that they and the publication of them were maliciously created.

Liability
RELEASE AND WAIVER OF LIABILITY AND INDEMNITY
I, the dancer, or the parent or legal guardian of the dancer if the dancer is under the age of eighteen, understand that dance, in all its forms, has the potential to result in some degree of risk of bodily or personal injury. I understand dance involves several potentially dangerous/risky activities including, but not limited to: jumping, spinning, stretching, partner weight-bearing activities, tumbling, pointe work, aerobic exercise, and other vigorous or strenuous physical activity which may result in injury to the bones, tendons, ligaments, and soft tissues of the body. I also understand that airborne illnesses such as viruses(Influenza, SARS-Cov- 2, Rhinoviruses, etc) and bacterial infections can be transmitted to and from any person in the dance studio. As such if any dancer has a cold then they cannot come back and take class until certain criteria have been met. These criteria are outlined above in the student health section of this Registration Agreement. Please re-read the section on student health for further information.
Due to the potential of injury, I understand that if the dancer is more than 10 minutes late to class they will be asked to sit and observe and not take class. By executing this Release and Waiver of Liability and Indemnity Agreement, I hereby freely and expressly assume and accept any and all of the above described risks of infection, bodily or personal injury to the dancer during all Ultimate Dance Center, LLC affiliated activities. Additionally, I hereby release and hold harmless Ultimate Dance Center, LLC, its owners, agents, and employees from liability for bodily or personal injury and from liability for damage or loss of personal property, resulting from the dancer's participation in Ultimate Dance Center, LLC affiliated activities and waive my right to sue or make a claim against any Ultimate Dance Center, LLC held insurance policy for the same. I further agree to indemnify Ultimate Dance Center, LLC, its owners, agents, and employees for any claim or cause of action which may arise as a result of my/the dancer's participation in all Ultimate Dance Center, LLC affiliated activities.
