What to wear: Athletic type clothing that is free of any buttons, clips, zippers, strings, skirts, jeans and hoods. Please bring a non-spill plastic water bottle. Only water is permitted in the gym.

Required Policies and Agreements
In order to complete your registration we require full payment of the session fee, along with a Gymnastics Ontario $35.00 Recreational/ $14.00 Camp registration fee (valid July 1 through June 30), plus all applicable taxes. This fee will automatically be charged to your credit card when your registration is received. By registering you agree to make all payments in full upon submitting your registration.
In the event that your credit card on file can not be completed for any reason you will be charged to $15.00 plus HST admin fee.
You will then have 24 hours to complete your payment in full.
Failure to do so will result in the discontinuation of your registration and you will be removed from the class.
All fees are subject to HST.

Whitby Gymnastics and Circus Centre and it's owners, officers, employees, and /or agents have the right to seek emergency medical treatment for the participant(s) in any event deemed necessary. Parents/guardians will always be contacted when emergency medical treatment has been contacted. The parent and/or guardian also agrees that Whitby Gymnastics and Circus Centre is not responsible for any financial debt incurred by above mentioned action.

As an active member of Whitby Gymnastics and Circus Centre, I agree to receive the eNewsletter containing information regarding programs and schedules, registration information and news bulletins. In compliance with CASL you may unsubscribe at any time from our eNewsletter.

It is understood that the participation in these physical activities involve a unique environment that carries a certain assumption of risk. I am aware that the sport of gymnastics involves risks including personal injury, death, property damage, expense and related loss, including loss of income. Included in theses risks could be unintentional negligence on the part of Whitby Gymnastics and Circus Centre, its officers, agents, employees, officials, volunteers, other participants and owners of the facilities where the activities occur. I freely accept and fully assume all such risks and the possibility of personal injury, death, property damage, expense and related loss including loss of income.

In consideration of Whitby Gymnastics and Circus Centre accepting my application to participate in our activities, I agree
1. To waive any and all claims that I may have in future against Whitby Gymnastics and Circus Centre and OTHERS
2. To release Whitby Gymnastics and Circus Centre and OTHERS from any and all liability for a personal injury, death, property damage, expense and related loss, including loss of income that I or my next of kin may suffer as a result of my participation in this activity, due to any cause whatsoever, including unintentional negligence, breach of contract or breach of any statutory duty of care.
3. To hold harmless and indemnify of Whitby Gymnastics and Circus Centre and OTHERS from any all liability for any damage to property of or personal injury to any third party, resulting from my participation in our activities.
4. That this agreement is binding on not only myself but my next of kin, heirs, executors, administrators and assigns.

I authorize Whitby Gymnastics and Circus Centre to collect and use personal information about me/my child/ward, including name, address, email, telephone number, cell phone number, sex, age, date of birth and any other additional information required by Whitby Gymnastics and Circus Centre. I understand that Whitby Gymnastics and Circus Centre needs to disclose this information to Gymnastics Ontario and Gymnastics Canada for the following purposes:
1. Receive communications/direct mailings from Whitby Gymnastics and Circus Centre, Gymnastics Ontario/Gymnastics Canada about programs, events, activities, and services.
2. Receiving information from Whitby Gymnastics and Circus Centre, Gymnastics Ontario or Gymnastics Canada's official sponsors, partners and suppliers.
3. For regular demographic records.
4. For club, provincial and national competition registrations and protected databases.
5. Limited information used for posting competition information and results.
6. In the case of medical emergencies personal information may need to be supplied to medical professionals only.

I consent to Whitby Gymnastics and Circus Centre/Gymnastics Ontario/Gymnastics Canada to take photographs, videotape or digital recording of me/my child/ward. These can be used in any and all print and social media, including the Whitby Gymnastics and Circus Centre or Gymnastics Ontario/Gymnastics Canada websites.

You may transfer into a new class with written notice to the office at any point during the session if there is space available in your new class selection.
If you wish to discontinue the class entirely, 7 days written notice is required to the office. You will be refunded the amount remaining after the 7 days notice. Your refund will be issued in the same form that your initial payment was accepted during registration.
Camp Registration cancellation or transfer requires a minimum of 24 hours notice for full credit.
Please note, the $35.00 Recreational Gymnastics Ontario Fee or $14.00 Camper Fee is non refundable.
