As the legal parent or guardian, I release and hold harmless The Carroll School of Dance, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of The Carroll School of Dance, its owners and operators or in route to or from any of said premises.
Parents/Guardians have 2 weeks from the student(s) start date to make changes to the number of classes that their child is enrolled in or to withdraw their child/children from The Carroll School of Dance, penalty free.
After the 2 week period, the following fees will be charged:
• $200 withdrawal fee per class for Students to withdraw from a single class if they are taking multiple classes.
• $450 Contract Termination Fee for Students to withdraw from The Carroll School of Dance for the year.
All fees will be charged to the student's account, for the child to be released from the program. We kindly ask that you please notify the office immediately if your child will be adding or dropping a class. If you choose to drop your child from the program, we must have an EMAIL CONFIRMATION, if written notice is not given, tuition will be charged in full. Please note your account must be at a zero balance and drop fee must be paid for The Carroll School of Dance to drop release your child. All accounts will be charged for past due amounts.
If students misses classes for 1 month at The Carroll School of Dance, with no notification, they will automatically be dropped from the program, and the card on file will be charged a $450 penalty on their account. If the account on file is declined, the account will accumulate fees, tuition charges, late fees, and drop charges, until legal action is taken.
