TUITION: Enrollment in the Tri-Cities Academy of Ballet and Music is based on a 36 week school year. Accordingly, monthly tuition is based on 9 equal payments regardless of the number of lessons in a particular month. No tuition deduction or allowance shall be made by reason of the absence, withdrawal, suspension, or dismissal of the student.
THERE IS A TWO MONTH MINIMUM FOR ALL LESSONS. However, NEW DANCE STUDENTS may withdraw up to the fourth lesson for a full refund (including registration fee). This refund does not apply to current students, returning students, or music students. After the fourth lesson, all regular tuition policies apply.
Tuition is not transferable and is NON-REFUNDABLE unless withdrawal is due to prolonged illness or injury verified by a doctor's certificate.
PAYMENT BY AUTOMATIC BANK ACCOUNT DEBIT OR CREDIT CARD CHARGE : Tuition will be debited from your bank account or charged to your credit card upon registration for the first month of lessons and on the 5th day of each month thereafter through May. The first month is non-refundable. Payment must be made by pre-authorized automatic monthly payment. All NSF returns will be charged a $20 NSF fee. Only regular monthly fees may be paid by auto-debit or credit card.
REGISTRATION: A non-refundable fee of $30 per family is required yearly at time of registration.
DISCOUNT: There is a 20% discount on any additional dance classes taken. There are no discounts for music lessons.
FAMILY DISCOUNT: There is a 20% discount on the tuition of a second immediate family member, and a 50% discount on the tuition of each additional family member. There are no family discounts for music lessons.
WITHDRAWALS: One-month notice from the first of the month is required to discontinue any classes. Withdrawal for the months of September through April must occur within the first 7 days of the month. No withdrawals will be accepted after May 1. Withdrawal must be done in person and will not be accepted over the phone. Withdrawal must be done at the school office. To withdraw from lessons a parent or adult student must complete and sign a withdrawal form provided by the school office. All automatic bank debiting or credit card charges will stop after the one-month notice period. Tri-Cities Academy of Ballet and Music reserves the right to terminate lessons to any students without notice. In such a case a refund for unused lessons will be given.
For "Annual Tuition Students," refunds will be based on the regular monthly tuition rate and not the discounted annual tuition rate.
MISSED LESSONS AND MAKE-UP LESSONS:
For DANCE students, missed classes can be made up in another class as long as the student is currently enrolled.
For MUSIC students, no make-up lessons will be given for missed lessons unless absence is due to serious illness and 24 hour notice has been given to the school office. Make-up lessons will be limited to 2 per teaching year. No refunds are given for missed lessons.
SUBSTITUTIONS: Tri-Cities Academy of Ballet and Music reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to attend a lesson. If a teacher is ill and the school cannot arrange a substitute any missed lessons will be made up.
I hereby authorize the Tri-Cities Academy of Ballet and Music (TCAB) to record the student's picture and voice on photographs, films, DVD's, and tapes, to it these recordings at its discretion, and to incorporate these recordings into movie and sound films on tapes, DVD's, radio, television, and broadcast programs. I also give my permission for TCAB to use and license others to use these materials in any manner or media whatsoever. TCAB is permitted to use these materials for publicity, advertising and sales promotion and to use the student's name, likeness and voice and biographic or other information in connection with them. I acknowledge that no promises of compensation were made by TCAB for such use.
I am aware that dance training and the athletic exercises associated with it place unusual stress on the body and carry the risk of physical injury, and I understand that good dance training involves touching and adjustment of the student's body by the instructor. On behalf of my child and myself (and if I am no longer a minor, on my own behalf), I assume the risk and agree that the Tri-Cities Academy of Ballet and Music (TCAB), its principals, teachers, and staff shall not be liable in any way for injuries sustained during attendance at the dance school or any of its related functions.
As the legal parent or guardian, I release and hold harmless Tri-Cities Academy of Ballet and Music (TCAB), its principals, teachers, and staff from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of TCAB, its principals, teachers, and staff or in route to or from any of said premises.
