Release of Liability

Release of Liability, Waiver of Claims, Assumption or Risks and Indemnity Agreement. By signing this document you will waive certain legal rights, including the right to sue. PLEASE READ CAREFULLY. Awareness and Assumption of Risk. I am aware that severe injuries, including permanent paralysis or death can occur in sports or activities involving height or motion, those activities include but are not limited to gymnastics, tumbling, trampoline and cheerleading. In addition, I recognize that participation in day camps involves transportation to and from field trips and such transportation could cause injury or death in a vehicular accident. Being fully aware of these dangers, I hereby give consent for my child(ren) to participate in any and all Cheer Fuzion's and activities and I accept ALL RISKS associated with this participation. Release of liability, waiver of claims and indemnity agreement. In consideration of Cheer Fuzion accepting my application to participate in this activity, I agree to waive any and all claims that I may have in future against Cheer Fuzion. I agree to release of Cheer Fuzion from any and all liability for any personal injury, death, property damage, expense and related loss, including loss of income that I or my next of kin may suffer as a result of my participation in this activity, due to any cause whatsoever, including negligence, breach of contract or breach of any statutory duty of care. I agree to hold harmless and indemnify of Cheer Fuzion and agree that this agreement is binding on not only myself but my next of kin, heirs, executors, administrators and assigns.
I have read this agreement and understand it. I am aware that by signing this document. I am waiving certain rights which I or my next of kin, heirs, executors and administrators may have against Cheer Fuzion.

Payment

I/WE agree to pay Cheer Fuzion All-Stars in installment/s being due the 1st of each month starting June 1, 2019 the last payment being due April 1, 2020. I understand this is a payment plan not a cost per month. If my child leaves the team before the end of the season and she has accrued expenses that have not been covered I am responsible to pay the amount she/he has accrued.

Medical Emergencies

I/We understand that the programs at Cheer Fuzion All-Stars is a year-round commitment and that your son/daughter's spot is reserved for them, therefore, we do not pro-rate nor refund tuition for any reason including injuries.

BID competitions

For teams accepting BID's: Coach's expense including food travel and accommodations must be divided amongst the team competing. All travel teams must travel with a minimum of two coaches per team. This will ensure the safety of all athletes.
Travel competition athletes must purchase Cheer Fuzion travel gear.
