I hereby acknowledge that a digital copy of the Cleveland City Dance Handbook is online, wherein there is set forth the policies and programs of CCD, and I understand enrolling myself or my child as a student of CCD, I am bound by the terms set forth in the CCD Handbook. I agree to release and hold harmless Cleveland City Dance and Laves Dance, LLC, including its teachers, dancers, staff members and facilities used by the company, from any cause of action, claims, or demands now and in the future. I will not hold CCD, Laves Dance LLC or its teachers, dancers and staff liable for any personal injury or any personal property damage which may occur while I am present for class.
Liability Waiver Addendum
In March 2020, the coronavirus/COVID-19 forced our studio to close its doors amid the worldwide pandemic. According to the World Health Organization, this illness is extremely contagious and is believed to be spread mainly from person-to-person contact. Federal, state, and local governments and health agencies recommend social distancing and wellness protocols and have prohibited the congregation of large groups of people.
As Cleveland City Dance/Laves Dance, LLC reopens, we have instituted measures to prevent the spread of COVID-19. However, Cleveland City Dance/Laves Dance, LLC cannot guarantee with 100% certainty that students will not become infected with COVID-19 as a result of receiving in-person instruction. Given the nature of the instruction and despite our social distance markers, close contact between students and teachers cannot be avoided.
By signing this legalese, I acknowledge the contagious nature of COVID-19 and assume the risk that my child (ren) and I may be exposed to or infected by COVID-19 via attending classes at Cleveland City Dance/Laves Dance, LLC and that exposure may result personal injury, illness, permanent disability and death. I accept responsibility for these risks and hereby release, covenant not to sue, discharge and hold harmless Cleveland City Dance/Laves Dance, LLC, its staff, instructors, volunteers, agents, and representatives, of all claims, liabilities, actions, damages, costs or expenses of any kind arising out of or relating thereto.
Student (s) and family members understand and agree that they must follow all lawful instructions, social distancing protocols, or direction from Cleveland City Dance/Laves Dance, LLC and its teachers, instructors, volunteers and staff while receiving instruction and that failure to do so may result in being asked to leave the premises.

Financial Agreement 1
I hereby acknowledge that a digital copy of the Cleveland City Dance Handbook is online, wherein there is set forth the policies and programs of CCD, and I understand enrolling myself or my child as a student of CCD, I am bound by the terms set forth in the CCD Handbook.
I understand that I am obligated to pay all tuition and charges required for the entire session (September - June). I understand that the first month's tuition payment is due at the time of registration, non-refundable and required to hold my space in class and that all further tuition payments are due by the 1st of each month. The amount due each month will be based on the number of classes taken per week.
I understand that no refunds will be given unless a written withdrawal notice is accompanied by a verified doctor's excuse stating extreme illness or injury, or documentation of a move out of state. I understand this written notification must be received 30 days prior to the next fee schedule to avoid accruing an automatic charge.
By agreeing to these terms, I authorize Cleveland City Dance and Laves Dance, LLC to process the account I have an file for any and all tuition charges.
Financial Agreement 2
I hereby acknowledge that a digital copy of the Cleveland City Dance Handbook is online, wherein there is set forth the policies and programs of CCD, and I understand enrolling myself or my child as a student of CCD, I am bound by the terms set forth in the CCD Handbook.
When registering for summer classes and camps, I understand that my obligation is to pay the amount listed in the brochure in full or in two payments to be paid at the time of registration if paying in full or by the fourth week of classes or camps if paying in two installments. If a camp is only offered for two weeks, full payment is due at the beginning of camp. I understand the policies and programs of Cleveland City Dance as set forth in the summer brochure and I accept the responsibility for payment of tuition and charges incurred by my child or myself.
By agreeing to these terms, I authorize Cleveland City Dance and Laves Dance, LLC to process the account I have an file for any and all tuition charges.
Financial Agreement 3
I hereby acknowledge that if I am late with a tuition payment, I will pay the $25 late fee in addition to the tuition. I understand that my child will not be able to take class until tuition has been received. TUITION MUST BE PAID BEFORE YOUR CHILD CAN ENTER CLASS.
Refund Agreement
I hereby acknowledge that tuition paid for classes taken is non-refundable. Students needing to withdraw from classes must submit written notice to the Front Desk 30 days prior to withdrawing. Parents, guardians, and students are responsible for all tuition payments due up to the point in which Cleveland City Dance receives a written notice for withdrawing from classes.
• Written withdrawal must be received one month prior to start of the session to receive a refund of tuition. In this event, $50 of the tuition amount is non-refundable.
• No financial adjustments are given for missed classes. Missed classes cannot be transferred to another session.
• Refunds may be given in cases of physician-documented serious illness or injury, or if the student is moving out of town.
1) All documentation must be received within 30 days of the injury/illness or the refund will not be processed.
2) Refunds will be issued for the remainder of classes paid for in the semester.
Medical Release
On behalf of my child and myself, I understand that classes involving physical activity, such as dance, present a risk of physical injury or illness, and acknowledge and agree that neither CCD, nor its instructors or staff shall bear any responsibility, or have any liability, for any personal injury or illness that may result from participation in classes or any of its related functions.
Media Release/Optout
I understand that from time to time CCD may use photographs or television footage that include my child to be used for promotional purposes. I understand that CCD will contact me prior to taking any photographs that may be used on the CCD website, television, magazines, brochures, billboards or any other form of advertising. I recognize that I have the option to grant or deny permission for CCD to use my child's photo for the aforementioned purposes.
Media Release Addendum
I hereby acknowledge that virtual classes may be recorded for instructional purposes and/or to share a virtual performance with family. I allow Laves Dance, LLC and my child's instructor to share any videos with only members of the class. I reserve the right to hide my video and mute my audio on Zoom if I am not comfortable with this policy.
Class Cancellation Policy
All classes must have a minimum of four students registered by the Thursday prior to the start of a given session. The class will be delayed start or cancelled if the minimum number of students is not met at this time.
Class Changes Due to Extenuating Circumstances
I hereby acknowledge the following statement regarding situations beyond the control of Cleveland City Dace,
In the event, Cleveland City Dance needs to adjust classes to a virtual platform at any point during the year due to unforeseen circumstances beyond our control, tuition will be discounted 20% of total monthly tuition. Families and students will be notified in a timely manner prior to any changes and have 15 days to withdraw from class should a student not wish to continue the virtual platform.
If a student decides to withdraw from classes during this time, tuition will not be charged so long as Cleveland City Dance (Laves Dance, LLC) is notified prior to the 15-day window.
If Cleveland City Dance is notified after the 15-day period has passed and tuition has been processed, it can be applied as a credit for a future class at Cleveland City Dance.
Cleveland City Dance will continue to use best safe practices for the health and safety of its faculty, staff, and families.
Wellness Protocols
I hereby agree to the protocols instituted by Laves Dance, LLC to help protect my child, myself and others from the spread of communicable illnesses. These protocols include allowing my child's temperature to be taken prior to entering the building, answering a series of questions about our health, wearing a mask while inside the Cleveland City Dance studios and lobby, remaining at home for a 14-day period if unwell or after travel outside of the state, adhering to social distancing guidelines, etc. A full list of protocols is available on the website and will be emailed prior to the start of classes.
Please note, as of June 2020, it is mandatory that everyone who enters Cleveland City Dance studios and lobby wear a cloth mask. This policy will be updated as we learn more information.
