RELEASE FROM LIABILITY, WAIVER OF CLAIMS & ASSUMPTION OF RIS
By signing this Release, Waiver & Assumption of Risk, (the 'Release') I agree that Step 2 This Dance & Performing Arts Dance Studio shall not be responsible for any injury suffered by the student. The Release extends to Step 2 This Dance & Performing Arts Dance Studio, and its owners, officers, agents, instructors and employees (all of which are collectively referred to herein as 'studio'). The Release extends to every claim, demand or liability of any kind based on any injury or damage described below.
I understand that dance and tumbling activities as conducted and taught at the studio have inherent risks of injury. These risks include muscle pain and pulls, broken bones, ankle injuries and other personal injury. I recognize that the student is exposing himself/herself to such risks when undertaking dance activities. I understand that these risks cannot be fully eliminated without jeopardizing the essential qualities of the activity. The student and I assume and accept all risks of injury or damages resulting from such dance/tumbling activities. The student's participation in this activity is purely voluntary, and the student elects to participate, and I join in the election, in spite of the risks.
I also agree that the studio, including its owners, officials, agents, instructors and employees, is not responsible for any property loss or damage suffered by the student or any guests or parents of the student that results upon entry into or presence in the studio or its surrounding property. I further understand that I have been advised of the need for the student to be covered by adequate insurance to cover any injury or damage that may be suffered while participating, and I have obtained such insurance or have agreed to bear the costs of any such injury or damage myself.
I understand the terms of this Release and have had the opportunity to consider and discuss it with such individuals and advisors as I deem appropriate. By Agreeing to this Release, I confirm that I have read the Release in full, that I understand its terms, and that I agree with those terms. I further confirm that by signing this Release, I acknowledge that if the student is hurt during participation in this activity, I may be found by a court of law to have waived my right and any right of the student to maintain a lawsuit against the studio on the basis of any claim which is released hereby. In consideration of the student's being permitted by the studio or participate in the activities provided by the studio, I further agree to hold harmless and indemnify the studio from any and all claims which are brought by or on behalf of any student who is a minor and which are in any way connected with the activities performed at the studio by any such minor student.
Tuition
We offer 2 methods of payment:
1. Auto Pay
We will deduct tuition amounts on the first of each month. (See form for detail)
2. Cash Payments-Only
At registration if you only want to pay in cash for the season, you will need to pay 3 months of
lessons at a time.
If for any reason tuition is paid after the 5th of the month, there is a $25.00 processing late fee to cover
the cost of the Administration taking the extra time to collect tuition in a timely matter.
1. There is a $25.00 fee for returned checks and declined auto payments.
2. Monthly tuition payments are required to keep your account in good standing and to reserve your student's spot in class.
3. Step 2 This cannot extend any credits or allow any account to go past 30 days past due. We reserve the right to send past due accounts to collections.
4. There are no refunds for missed classes however; whenever possible a make-up class may be available. After January makeup classes available only for technique due to each class learning a Recital Dance.
5. Tuition is due and payable in full every month whether the student has attended all scheduled classes or not.
6. If your family is taking a vacation or will be absent for more than a few days, we require at least 2 weeks written notice prior to absence in order to hold your dancers spot in class otherwise the students spot may be filled by a student on a waiting list.
7. Tuition will NOT be prorated for holidays and studio closures.
8. Tuition will NOT be prorated for those who register late during the month.
9. Tuition and registration fees are NON-Refundable.
10. Unpaid and late accounts may result in dismissal from dance class and the recital.
11. Unpaid and late accounts may result in the Dance Studio keeping dance costumes and dance pictures until the account is paid to date.
12. If you pay for classes that your child does not attend or drop a class your tuition will not be refunded for any reason.
13. If you wish to stop a class, please note, we must have 2 weeks written notice. Preferably two weeks before the 1st of the month, that way you will not be charged for the next month tuition. Example: if you call on the 1st of the month to drop your class, we will consider that the two weeks notice and you will be responsible for 2 weeks of the next month's tuition.
General Rules
1. Please do not drop your child off more than 10 minutes before class
2. Please pick up your child no later than 10 minutes after class
3. The studio cannot be responsible for supervision before or after class
4. Please make sure you advise your dancer to wait for parent inside the studio
5. Smoking is not permitted inside the studio or within 20 feet of the door. This includes the Nicotine smoking devices
6. Chewing gum is not permitted in the studio.
7. Food and drink is not permitted on the dance floor (water only).
8. Please keep walkways clear from dance bags or personal items.
9. Please! No PETS.
10. It is not the responsibility of Step 2 This to monitor dancers between classes.
11. Respect your fellow dancers, teachers and parents! If a dancer or parent's behavior is offensive, violent, aggressive or disrespectful, we reserve the right to refuse service.
