Required Policies and Agreements
A membership is required to attend any and all activities in GymnasticsEtc. Participation in GymnasticsEtc will not be allowed until the membership fee has been paid. This membership is due every August.

Should the parents desire GymnasticsEtc to administer their child(ren)'s medication, a written consent form must be completed by the parent and given to GymnasticsEtc. GymnasticsEtc may refuse to give medication. The medication must be in a proper container with the child's name and dosage information clearly marked.
If the child(ren) has suffered a serious accident or illness within the past twelve moths, is subject to a serious health condition, or there is any question about activity restriction, further information for the child(ren)'s doctor or specific permission to participate may be required at the discretion of the Director of GymnasticsEtc. GymnasticsEtc staff and volunteers are not qualified to care for some children with specil needs. GymnmasticsEtc will only accept those children after evaluation.

Tuition for classes is due by the 25th of the month (or on the last class of the month) to cover classes for the following month. If tuition is not paid by the last class of the month, the child(ren)'s card will be pulled and the parents will need to go the front desk and pay for class in order to receive the child(ren)'s card. So payment is due prior to the child(ren) attending the first class of the month. In addition, if no payment has been made by the first week of the month and we have not received any communication from the parents concerning tuition, the child(ren)'s spot in class will be made available to the general public. To make paying tuition more convenient, there is a payment drop-box at the front desk that check may be placed in. Auto-payment is also an option. Please see the front desk for more information.
Classes that are missed may be made up during the month in which they were missed providing there is availability in another class. If for any reason, a class is not able to be made up, there will be no credit or refund administered.

GymnasticsEtc is pleased to have you as a customer. Gymnastics, cheerleading, and tumbling are wonderful activities for healthy people of virtually all ages and levels of ability. These sports not only improve strength, muscularity and coordination, but our program also inspires focus, discipline and self-esteem. Above all gymnastics, cheerleading, and tumbling can be wonderful family-oriented activies.
However, gymnastics, cheerleading, and tumbling, like most other sports, carries with them an inherent risk of bodily injury or even death. These risks also extend to those present in our facility even if they are not actively participating in gymnastics, cheerleading, or tumbling activities. Such risks can be minimized through proper instruction, supervision, and education. However, strive as we may, such risks can never be eliminated. Your knowledge and appreciate of these risks is extremely important to your making an informed decision.
By signing this document, either individually, and/or in the capacity of a natural legal guardian, you acknowledge the inherent risks of bodily injury, psychological injury, or even death, in the activities of gymnastics, cheerleading, and tumbling, as well as through presence as a spectator. By signing this document, you either individually, and/or in the capacity of natural legal guardian, hereby release, hold harmless and exculpate GymnasticsEtc Inc, its officers, agents, assigns representatives, and employees, from any and all liability for their negligence in allegedly bringing about bodily injury, psychological injury or death, as well as to any claims of negligence, as set forth above, further extends to any defective condition of the premises whether or not known to GymnasticsEtc or its officers, agents, assigns, representatives, and employees, occurring off-premises or during transportation to or from related events.

I acknowledge that if a credit card is left on file on my account at GymnasticsEtc, I am giving the facility permission to run said card for any and all related charges I accumulate. If I am unable to pay any fees on my account, it is my responsibility to notify an employee in order to set u p a payment plan to get these charges paid off.

In the event that my child suffers any illness or injury requiring emergency treatment while involved in GymnasticsEtc. activity, I hereby give my permission for anynecessary hospitalization or medication on the recommendation of medical personnel, in which case I will be responsible for all costs. By signing this document, either indvidually and/or in the capacity of a natural or legal guardian, you acknowledge the inherent risks of bodily injury, psychological injury or even death, in the activies of gymnastics, swimming and any other activities GymnasticsEtc. may transport my child to as part of the camp or after school care program, as well as through presence as a spectator. By signing this document, you either individually and/or in the capacity of a natural guardian, hereby release, hold harmless and exculpate GymnasticsEtc. Inc., it's officers, agents, representatives and employees from any and all liability for their negligence in allegedly bringing about bodily injury, psychological injury or death. Your release of liability for negligence, set forth above, further extends to any defective condition of the premises whether or not known to occurring off-premises during transportation to or from related events.

In the event that my child(ren) suffers any illness or injury requiring emergency treatment while involved in any GymnasticEtc activity, I hereby give my permission for any necessary hospitalization, medication, or surgery on the recommendation of the medical personnel, in which case I will be responsible for all costs.

I give my permission for my child to participate in all GymnasticsEtc activities for the time period indicated, including those activities held away from the regular activity sites. I understand that GymnasticsEtc will inform me as to off- site activities and locations prior to my child(ren)'s participation. I will be responsible for transportation to and from activities or am willing to permit GymnasticsEtc to select transportation methods of my child.

I agree that the information I have provided is true and complete to the best of my knowledge. I understand that it is my responsibility to notify GymnasticsEtc as to any changes in the status of my permission or my child(ren)'s health history prior to their participation in any GymnasticsEtc activities. I understand that it is also my responsibility to inform GymnasticsEtc when my or my emergency contact's information changes.
