COVID-19
I acknowledge that an inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. According to the Centers for Disease Control and Prevention, senior citizens and people with underlying medical conditions are especially vulnerable.
By enrolling my child at Dynamic Gymnastics I voluntarily assume all risks related to exposure to COVID-19.
Release of Liability
I hereby represent and warrant to the Released Parties that I have the authority to execute this Participant Waiver Form on behalf of myself and/or on behalf of my minor child(ren) or ward(s) as parent, guardian and/or next friend, if applicable. In the event of any misrepresentation or breach of the foregoing warranty by me, or in the event that I, my minor child(ren) or ward(s), or any other person nevertheless asserts any claim against the Released Parties arising out of my or my minor child(ren)'s or ward(s)' attendance or participation in the Activities as set forth herein, I agree to indemnify, hold harmless and defend the Released Parties from and against any and all liability, claims, losses, costs, expenses or damages resulting therefrom, including, but not limited to, claims of loss, damage, illness, injury or death to person or property whether or not such loss, damage, illness, injury or death results from the negligence of the Released Parties or from some other cause.
Assumption of Risk
I expressly acknowledge that there are certain dangers, risks, illnesses, personal injuries and death inherent in attending and/or participating in Dynamic Gymnastics' programs, events, classes, and any other activities (including exercise) at and/or sponsored by Dynamic Gymnastics, which may result from falls, accidents or injuries, negligence of any person or organization, my participation in programs, events, classes and any other activities, the use of any facilities and equipment, exercise, or from my or my minor child(ren)'s or ward(s)' physical condition (collectively, the "Activities"). I understand that Dynamic Gymnastics, its owners, employees, counselors, teachers, trainers, representatives, successors and assigns (collectively "Released Parties") assume no responsibility for loss, damage, illness, injury or death to person or property that I or my minor child(ren) or ward(s), if applicable, may sustain arising out of, resulting from or in any way connected with the Activities, including by reason of the active or passive negligence of any of the Released Parties. I expressly acknowledge, on behalf of myself and my minor child(ren) and ward(s), heirs and executors, that I voluntarily assume the sole risk for any and all losses, damages, illnesses, personal injuries or death that may result from my or my minor child(ren)'s or ward(s)' Activities at and/or sponsored by Dynamic Gymnastic. In consideration of the Released Parties allowing me and/or my minor child(ren) or ward(s) to attend and/or participate in the Activities, I hereby, for myself, my minor child(ren) or ward(s), heirs, and executors, waive, release and forever discharge the Released Parties from and against any and all rights and claims for any loss, damage, illness, injuries or death to person or property sustained arising out of, resulting from or in any way connected with the Activities, whether or not such loss, damage, illness, injury or death results from the negligence of the Released Parties or from some other cause.
Photo & Video Release
I also acknowledge that the Released Parties may use photographs, videotapes, television programs, motion pictures, tape recordings, or other similar media pertaining to the Activities for promotional purposes. I hereby consent to the use of my and/or my minor child(ren)'s or ward(s)' name(s) and/or likeness(es) in such materials to be exhibited and used for advertising, trade purposes, solicitation of patronage, promotional purposes, or other similar purposes, even if my and/or my minor child(ren)'s or ward(s)' name(s) and/or likeness(es) are an integral part of such photograph, videotape, television program, motion picture, tape recording, or other similar media. I further waive any and all rights to inspect or approve the photograph, videotape, television program, motion picture, tape recording or other use of my and/or my minor child(ren)'s or ward(s)' name(s) and/or likeness(es), including any written article, script, caption or other writing that may accompany such use of my and/or my minor child(ren)'s or ward(s)' name(s) and/or likeness(es). I hereby, for myself, my minor child(ren) or ward(s), heirs, and executors, waive, release and forever discharge the Released Parties from and against any and all liability, claims, losses, costs, expenses or damages for libel, slander, invasion of privacy, conversion, defamation, appropriation of likeness or any other claim based on the use of my and/or my minor child(ren)'s or ward(s)' name(s) and/or likeness(es) in any such materials.
Weather Policy
Dynamic Gymnastics follows the Fairfax County Public Schools weather closings and delays. If Fairfax schools are closed, we are closed. In the event of a 2 hour delay, Dynamic Gymnastics will open at noon. Credits will not be given for any classes missed due to weather related closures.
Drop off/ Pick up Policy
The staff of Dynamic Gymnastics makes the safety of your child our top priority. For this reason, we ask parent/guardians of children in instructional classes to come into the gym to get their children. Please advise your child to check with the front desk if they do not see their ride. Dynamic Staff may not escort a child outside the building. Late pickups will incur a $25 charge per 15 minute increment to enable us to pay staff overtime as they wait for your pickup.
If you know you will be delayed, please call the gym at (703) 738-6000 Bailey's Location, so we can let your child know you will be late.
No class makeups
Dynamic Gymnastics does not offer class makeups because class sizes are set with a coach-to-gymnast ratio to ensure your child's safety. Adding additional children to a class to accommodate makeup requests would impact these ratios and put children at risk.
Medical/ Injury Policy
Medical bracelets are allowed in class but are encouraged, for the safety of the athlete and the instructor, to be covered with either a wrist band or athletic tape.
Should your child arrive to the gym with a minor injury, please come to the gym with the appropriate dressings.
Should your child fall ill during class, please be available to pick them up. If you are not available, we will refer to the emergency contact that you have provided.
Should your child become sick or injured so much that he/she can not attend class, a physician's written approval must be provided for your child to return to class participation.
This is a nut free facility, please leave all food & drinks containing any tree nut products at home.
Rolling Enrollment Policy
I understand I am choosing a year-round class. This means my child will be enrolled in this class until I ask to remove my child or until my child advances to the next level. I understand my credit card on file with Jackrabbit will be charged on the 1st of each month until I withdraw my child from this class in writing. To withdraw my child I will email frontdesk@griffingymnastics.com by the 15th of the month prior to my requested drop date. I understand that if I do not email the front desk my card will automatically be charged. Tuition is due in full on the 1st of the month. If for any reason my card is declined, I understand I will incur a $25.00 late fee unless I provide Dynamic Gymnastics with new credit card information. I understand that if I have not paid by the second class of the month my child will be dropped from the class.

If your child isn't happy in class during the first 30 days, simply email our customer service team at frontdesk@griffingymnastics.com and we'll refund your tuition. After your first 30 days tuition is not refundable unless Dynamic Gymnastics cancels a class. If a student decides to voluntarily drop out of a class mid-month they are still responsible for that month's tuition. If Dynamic Gymnastics cancels a class, you have the option to receive a refund or receive a credit on your account towards tuition within a certain time period. Household credits are not transferable to other households. Refunds may also be granted with proper documentation in the case of relocation orders or medical circumstances only.
