Assumption of Risk/Indemnity Waiver
AMP LLC/AMP Athletics strives to be completely safe in all areas of coaching/instruction with certified instructors. As parent/guardian of the above listed child, I understand there is always a risk of injury while performing skills, while participating in gymnastics, cheerleading, dance, and/or tumbling. I have found the facilities at 197 Kinetic Drive, Huntington WV 25701 (the Facilities) to be safe and reasonably suited for the purposes for which my child will participate and shall only enter the Facilities in the future after making the same determination. I agree to indemnify, defend, and hold harmless AMP LLC/AMP Athletics against all claims, Causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may arise, in whole or in part, from my or my child's use of, or presence upon the Facilities. I agree to pay for all damages to the Facilities caused, in whole or in part, by my or my child's negligent, reckless, or willful actions. I agree that any claim arising under or related to this waiver, release of liability or indemnity shall be governed by West Virginia law.
Payment Policies
Tuition is due on or before the 15th day of each month. Your monthly payment is to ensure that you have reserved your spot in your class and to allow us to keep our classes to a manageable size. If payment is not received by the 15th, day of each a late charge of $10 will be assessed. You are able to make your monthly payment online through the parent portal. It is not AMP Athletics responsibility to remind you that tuition is due, however notices will be posted in the gym. We accept cash, check, credit cards, or money orders.
Withdrawal From Class
PARENTS MUST NOTIFY AMP ATHLETICS TO DROP A STUDENT FROM ANY CLASS OR TEAM. There are withdrawal forms available at the front counter and our website. This form must be completed, dated, and returned to the office. Please note: You are responsible for payment for your student's classes whether or not your student attends class.
Photo Waiver
As parent/guardian, I hereby allow AMP LLC (dba AMP Athletics) the copyright to publish and use photographic portraits, pictures or videos of the above listed child for use through reasonable promotion of AMP Athletics (gymnastics, cheerleading, dance, etc.).
Medical Release/Medical Insurance
I hereby authorize any AMP LLC (dba AMP Athletics) certified employees or instructors to treat my child in case of a medical emergency. I also authorize any medial employee, emergency unit, hospital or doctor to render immediate first aid as might be required in case of a medical emergency. This would include any emergency on the premises of AMP Athletics, any event at another location that AMP Athletics participates in, or any other event(s) involving the students at AMP Athletics. In addition as parent/guardian I agree to carry medical insurance on the above listed child for the duration of time the child participates in gymnastics, cheerleading, dance, tumbling, or any other activity at AMP LLC/AMP Athletics.
COVID-19 Policy
With the widespread pandemic of the Coronavirus (COVID-19), AMP Athletics is continuing to take proactive steps to maintain a safe environment to help protect the health of our staff, athletes, and families.
As our fall session begins, we are asking parents to be diligent about entering the gym.
- Face coverings are mandatory and must be left on the entire length of your visit.
- We will continue monitoring the door by taking temperatures and asking the following questions.
* Q1 Have you traveled out of state in the past week?
* Q2 Are you experiencing a cough, shortness of breath, or sore throat?
* Q3 Have you had a fever in the last 48 hours?
* Q4 Have you had loss of taste or smell?
* Q5 Have you had vomiting or diarrhea in the last 24 hours?
- Parents are welcome.
- We are asking you to limit the number of people you bring with you.
- If possible, leave siblings at home. if not possible, we ask you watch them closely, bring items to entertain them and limit the surfaces your child touches.
- If your or your child are not feeling well, please stay home and call for a make-up class.
- If you or anyone in your family has tested positive for COVID-19, we ask you call the gym and let us know immediately.
Our policy for being EXPOSED to COVID-19: Quarantine for 10 days after exposure or have a COVID-19 test no sooner than 3 days after exposure with a negative result.
Our policy for anyone who tests positive for HAVING COVID-19: Quarantine until a COVID-19 test is negative or be symptom free for 3 days.
There will be no refunds on classes, but make-up are always available. We will work with each family on an individual basis.
