• Pay by credit card through our secure parent portal, with the options of keeping your card on file for autopay or making a one-time payment.
• September payment must be made on or before the 1st of the month including the annual registration/insurance fee.
• All subsequent payments are required by the 1st of each month.
• Students taking one class per week will be required to pay in 2 month increments.
• A $10 late fee will be imposed on payments made after the 15th day of the month.
• If tuition is in arrears, students will not be permitted to return to classes the following month until account is in good standing
• Should the school be required to temporarily close its doors due to Covid, all classes will continue on Zoom.
• Students who do not want to continue classes on Zoom will receive a tuition refund beginning the following month.
• Students who continue on Zoom will receive classes at a discounted rate.
State COVID protocols
We have built a new schedule and set protocols to accommodate and adhere to State Covid guidelines:
• Small classes with 6 students in the upstairs studio, 8 students in the downstairs studio.
• 15 minutes between classes to clean.
• Separate entrances for each studio.
• Specific protocols for students to enter and exit the building.
• No family in the building. Students must be dropped off and picked up outside only.
• One volunteer parent in the building during the level K- 3 classes to watch and supervise.
• Open windows and ventilation procedures.
• Mask wearing by teachers.
• Zoom classes available for all classes.
Waiver/Release
Permission Slip and COVID-19 Precautions Form and Release
I/We, the parents/guardian, give our permission for my dancer to take classes at Main Street Ballet ("MSB") and participate in MSB performances, events and/or productions ("Productions"). I hereby grant to the Main Street Ballet ("MSB"), the right to use any photographs, films, videos or recordings that MSB makes and/or has made of my child (collectively the "Recordings) or any portions thereof, edited or unedited, in any medium, including but not limited to print, electronic, video, audio or film for the purpose of promoting or advertising MSB and its productions or classes.
I agree that my child will abide by all rules and regulations reasonably adopted in connection with MSB, any Production and the use of the facilities provided for MSB classes or Production. I understand that the failure of my child to observe these rules and regulations may result in my child being excluded from classes or from participation in any Production.
I agree to waive any and all claims of any kind whatsoever, whether resulting from an injury or otherwise, and further agree to release, indemnify and hold the MSB, its employees, contractors, volunteers, officers, directors, and representatives harmless from and against any and all liability arising out of or relating to my child's participation in MSB classes or any Production.
I further understand that MSB assume no liability for lost, misplaced stolen and/or damaged property and I hereby agree to release MSB from such liability.

Furthermore, I will immediately inform Main Street Ballet School and discontinue classes if I, or any member of my household, develops any of the aforementioned symptoms. I affirm that neither I, nor any member of my household, has been diagnosed with COVID-19 within the past 30 days. Furthermore, I will immediately inform Main Street Ballet and discontinue classes if I, or any member of my household, is diagnosed with COVID-19. I affirm that neither I, nor any member of my household, has knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days. Furthermore, I will immediately inform Main Street Ballet and discontinue classes if I, or any member of my household, is knowingly exposed to anyone diagnosed with COVID-19. I affirm that neither I, nor any member of my household, has traveled outside of the country or to any city considered to be a "hot spot" for COVID-19 infections within the past 30 days. Furthermore, I will immediately inform Main Street Ballet and discontinue classes once I, or any member of my household, returns from traveling outside of the country or to any city considered to be a "hot spot" for COVID-19 infections. I understand that Main Street Ballet cannot be held liable for any exposure to the COVID-19 virus caused by any misinformation on this form or the health history provided by each Student.

Clean/Disinfect frequently touched surfaces thoroughly and at regular intervals.
Wash/Sanitize our hands before and after each session.
Maintain a safe distance at all times.
Accommodate a limited number of students in each class to ensure physical distancing.
Schedule classes so as to allow time for cleaning/disinfecting surfaces and allow students to finish and leave the studio before the next set of students arrive.
Provide sanitizers at the entrance to the studio
