ALL PARENTS (OR DANCERS, IF NOT A MINOR) MUST SIGN BEFORE ATTENDING ANY THE STUDIO SCHOOL OF DANCE.
I, THE UNDERSIGNED, AGREE THAT THE ABOVE INFORMATION IS CORRECT. I RECOGNIZE AND UNDERSTAND THE RISKS OF PHYSICAL INJURY INHERENT IN DANCE TRAINING AND I AM WILLING TO ASSUME THOSE RISKS. I WILL NOT HOLD THE STUDIO SCHOOL OF DANCE AKA PEACOCK COLLECTIONS, LLC. OR ANY FACULTY MEMBER, EMPLOYEE, GUEST INSTRUCTOR, OR VOLUNTEER LIABLE FOR INJURIES SUSTAINED OR ILLNESSES CONTRACTED BY THE PARTICIPANT WHILE IN ATTENDANCE AND/OR PARTICIPATING IN CLASSES IN THE ABOVE-MENTIONED SCHOOL. I UNDERSTAND THAT THE STUDENT MAY BE PHYSICALLY TOUCHED DURING DANCE TRAINING AND/OR DANCE INSTRUCTION. I WILL NOT HOLD THE STUDIO SCHOOL OF DANCE AND ITS FACULTY MEMBERS LIABLE FOR PHYSICAL TOUCHING WHICH IS INCIDENTAL TO AND IN THE COURSE OF DANCE INSTRUCTION AND TRAINING. I AGREE TO INDEMNIFY THE STUDIO SCHOOL OF DANCE AND ITS FACULTY MEMBERS AND VOLUNTEERS OF BOTH FOR ALL LIABILITIES, COSTS, AND JUDGMENTS ARISING FROM ACTS OF OMISSION COMMITTED BY ME OR MY CHILD WHICH MAY RESULT IN INJURY OR DAMAGE TO ANY PERSON OR PARTY.
COVID-19 ACKNOWLEDGEMENT AND DISCLOSURES STUDENT / FAMILY
1. I UNDERSTAND THAT DURING THIS COVID-19 PUBLIC HEALTH EMERGENCY I AS A PARENT WILL NOT BE PERMITTED TO ENTER THE FACILITY BEYOND THE DESIGNATED DROP OFF AND PICK UP AREA. I UNDERSTAND THAT THIS PROCEDURE CHANGE IS FOR THE SAFETY OF ALL PERSONS PRESENT IN THE FACILITY AND TO LIMIT THE EXTENT POSSIBLE EVERYONE'S RISK OF EXPOSURE.
2. I UNDERSTAND THAT IF THERE IS AN EMERGENCY REQUIRING ME TO ENTER THE FACILITY BEYOND THE DESIGNATED DROP-OFF / PICKUP AREA, I MUST SANITIZE MY HANDS BEFORE ENTERING, REMOVE MY SHOES AND WEAR A MASK. WHILE IN THE FACILITY I MUST PRACTICE SOCIAL DISTANCING AND REMAIN 6 FEET FROM ALL OTHER PEOPLE EXCEPT FOR MY OWN CHILD.
3. I UNDERSTAND THAT TO ENTER THE STUDIO PREMISES MY CHILD MUST BE FREE OF COVID-19 SYMPTOMS. IF, DURING THE DAY, ANY OF THE FOLLOWING SYMPTOMS APPEAR MY CHILD WILL BE SEPARATED FROM THE REST OF THE PEOPLE IN THE STUDIO, I WILL BE CONTACTED AND MY CHILD MUST BE PICKED UP FROM THE STUDIO WITHIN 15 MINUTES.
SYMPTOMS INCLUDE:
• FEVER OF 100.4 DEGREES FAHRENHEIT OR HIGHER
• DRY COUGH
• SHORTNESS OF BREATH
• CHILLS
• LOSS OF TASTE OR SMELL
• SORE THROAT
• MUSCLE ACHES
WHILE MANY SYMPTOMS MIGHT BE NON-COVID-19 SYMPTOMS, WE ARE PROCEEDING WITH BY ASSUMING THEY ARE COVID-19 DURING THIS PUBLIC HEALTH EMERGENCY. THESE SYMPTOMS TYPICALLY APPEAR 2-7 DAYS AFTER BEING INFECTED, SO PLEASE TAKE THEM SERIOUSLY. YOU WILL NEED TO BE SYMPTOM FREE FOR 72 HOURS BEFORE RETURNING TO THE FACILITY
4. I UNDERSTAND THAT MY CHILD MUST WEAR A MASK WHEN MOVING THROUGHOUT THE BUILDING. I UNDERSTAND MY CHILD WILL HAVE THE OPTION TO REMOVE THE MASK TO PARTICIPATE IN CLASS AND ONLY WHEN PARTICIPATING IN CLASS.
5. I UNDERSTAND THAT STUDENTS WILL BE REQUIRED TO USE HAND SANITIZER ACCORDING TO THE CDC RECOMMENDED HAND WASHING PROCEDURES EVERY TIME THEY TOUCH A SURFACE IN THE STUDIO
6.I UNDERSTAND THAT THE STUDENT'S TEMPERATURE WILL BE TAKEN PRIOR TO ENTERING THE BUILDING. THEY WILL BE GIVEN HAND SANITIZER FOR THEIR HANDS; THEY WILL STEP ON A DISINFECTANT MATT TO CLEAN SHOES PRIOR TO GOING INTO STUDIO MAIN LOBBY. STUDENTS WILL THEN REMOVE THEIR SHOES AT THE ENTRANCE OF THE STUDIO AND PLACE THEM IN A DESIGNATED CUBBY. DANCERS WILL THEN SELECT A DESIGNATED SPOT ON THE FLOOR FOR THEIR BELONGS TO REMAIN DURING THEIR CLASS TIME.
7. I WILL IMMEDIATELY NOTIFY THE STUDIO SCHOOL OF DANCE IF I BECOME AWARE OF ANY PERSON WITH WHOM MY CHILD OR I HAVE HAD CONTACT EXHIBIT ANY OF THE SYMPTOMS LISTED ABOVE, IS ADVISED TO SELF-ISOLATE, QUARANTINE, OR HAVE TESTED POSITIVE, OR IS PRESUMED POSITIVE FOR COVID-19 .
8. I UNDERSTAND THAT STUDENTS ENTERING OUR STUDIO WILL BE IN CONTACT WITH CHILDREN, FAMILIES AND EMPLOYEES WHO ARE AT RISK OF COMMUNITY EXPOSURE. I UNDERSTAND THAT NO LIST OF RESTRICTIONS, GUIDELINES OR PRACTICES WILL REMOVE 100% OF THE RISK OF EXPOSURE TO COVID-19 AS THE VIRUS CAN BE TRANSMITTED BY PERSONS WHO ARE ASYMPTOMATIC AND BEFORE SOME PEOPLE SHOW SIGNS OF INFECTION. I UNDERSTAND THAT I PLAY A CRUCIAL ROLE IN KEEPING EVERYONE IN THE FACILITY SAFE AND REDUCING RISK BY FOLLOWING THE PRACTICE IS OUTLINED HERE
9. SHOULD THERE BE A SPIKE IN COVID-19 CASES, AND WE ARE FORCED INTO A STAY AT HOME ORDER AGAIN, ALL CLASSES WILL CONTINUE VIRTUALLY. TUITION WILL REMAIN THE SAME FOR ANY AMOUNT OF TIME SPENT DOING VIRTUAL CLASSES. NO REFUNDS WILL BE GIVEN.
I CERTIFY THAT I HAVE READ, UNDERSTAND, AND AGREE TO COMPLY WITH THE PROVISIONS LISTED HEREIN. I GIVE PERMISSION FOR THE STUDIO SCHOOL OF DANCE TO TAKE PHOTOS AND/OR VIDEOS OF ME OR MY CHILD WHILE PARTICIPATING IN THE STUDIO SCHOOL OF DANCE ACTIVITIES FOR PROMOTIONAL PURPOSES, INCLUDING, BUT NOT LIMITED TO POSTING ON FACEBOOK, TWITTER, PINTEREST, INSTAGRAM, YOUTUBE, SNAPCHAT, THE STUDIO WEBSITE AS WELL AS PRINTED PROMOTIONAL MATERIAL. NAMES OF STUDENTS WILL NOT BE USED OR DISCLOSED UNLESS WRITTEN CONSENT FROM PARENTS HAS BEEN RECEIVED.

STUDIO COMMUNITY EXPECTATIONS
CONDUCT: THE STUDIO SCHOOL OF DANCE STUDENTS ARE EXPECTED TO FOLLOW DRESS CODE GUIDELINES AND TO BE PUNCTUAL, COURTEOUS HARDWORKING, DEDICATED, AND ENTHUSIASTIC.
CONDUCT OVERSIGHT: THE STUDIO SCHOOL OF DANCE DIRECTOR, FACULTY AND STAFF WILL ACTIVELY MANAGE ANY BEHAVIORAL SITUATION THAT INVOLVES RUDENESS, DISRESPECT, LATENESS EXCESSIVE ABSENTEEISM, LACK OF PREPAREDNESS AND BULLYING. DEPENDING ON THE SEVERITY OF THE INFRACTION, MYRIAD CONSEQUENCES CAN INCLUDE:
• PARENT MEETING
• REVOKING SCHOLARSHIPS
• LEVEL DEMOTION
• REMOVAL FROM ANY PERFORMANCE OPPORTUNITY
• SCHOOL SUSPENSION OR EXPULSION
STUDIO ETIQUETTE: ALL STUDENTS ARE EXPECTED TO REMAIN OUTSIDE OF THE STUDIO UNTIL THEIR INSTRUCTOR ARRIVES. ONCE INSIDE THEY MAY NOT SIT, HANG OR LEAN ON THE BARRES AND MIRRORS. STUDENTS ARE EXPECTED TO ACTIVELY PARTICIPATE AND REMAIN IN CLASS FOR THE ALLOTTED PERIOD OF TIME. STUDIOS THAT ARE NOT ACTIVELY HOSTING A CLASS SHOULD NOT BE USED AS A PLAYGROUND FOR SIBLINGS OR KIDS VISITING THE STUDIO. IT IS IMPORTANT THAT THAT THE STUDIO SPACE IS TO BE RESPECTED AND REMAIN A PLACE OF CREATIVITY. ONLY WATER IS PERMITTED IN NON-GLASS CONTAINERS. NO STREET SHOES ARE ALLOWED IN THE STUDIOS.
DISCIPLINARY PROTOCOL: THE STUDIO SCHOOL OF DANCE HAS A ZERO- TOLERANCE POLICY FOR DISCRIMINATION BULLYING TEASING OR NEGATIVE COMPORTMENT AT ANY TIME FOR ANY REASON. ALL PARTIES INVOLVED WILL BE SPOKEN WITH AND PARENTS WILL BE CONTACTED. DISCIPLINARY AND SCHOOL COMMUNITY ISSUES AND IN FRACTIONS OF RULES WILL BE HANDLED IN THE FOLLOWING MANNER AT THE DISCRETION OF THE SCHOOL LEADERSHIP:

IT IS A RESPONSIBILITY OF THE PARENT OR GUARDIAN TO CHECK EMAIL, ENSURE THE OFFICE HAS A PROPER EMAIL ADDRESS FOR THE PERSON MANAGING THE DETAILS OF THE CHILD. THE STUDIO SCHOOL OF DANCE WILL ALSO POST IMPORTANT INFORMATION TO THEIR MEMBER ONLY FACEBOOK PAGE AND MOBILE APP. IT IS HIGHLY RECOMMENDED THAT ALL REQUEST ACCESS TO OUR STUDIO MEMBER FACEBOOK PAGE AS WELL AS DOWNLOAD OUR MOBILE APP.
ATTENDANCE/ LATENESS
ATTENDANCE:
• ATTENDANCE AND LATENESS ARE TRACKED DAILY. IF YOU CHILD IS ABSENT YOU WILL RECEIVE NOTIFICATION FROM OUR ADMINISTRATIVE TEAM.
• INCONSISTENT ATTENDANCE WILL SLOW DOWN YOUR CHILD PROGRESSION AND CAUSE FRUSTRATION FOR EVERYONE IN THE CLASS. ATTENDANCE IS A MUST FOR YOUR CHILD TO EXCEL PHYSICALLY AND MENTALLY WITH HIS /HER TECHNIQUE AND IS NECESSARY FOR REASONS OF CHOREOGRAPHY USED IN CLASS AND PLACEMENT WITHIN CHOREOGRAPHY. EXCESSIVE ABSENCES MAY AFFECT LEVEL OF ADVANCEMENT.
• ANTICIPATED ABSENCES MUST BE COMMUNICATED TO THE STUDIO MAIN OFFICE VIA EMAIL AT THESTUDIONOLA@GMAIL.COM OR BY CALLING THE STUDIO OFFICE AT (504) 941-7345. EACH PROGRAM IN THE STUDIO SCHOOL OF DANCE HAS ITS OWN DETAILED ATTENDANCE POLICY PLEASE REFER TO YOUR PROGRAM DIVISION FOR THESE DETAILS.
• FAILURE TO ADHERE TO OUR ATTENDANCE POLICIES WILL RESULT IN POSSIBLE SUSPENSION FROM THE PROGRAM, REMOVAL FROM PROGRAM, AND OR REMOVAL FROM PERFORMANCE OPPORTUNITIES. THE DECISION TO IMPLEMENT A CONSEQUENCE IS MADE IN CONSULTATION WITH TEACHERS AND SCHOOL LEADERSHIP.
LATENESS: A CHILD'S PREPARATION FOR DANCE AND LIFE STARTS HERE. STUDENTS WHO ARE LATE TO CLASS MUST WAIT AT THE DOOR UNTIL THEY ARE INVITED TO JOIN OR TO OBSERVE THE CLASS. IF A STUDENT IS LATE, THEIR PARTICIPATION IS UP TO THE DISCRETION OF THE TEACHER, AS THEIR LATENESS INCREASES THEIR LIABILITY FOR INJURY, AND IT IS DISRUPTIVE TO THE FLOW OF THE CLASS. EXCESSIVE LATENESS WILL AFFECT LEVEL PLACEMENTS, SCHOLARSHIP, AND PARTICIPATION IN PERFORMANCES.

ARRIVAL, DISMISSAL AND LEAVING:
STUDENTS ARE EXPECTED TO ARRIVE 10 TO 15 MINUTES BEFORE CLASS. ALL STUDENTS MUST HAVE AN ASSIGNED KEY CARD TO BE SCANNED INTO THE STUDIO SCHOOL OF DANCE UPON ARRIVAL TO THE BUILDING. ONLY A MAXIMUM OF TWO KEY CARD PER STUDENT CAN BE ASSIGNED. LOST CARDS WILL INCUR A REPLACEMENT FEE OF $30.00. PETITE DANCER FAMILIES SHOULD REFER TO THE DISMISSAL PROTOCOL DETAILED ON PAGE 7 OF THE STUDENT HANDBOOK. IF STUDENT IS PERMITTED TO LEAVE THE PREMISES WITHOUT SUPERVISION A CONSENT FORM MUST BE COMPLETED BY A PARENT OR GUARDIAN AT THE START OF THE SCHOOL YEAR AND THEY ARE AVAILABLE IN THE MAIN OFFICE.
LOW ENROLLMENT
LOW ENROLLMENT: THE STUDIO SCHOOL OF DANCE RESERVES THE RIGHT TO CANCEL CLASSES DUE TO LOW ENROLLMENT. THE STUDIO SCHOOL OF DANCE REQUIRES A MINIMUM OF 4 STUDENTS FOR THE CLASS TO REMAIN ON OUR SCHEDULE. FAMILIES WILL BE NOTIFIED OF LOW ENROLLMENT CANCELLATIONS WITH ADVANCED NOTICE AN OFFERED THE OPTION TO TRANSFER TO ANOTHER CLASS. IF WE ARE UNABLE TO FIND A SUITABLE CLASS FOR THE STUDENTS TO TRANSFER TO, A STUDIO CREDIT FOR REMAINING CLASSES WILL BE OFFERED.
TUITION AND OTHER PAYMENT INFORMATION
BY REGISTERING FOR THE SCHOOL YEAR, STUDENTS AND PARENTS ARE COMMITTING TO PAY FOR THE PROGRAM. REGISTRATION IS NOT CONSIDERED COMPLETE UNTIL PAYMENT IS SUBMITTED. ALL FAMILIES ENROLLED AT THE STUDIO ARE REQUIRED TO PROVIDE A CREDIT CARD AND/OR BANK ACCOUNT INFORMATION TO BE KEPT ON FILE. THE STUDIO SCHOOL OF DANCE DOES NOT PROVIDE MONTHLY INVOICES. IT IS THE ACCOUNT HOLDER'S RESPONSIBILITY TO PAY TUITION ON TIME. TUITION IS BASED ON TOTAL CLASS HOURS.
• TUITION PAYMENTS ARE DUE BY THE 3RD OF EACH MONTH AND ARE CONSIDERED DELINQUENT BY THE 6TH OF EACH MONTH.
• A $25 LATE FEE WILL BE INCURRED FOR EACH MONTH THAT PAYMENT HAS NOT BEEN RECEIVED BY THE 10TH OF THE MONTH.
• ACCOUNTS MORE THAN 60 DAYS DELINQUENT MAY RESULT IN SUSPENSION OF LESSONS.
• ACCOUNTS MORE THAN 90 DAYS DELINQUENT WILL BE SUBMITTED TO A COLLECTION AGENCY.
• ALL RETURNED CREDIT CARD TRANSACTIONS WILL INCUR A 3.5% TO COVER PROCESSING FEES. $35.00 FEE WILL APPLY TO BANK OVERDRAFTS AND RETURNED CHECKS.
• ALL TUITION AND FEES PAID TO THE STUDIO SCHOOL OF DANCE ARE NON-REFUNDABLE AND NON-TRANSFERABLE.
PAYMENT TERMS
TUITION MUST BE PAID IN ADVANCE. ALL SEMESTER PAYMENTS MUST BE MADE WITHIN THE FIRST WEEK OF THE SEMESTER TO RECEIVE THE DISCOUNT. TUITION IS DIVIDED INTO 9 MONTHS (SEPTEMBER- MAY) TO HELP THOSE THAT CANNOT PAY THE SUM IN FULL. TUITION MUST BE PAID EVEN IF CLASSES ARE MISSED,STUDIO NEEDS TO CLOSE DUE TO WEATHER OR HEALTH CRISIS. ANY STUDENT WISHING TO BE REMOVED FROM A CLASS MUST SUBMIT A 30-DAY NOTICE IN WRITING TO BE DROPPED FROM THE CLASS AND THE ACCOUNTING SYSTEM WILL BE ADJUSTED.
YOU ARE RESPONSIBLE FOR YOUR ACCOUNT BALANCE UNTIL SUCH NOTIFICATION IS RECEIVED. ANY STUDENT WISHING TO DEPART THE STUDIO SCHOOL OF DANCE MUST PAY ACCOUNT BALANCE IN FULL AT TIME OF DEPARTURE.
THE FOLLOWING ARE THE PAYMENT OPTIONS:
• QUARTERLY- TUITION TO BE PAID IN 4 INSTALLMENTS
• TWO-INSTALLMENTS (SAVE 5%) - TUITION TO BE PAID AT THE BEGINNING OF EACH SEMESTER IN 2 INSTALLMENTS.
• FULL SEASON (SAVE 10%) - TUITION PAID IN FULL AT TIME OF REGISTRATION OR BY THE FIRST DAY OF CLASS.
• MONTHLY- 9 INSTALLMENTS, INCLUDES A CONVENIENCE FEE OF $10 EXTRA PER MONTH (THIS IS ALREADY INCLUDED IN TUITION)
AUTO PAY
AUTO PAY: ALL FAMILIES HAVE THE OPTION TO HAVE A CREDIT CARD CHARGED AUTOMATICALLY FOR TUITION. TO HAVE THE PAYMENT AUTOMATICALLY PROCESSED, FAMILIES MUST FILL OUT AUTO-PAY FORM AT THE STUDIO.
AUTO PAY ONLY APPLIES TO TUITION. ADDITIONAL FEES, SUCH AS RECITAL FEE, COMPETITION FEES, MASTERCLASSES, SUMMER WORKSHOPS, ETC. WILL REQUIRE ADDITIONAL AUTHORIZATION FROM THE PERSON RESPONSIBLE FOR THE ACCOUNT.
ACCOUNTS ON AUTO PAY WILL BE PROCESSED ON THE DUE DATE OR, IF THE DUE DATE FALLS ON A WEEKEND, THE NEXT BUSINESS DAY. FAMILIES ARE RESPONSIBLE FOR UPDATING CREDIT CARD INFORMATION.
PARENTS/ GUARDIAN GUIDELINES
PLEASE BE A GOOD ROLE MODEL. COMMUNICATE CONCERNS SOONER RATHER THAN LATER BY CONTACTING THE STUDIO SCHOOL OF DANCE OFFICE TO MAKE AN APPOINTMENT. EVERY EFFORT WILL BE MADE TO ADDRESS YOUR CONCERNS IN A TIMELY MANNER. OUR FACULTY IS NOT TO BE INTERRUPTED BEFORE DURING OR IN BETWEEN CLASSES. WE ASK THAT YOU REFRAIN FROM DISTRACTING CLASSES WITH LENGTHY STAYS BY THE OBSERVATION WINDOW OR DOOR SO INSTRUCTION IS NOT INTERRUPTED. YOU MAY NOT TAKE PHOTOS OR VIDEOS OF CLASSES EXCEPT DURING PARENT VISITING WEEK. POOR INAPPROPRIATE PARENT OR GUARDIAN COMPORTMENT WILL RESULT IN A MEETING WITH THE SCHOOL LEADERSHIP.
