Liability Release
I fully understand that Charlotte Allstar Gymnastics & Cheerleading Staff members are not physicians or medical practitioners of any kind. With this in mind, I hereby release the Charlotte Allstar Gymnastics & Cheerleading Staff to render temporary first aid to my child or children in the event of any injury, and if deemed necessary by the Charlotte Allstar Gymnastics & Cheerleading Staff, to seek medical help, or the calling of an ambulance for said child should the Charlotte Allstar Gymnastics & Cheerleading staff deem necessary.
We, the staff of Charlotte Allstar Gymnastics & Cheerleading recognize our obligation to make our students and their parents aware of the risks and hazards associated with the sports of Gymnastics, tumbling, and Cheerleading. Students may suffer injuries, possibly minor, serious or catastrophic (paralysis or even death) in nature. Gymnastics, tumbling, and Cheerleading can be dangerous and can lead to injury!!
Parents should make their children aware of the possibility of injury and encourage their children to follow all the safety rules and the coaches' instructions.
Charlotte Allstar Gymnastics & Cheerleading, its coaches and other staff members, will not accept responsibility for injuries sustained by any student during the course of gymnastics, tumbling, or cheerleading instruction, or open workouts, or in the course of any exhibition, competition, or clinic in which he or she may participate.
With the above in mind, and being fully aware of the risks and possibility of injury involved, I consent to have my child or children participate in the program(s) offered by Charlotte Allstar Gymnastics & Cheerleading and I waive all rights and claims for damages that I or my child may have against Charlotte Allstar Gymnastics & Cheerleading and/or its representatives whether paid or volunteer.
Covid Waiver
I am aware that there will be times when incidental contact will occur while my child is participating in Cheerleading and Gymnastics. This can include tumbling on the Spring Floors, Tumble Tracks, Rod Strip and using the Trampolines. Charlotte Allstar Gymnastics & Cheerleading programs are operating in a social and physical distancing environment but even with best efforts and intentions there will be times when the children will breach the prescribed (currently 6') distancing recommendation. In addition, our teaching and coaching staff will spot (physically assist) when the circumstances require it. Spotting our students and athletes is sometimes necessary in order to teach skills safely, to help athletes perform skills correctly and to prevent injury. I understand and agree that spotting will be part of the learning process at Charlotte Allstars and I agree to permit my child's instructor and/or coach to physically assist my child when needed. Direct assistance will also be provided in the event of injury as needed.
I hold harmless Charlotte Allstar Gymnastics & Cheerleading in the event my child should contract COVID-19.
Gym Policies
Students are only allowed on the floor/equipment if they are with an instructor. All other siblings etc., who are
not in a supervised activity, must be monitored in the appropriate waiting areas.
Also, parents/guardians are not permitted on the gym floor at any time.
Registration Fee and Monthly Fees
Recurring monthly fees will be charged by Charlotte Allstar Gymnastics & Cheerleading by automatic draft on the 1st of every month.
For School Year Classes: There is an annual $40.00 (per school year) non-refundable membership fee per first child, $30.00 per second child and $20.00 per third child. Your first month fees, including the Registration Fee, will be charged upon receipt of registration (pro-rated if after the first week). All subsequent month's fees will be drafted automatically on the 1st of the month.
"Option C" Camp: You will be charged for any Option C Camp blocks when registration is received. You MUST be registered ONE WEEK in advance for our "Option C" Camp.
Insufficient Funds Fee
Charlotte Allstar Gymnastics & Cheerleading will charge a fee of $35.00 should a payment you make comes back with a notice of Insufficient Funds (ie: a bounced check or declined credit card payment).
30-Day Drop Notice
For School year classes: Once your child is enrolled we require a 30-day drop notice to the Charlotte Allstar Gymnastics & Cheerleading office if for any reason the child should need to withdraw from our program (notifying an instructor or any other personnel will not be adequate).
The child's enrollment will end 30 days from when the notice was given and your last monthly payment will reflect the remaining amount of classes in that month. If a notice is not given, you will continue to be billed monthly.
For Team athletes: A 30 day drop notice will be required. Drop notices are to be given to ONLY Murielle Miller at the front desk AND communicated with your Head Coach. Once a drop notice is properly given, you will be drafted on the first of the next month, then no additional drafts will be made.
EX: A notice given on December 30th: You will be drafted on January 1st and then no additional drafts will be made. A notice given on January 2nd: You will be drafted on February 1st and then no additional drafts will be made.
Make-up Policy
Charlotte Allstar Gymnastics & Cheerleading does not offer make up classes for any circumstance including closings due to inclement weather.
